    The Origin, Changes and Compositive Principles of Palmulgunja-tang

    1. Objectives This paper investigates the origin, the progressive changes and the constructive principles of Palmulgunja-tang (八物君子湯). 2. Methods Palmulgunja-tang and other related prescriptions were analyzed in terms of their pathological indications, based on previous literature including Donguisusebowon·Chobongwon (東醫壽世保元·草本卷) , Donguisusebowon·Gabobon (東醫壽世保元·甲午本) , Donguisusebowon·Sinchukbon (東醫壽世保元·辛丑本) and Dongmuyugo (東武遺稿) . 3. Results and Conclusions 1) The Palmulgunja-tang most likely originates from Paljin-tang introduced in Donguisusebowon·Chobongwon (東醫壽世保元·草本卷) . Paljin-tang progressively transformed into Seungyangpalmul-tang ( Donguisusebowon·Gabobon (東醫壽世保元·甲午本) ) and ultimately into Palmulgunja-tang ( Donguisusebowon·Sinchukbon (東醫壽世保元·辛丑本) ), a prescription appropriate for usage in the Ulgwang symptomatology (鬱狂證). Also,Seungyangikgi-tang in Donguisusebowon·Sinchukbon (東醫壽世保元·辛丑本) can be presumed to have been affected by Seungyangpalmul-tang. 2) The variational prescriptions (變方) of Palmulgunja-tang shows increasing Seungyang (升陽) effect in order of Baekhaogunja-tang, Sipjeondaebo-tang, Palmulgunja-tang, and Doksampalmul-tang. 3) Palmulgunja-tang is composed of 8 herbs. Of these, Paeoniae Radix Alba (白芍藥), Glycyrrhizae Radix(甘草),Angelicae gigantis Radix (當歸), and Cnidii Rhizoma (川芎) fortify the Soeumin Spleen Element (脾元). Ginseng Radix (人蔘) and Astragali Radix (黃芪) support the ascension of Yang, whereas Atractlodis Rhizoma White (白朮) and Citrus unshiu (陳皮) encourage the descension of Yin.

    Literature Review on Effectiveness of Pharmacopuncture According to Sasang Constitutional Medicine

    1. Objectives To evaluate effectiveness of one hundred thirty kinds of pharmacopuncture made of herbs used in Sasang Constitutional Medicine. 2. Methods We searched at fifteen kinds of Korean oriental medical journals websites and four kinds of Korean oriental medicine databases. '약침', '수침', 'pharmacopuncture', 'aqua acupuncture' and 'herbal acupuncture' were keywords. Searching period was March to April 2010. 3. Results One hundred eighty three articles are searched. And these are the results. 1) A lot of Soeum herb pharmacopunctures are effective in anti-tumor, brain damage, rheumatoid arthritis. 2) A lot of Soyang herb pharmacopunctures are effective in kidney injury, asthma, rheumatoid arthritis. 3)A lot of Taeeum herb pharmacopunctures are effective in obesity, asthma, anti-tumor, anti-oxidation, rheumatoid arthritis. 4) Taeyang herb pharmacopunctures are effective in rheumatoid arthritis, amyotrophia.. 4. Conclusions There was similar effectiveness between oral administration and pharmacopuncture in Sasang Constitutional herbs. It suggest that pharmacopuncture could be used for Sasang constitutional disease and symptoms. Well-designed clinical studies for Sasang constitutional pharmacopuncture are needed.

