    Tasks for Insuring the Composite Herbal Preparations of Sasang Constitutional Medicine in the National Health Insurance

    Reforming the insurance scheme for herbal drugs in the National Health Insurance is a long-cherished desire for Korean Medicine doctors. Because most Korean Medicine doctors distrust the quality of existing insured herbal drugs,which are powdered mixes of each herbal extract, the use and the expenditure of insured herbal drugs have been decreased in the last ten years. To address this, it has been demanded to insure the composite type of herbal preparation, which is the extracted powder of the whole prescription, to the benefit coverage for herbal drugs. Many stake holders, however, have so far been unable to reach an agreement on this. In this situation, Sasang Constitutional prescriptions are expected to make a breach of insuring the composite herbal preparations, because some of them were approved as prescription drugs in 1999. In this review, I discussed the problems of insured herbal drugs, the necessity of insuring the composite herbal preparations and Sasang Constitutional prescriptions, and the tasks of Sasang Constitutional Society to insure them.

    Insurance Coverage on the Sasang Constitutional Herbal Medicine Extracts

    1. Objectives Sixty eight kinds of single herbal medicine extracts and fifty six kinds of mixed herbal medicine extracts have been used under a national health insurance since 1987. The number of herbal medicine extracts under an insurance coverage remains unchanged. The demand for covering complex herbal medicine extracts and Sasang Constitutional medicine extracts in a national health insurance increases. This study is to investigate the needs for the insurance coverage. 2. Methods Advantages and disadvantages between herbal medicine decoction and extracts were explained and Sasang Constitutional medicine extracts according to Sasang constitution were surveyed in production nowadays. 3. Results Herbal medicine decoction has many advantages of treating patients but herbal medicine extracts have advantages of the rapid adminstration and being easy to carry. From eleven to twenty two products of Sasang Constitutional medicine extracts have been in production in several factories. There are eleven kinds of Soyangin medicine extracts,nine kinds of Taeeumin medicine extracts and five kinds of Soeumin medicine extracts. 4. Conclusions Complex herbal medicine extracts and Sasang Constitutional medicine extracts have to be in use under a national health insurance as soon as possible

    A discussion on the philosophical associations between Dongmu (東武) and Nosa (蘆沙): an approach based on the Li-Qi (理-氣) theory

Objectives This study reviews the general theory on Li-Qi (理-氣) found in Dongmu Lee Jema (東武 李濟馬) and Nosa Gi Jeongjin (蘆沙 奇正鎭)'s works and explores the associations between their philosophical systems. 2. Methods The main ideas on Predisposition (氣稟論) found in Dongmu's works were explored in connection with the Li-Qi (理-氣) theory and compared with the perspectives suggested by Nosa. 3. Results and Conclusions 1) Nosa criticized the “weakened supervision of Li (理)” and the “separation of Li (理) and Qi (氣)”, proposing that this problem can be overcome through “mutual embracement of Li and its manifestations (理分圓融 理分相涵)”. 2) When Dongmu's theory on Predisposition (氣稟論) is interpreted in terms of Li-Qi (理-氣), the Seong-Li (laws governing the organ scheme) (性理(臟理)) represents the Li (理) while the formational variations in organ scheme (臟局短長) and and the level of self-cultivation (心地淸濁) represents the Qi (氣). 3) The concept of “Diversification of Li (理之異)”, bearing similarities to Nosa's Li-Qi (理-氣) theory, was introduced for the first time in Dongmu's theory on Predisposition (氣稟論), which presumably built the fundamental theories in the Sasang Constitutional Typology. 4) Within Dongmu's theory of Predisposition (氣稟論) can be found a dichotomous division of “similitude between the morally unaccomplished and the morally accomplished” in the ontological plane and “dissimilitude between the morally unaccomplished and the morally accomplished” in the axiological plane; this dichotomy is more extensive and developed compared to Nosa's attempt to find a consistent logic in both the ontological and axiological plane through a Li-Qi (理-氣) structure with Li-predominance.