    A Feasibility Study of Constitution Discrimination Using a Measurement Device for Dynamic Friction Coefficients of the Back of a Hand

    1. Objectives Our goal is to observe the feasibility of constitution discrimination from computing quantitative roughness index from dynamic friction coefficients and their gradients with the measurement device of skin friction with 3-Axis load cell sensor. 2. Methods In the traditional Korean medicine, skin diagnosis is one of the examination methods to discriminate Sasang constitution since it was known that Tae-eumin has rough skin, and Soyangin has smooth one. It is based on the skin roughness on the back of one's hand for the discrimination. The measurement device of skin friction with 3-axis load cell sensor has been developed in order to provide quantitative skin roughness through dynamic friction coefficients. The effective interval of the coefficients is obtained from the automatic sampling algorithm to use their curvature and slope. Then, Fisher's discriminant function of them makes the discrimination. 3. Results The success rate of extracting the effective interval was about 90% and the discriminant accuracy between Tae-eumin and Soyangin was 70% and 68% for men and women, respectively. The entire methods showed the possibility to distinguish between Tae-eumin and Soyangin by using stochastic properties of roughness index, which can make the entire system to include the measurement, the computation of the roughness index and the discrimination of constitution automatical. 4. Conclusions The measurement device, the automatic sampling algorithm of dynamic friction coefficients and the constitution discrimination algorithm were developed, respectively, and their combination can become the serial and automatic procedure for quantitative and objective skin diagnosis, which mimics the movement of the Oriental medical doctors' skin diagnosis. It can be applied to healthcare as well as the diagnosis of constitution in a u-Health system soon.

    A Study on the Correlation of Mean Velocity of Basilar Artery and Equilibrium Function with Sasang Constitution

    1. Objectives This study was carried out to investigate the equilibrium functional difference according to mean velocity of basilar artery with Sasang Constitution. 2. Methods We analyzed the data of 300 adults(194 males, 106 females), who visited oriental hospital of D university for medical examiation, which includes Sasang constitution examination, equilibrium function test, transcranial doppler ultrasonography(TCD). We evaluated the trace length and envelop area of open eyes and closed eyes, mean velocity of basilar artery and analyzed its statistical data. 3. Results and Conclusions 1) Within all subjects, trace length and envelop area of closed eyes were significantly lower, in the order of increased mean velocity of basilar artery group, normal mean velocity of basilar artery group and decreased mean velocity of basilar artery group. 2) No significantly difference was seen between the mean velocity of basilar artery and equilibrium function in Soyangin and Soeumin. 3) When closed eyes, trace length were significantly lower in order of increased mean velocity of basilar artery group, normal mean velocity of basilar artery group and decreased mean velocity of basilar artery group, in Taeeumin.

    Herbal Extracts Classification and Application in Terms of Sasang Constitution

    1. Objectives Herbal Extracts are used in Korean Oriental Medicine clinics. The sixty eight kinds of single herbal extracts and fifty six kinds of mixed herbal extracts has been using since the first year of execution of Korean medicine national insurance policy in 1987. This paper was performed to make a guideline of using herbal extracts. Single herbal extracts and mixed herbal extracts were discussed and classified according to Sasang constition. 2. Methods The sixty eight single herbal extracts were classified into four categories according to Sasang Constitution and fifty six mixed herbal extracts were classified by six Korean oriental medical doctors. Mixed herbal extracts were considered in terms of drug roles[sovereign, minister, assistant and courier(君臣佐使)] or most included ingredient herb. 3. Results Sasang constitutions which were suitable for fifty six mixed herbal extracts were Soeumin(46.4%), mixed constitution(39.2%) and Soyangin(8.9%), Taeeumin(5.3%). Sasang constitutions which were suitable for sixty eight single herbal extracts were Soeumin(47.1%), Soyangin(32.4%) and Taeeumin(20.6%). 4. Conclusions The rate of Soeumin herbal extracts was the most highest among all constitutions. It is regarded that warming and enforcing herb medicine were most used in all prescriptions.

    An Evaluation of the Statistical Techniques Used in Papers of the Journal of Sasang Constitutional Medicine (2000-2009)

    1. Objectives The purpose of this study is to assess the statistical methods used in the Journal of Sasang Constitutional Medicine and to identify the types of errors in the statistical analysis. 2. Methods We have reviewed quantitative articles published in the Journal of Sasang Constitutional Medicine from 2000 to 2009. Except for the case report, a total of 505 articles was reviewed by year. We focused on the methods of statistics in these articles. We have analysed the number of thesis used the statistical methods, the number of the statistical methods, the types of the statistical methods, and the errors of the statistical methods with the object of 202 thesis used the statistical method. 3. Results 1. Of the 370 articles, 202 used inferential statistics. 2. The most dominant was the thesis with one type of the statistical method forming 61.9% (125 thesis) of the total. 3. The ANOVA was the statistic of choice overall (40.1%), followed by the t-test (35.6%). 4. By examining the errors of the statistical methods, there were 49 thesis with errors among 81 thesis using ANOVA, 25 thesis among 72 thesis using t-test, and 18 over 62 thesis using chi-squared test. 4. Conclusions We have found various mistakes or misuses in the applications of statistical methodologies in the articles published in the Journal of Sasang Constitutional Medicine. It is necessary to present the cases of inappropriate statistical methods in order to improve the quality of academic researches. So long as Sasang constitutional medicine uses the statistical methods, researchers should fully examine and be aware of the statistical methods which they wish to use through other research articles, and statistic publications.

    Anti-inflammatory Activities of Herbal Formulas for Sasang Constitutional Medicine

    1. Objectives 4 herbal formulas (Yanggyeoksanhwa-tang, Yeoldahanso-tang, Cheongsimyeonja-tang and Taeeumjowi-tang) were applied to investigate the anti-inflammatory activities. In many studies, plant-derived anti-inflammatory efficacies have been investigate for their potential inhibitory effects on lipopolysaccharide (LPS)-stimulated macrophages. This study was performed to examine the anti-inflammatory activities of 4 herbal formulas on LPS-stimulated RAW 264.7 cells. 2. Methods The productions of nitric oxide (NO), prostaglandin (PG)E2, interleukin(IL)-6 and tumor necrosis factor (TNF)-α were examined in the presence of the 4 herbal formulas in RAW 264.7 cells. The cells were incubated with LPS 1 μg/mL and 4 herbal formulas for 18 hrs. The anti-inflammatory activity of 4 herbal formulas were investigate by carrageenin-induced paw edema in rats. The paw volume was measured at 2 and 4 hrs following carrageenin- induced paw edema in rats. 3. Results Yanggyeoksanhwa-tang and Cheongsimyeonja-tang showed inhibitory effect on PGE2 production in LPS-stimulated RAW 264.7 cells and a reduction in carrageenin-induced paw edema on rats. Yanggyeoksanhwa-tang showed inhibi- tory effect on IL-6 in LPS-stimulated RAW 264.7 cells. 4 herbal formulas not affect on NO and TNF-α inhibition in LPS-stimulated RAW 264.7 cells. 4. Conclusions These results suggested that Yanggyeoksanhwa-tang and Cheongsimyeonja-tang have anti-inflammatory activity.

    Effect of Hyeongbangdojeok-san on Acute Cocaine-induced Behavioral Effect and Immediate Early Gene Expression in Rats.

    1. Objectives The present study was designed to investigate the effect of Soyangin Hyeongbangdojeok-san(HBDJS) on acute cocaine-induced behavior effect and gene expression in the rat brain. 2. Methods Experimental animals were composed of saline(SAL), cocaine(COC), HBDJS + COC, HBDJS + SAL group. Rats received HBDJS(100, 200 mg/kg, p.o.) 1 h prior to cocaine hydrochloride(20 mg/kg, i.p.) treatment respectively. After cocaine injection, locomotor activity and rearing were measured in a rectangular container equipped with a video camera above the center of the floor for 60 min. In addiction, c-Fos expression in the rat brain was detected using immunohistochemistry 2 h after cocaine injection. And the effect of HBDJS on acute cocaine-induced pERK, pElk, pCREB upstream of c-Fos expression was detected using western blotting and immunohistochemistry 15 min after cocaine challenge. 3. Results The present results show that HBDJS at dose of 200 mg/kg attenuated cocaine-induced both locomotor activity and rearing. Also HBDJS at dose of 200 mg/kg significantly decreased c-Fos expression in the rat brain(nucleus accumebns and striatum). However HBDJS at dose of 200 mg/kg have no effect on cocaine-induced pERK, pCREB, pElK-1 expression. HBDJS is c-Fos expression through ERK-independent pathway. 4. Conclusions. These results suggest that HBDJS may be effective in suppressing the reinforcing effects of cocaine.