    1. Objectives This research was proposed to reinterpret 「Soeumin Bumron(少陰人 泛論)」through development of Soeumin exterior disease in Sasang Constitutional Medicine(SCM). 2. Methods We chronically compared the conceptions, symptoms, mechanisms and treatments of Soeumin exterior disease from 『Donguisusebowon-GabObon(東醫壽世保元甲午本)』(DGO) to 『Donguisusebowon-ShinChukbon(東醫壽世保元辛丑本)』(DSC)3. Results and Conclusions (1) The classification standard of Soeumin exterior disease was different in DGO and DSC. (2) Soeumin exterior disease classified Taeyang Disease(太陽病), Yangmyoung Disease(陽明病) in DGO and Ulkwang-syndrome(鬱狂證), Mangyang-syndrome(亡陽證) in DSC. (3) Based on the changes of understanding about Soeumin exterior disease from DGO to DSC, we attempted to reinterpret 「Soeumin Bumron(少陰人 泛論)」 in view of DSC.     Study on the Development of Diagnosis Algorithm of Soeumin Symptomology

Objectives This study is aimed to develop the algorithm, which can help clinicians diagnose Soeumin's symptomology, based on the indexes such as dry mouth, water drinking, sweat, urine, appetite, digestion, and stool, etc. 2. Methods This research analyzes the items of 『Donguisusebowon(東醫壽世保元)』 to reveal the inevitable and sequential indexes of Soeumin's symptomology diagnosis, in order of exterior-interior pattern differentiation, favorable-unfavorable pattern differentiation, and mild-severe-dangerous-urgent pattern differentiation. 3. Results and Conclusions 1) 1st step: Soeumin's exterior pattern and interior pattern are differentiated in terms of heat and cold, respectively. Stool and digestion are used to confirm the difference. 2) 2nd step: The existence of sweat is used to find out that an exterior pattern is with or without favor, while the indexes of stool, dry mouth, and generalized pain are used to identify an interior pattern with or without favor. 3) 3rd step: The favorably exterior-heat pattern can be either mild or severe by the indexes of cold-heat, stool,tidal fever, and manic raving, panting and straight looking, while the unfavorably exterior-heat pattern can be either dangerous or urgent by the ones of cold-heat, stool, and urine. And, the favorably interior-cold pattern can be either mild or severe by the indexes of stuffiness and fullness, jaundice, and edema, while the unfavorably interior-cold pattern can be either mild or severe by the ones of vexation level.

    The Study on the Mangeum-syndrome(亡陰證) of Soyangin in Sasang Constitutional Medicine (SCM)

Objectives This research was proposed to find out how the recognition on Mangeum-syndrome(亡陰證) of Soyangin in Sasang Constitutional Medicine(SCM) evolved from 『Donguisusebowon-GabObon(東醫壽世保元甲午本)』(DGO) to 『Donguisusebowon-ShinChukbon(東醫壽世保元辛丑本)』(DSC). 2. Methods We compared 『Donguisusebowon-GabObon(東醫壽世保元甲午本)』(DGO) with 『Donguisusebowon-ShinChukbon (東醫壽世保元辛丑本)』(DSC) to know how the symptom, mechanism, treatment principle and treatment methods on Mangeum-syndrome(亡陰證) of Soyangin in Sasang Constitutional Medicine(SCM). 3. Results and Conclusions (1) The way to categorize the Exterior Disease and Interior Disease was different between DSC and DGO. (2) There was only a fundamental mechanism about the Boktongseolsa-syndrome(gastralgia and diarrhea) and no treatment principles and treatment methods about it in DGO. But the Boktongseolsa-syndrome was evolved into Mangeum-syndrome(亡陰證) in DSC with a detailed mechanism and treatment principles of it. (3) The evolution of Mangeum-syndrome(亡陰證) in DSC from Boktongseolsa-syndrome in DGO was attributed to the experience of treatment on Seomeo-syndrome(譫語證).

    A Study on the Therapeutic Herbal Medications for Soyangin Mangeum