    Stimulation of the Immune Response by Yanggyuksanhwa-tang

Objectives Yanggyuksanhwa-tang for Soyangin was applied to investigate the immunological activities on antigen (Ag)-specific or Ag-non-specific immune responses on murine macrophage cell line (RAW 264.7) and ovalbumin/aluminium (OVA/Alum)-immunized mice. 2. Methods This study were carried out in nitric oxide (NO) synthesis on RAW 264.7 cells and cellular proliferation on mouse splenocytes. C57BL/6 mice were immunized intraperitonially with OVA/Alum (100 μg/200 μg) on day 1, 8, and 15. Yanggyuksanhwa-tang was administrated to mice orally for 3 weeks from day 1. On day 22, OVA-, lipopolysaccharide (LPS)-, and concanavalin A (Con A)-stimulated splenocyte proliferation and antibodies (OVA-specific antibodies of the IgG, IgG1, and total IgM classes) in plasma were measured. 3. Results Yanggyuksanhwa-tang significantly enhanced cellular proliferation by LPS and Con A on splenocytes from OVA/Alum-immunized mice (p<.001). Yanggyuksanhwa-tang also significantly enhanced plasma OVA-specific IgG (p<.001), IgG1 (p<.001), and total IgM (p<.01) levels compared with the OVA/Alum group. 4. Conclusions These results suggested that Yanggyuksanhwa-tang for Soyangin could be used as immunopotent.

    Effects of Suhwagije-tang Distillate on Serum and Testosterone in Aging Rats

    1. Objectives The purpose of this study is to investigate the effects of oral administration of distilled Suhwagije-tang in aging rats. 2. Methods The SD rats used in this experiment were 6, 48 and 68 weeks old. These groups were treated with distilled water(DW), 1% Vitamin C(Vit. C) and 50% Suhwagije-tang(SGT) distillate respectively, once a day for 4 weeks. Blood samples were taken from heart and the levels of various parameters in serum were analysed. 3. Results and Conclusions (1) At all concentration of SGT, survival rates of liver cells were higher than the control group. (2) The rate of which the 10W-SGT group gained weight was significantly higher than that of the control groups. (3) The levels of albumin in serum of 10W-SGT group were significantly increased in comparison to those of the DW group. The levels of ALP, total cholesterol and HDL-cholesterol in serum of 52W-SGT group were significantly decreased in comparison to those of the DW group. (4) The levels of testosterone in serum of SGT groups showed no significantly changes in comparison to those of the control groups. Testosterone levels were naturally decreased with advancing years, so 72W-DW group's testosterone level was significantly decreased in comparison to those of 10W-DW group's. In contrast, 72W-SGT group's level was the highest in comparison to those of the control groups and those of deviation from 10W, 52W and 72W in SGT groups was also smaller than control groups'. (5) The levels of glucose, total-bilirubin, ALT, AST, BUN, CRP, hs-CRP, homocystein, Na, K, Na/K ratio, Cl and Ca in serum of SGT groups showed no significantly changes in comparison to those of the control groups. These results suggest that oral administration of Suhwagije-tang distillate should be effective in promoting growth and preventing rapid decline of testosterone in aging rats does not cause any imbalance in liver, kidney and electrolyte.

    The Clinical Study on 1 Case of Soeumin Patient Diagnosed as Grave's disease, Treated with Palmulgunja-tang and Western Medicine

    1. Objectives The purpose of this case is to report that Soeumin patient diagnosed as Grave's disease was treated with Palmulgunja-tang and antithyroid drugs in combination. 2. Methods From April 7, 2010 to June 30, 2010, we treated him with Palmulgunja-tang and antithyroid drugs. We took his weight, pulse and checked visual analog scale(VAS) to evaluate the symptoms of Grave's disease. 3. Results After treatment, this patient's thyroid function test were improved faster than with western medicine treatment only in 2009. The symptoms also were improved. 4. Conclusions This case study showed an efficient results by giving Sasang Constitutional treatment in treatment of Grave's disease.

    A Case Report of Soeumin Atopic Dermatitis Patient Improved by Gwakhyangjeonggi-san

    1. Objectives This study is about a Soeumin Atopic Dermatitis Patient. In this study, we report the effects of Sasang constitutional treatment to this patient. 2. Methods This patient was treated by Soeumin's constitutional medications and acupunture according to the result of Sasang constitutional diagnosis. 3. Results and Conclusions This patient's chief complaints were edema, palule, erythema, squama and pruritus. By using Gwakhyangjeonggi-san, she showed positive response about her symptom. This study shows that Gwakhyangjeonggi-san has effect to Atopic Dermatitis in Soeumin.

    Three Cases of Cerebellar Infarction Patients with Dizziness and Ataxia who were Evaluated using the Equilibrium Function Test

    1. Objectives This study is about three cerebellar infarction patients with dizziness and ataxia. In this study, we evaluated the effects of Sasang constitutional treatment and suggested using the 'Equilibrium Function Test' as scale. 2. Methods These three patients were treated by Sasang constitutional herbal medications based on 『Donguisusebowon』. Acupuncture & moxa therapy and west medications were limited as much as possible. We evaluated the symptoms through two methods, the Visual Analogue Scale and the Equilibrium Function Test. 3. Results and Conclusions Dizziness and ataxia that were these three patients` chief complaints were improved. This case study shows that Sasang constitutional herbal medications are an effective treatment for cerebellar infarction patients.

Objectives The purpose of this case is to report the effects of Sibimijihwang-tang(十二味地黃湯) which is based on Sasang constitutional Medicine for Yin-Deficit Diurnal-Heat Symptomatology(陰虛午熱證) in Soyangin. 2. Methods We treated a Soyangin patients that had Fever Unknow Origin. We prescribed Sibimijihwang-tang(十二味地黃湯) for her physical symptoms. The improvement of her diseases was evaluated by her fever and other physical symptoms. 3. Results After the Sasang constitutional medication of Sibimijihwang-tang(十二味地黃湯) was given, the patient's fever and physical symptoms were improved. 4. Conclusions This case study show an efficient result of using Sibimijihwang-tang(十二味地黃湯) in the Yin-Deficit Diurnal-Heat Symptomatology(陰虛午熱證) of Soyangin.

    Two Cases of Psoriasis treated with Sasang Constitutional Therapy and Fasting Therapy

    1. Objectives The purpose of this study was to give an account of the effects of Sasang constitutional medicine and fasting therapy for the patient who has suffered from various signs or symptoms caused by psoriasis. 2. Methods The cases were one 50-year old man diagnosed as Taeeumin and one 29-year old man diagnosed as Soyangin. They all have the symptoms caused by psoriasis, such as rash, erythema, scaly skin, itching, etc. So we've recorded the progress of their symptoms as a score at the tables to check their conditions throughout the hospitalized period. We've diagnosed them as Soyangin, Taeeumin have prescribed Yangdokbaecho-tang and Galgeunhaegi-tang and have carried out fasting therapy. 3. Results and Conclusions The general condition of the patient have improved incredibly, where the each scores at the tables dropped from 4 to 0,1 and their symptoms like rash, erythema, scaly skin, itching, edema have almost disappeared at the end of the hospitalization period. This indicates that Yangdokbaecho-tang and Galgeunhaegi-tang can be used to treat main symptoms of psoriasis and fasting therapy can be used as a subsidiary therapy for the psoriasis.
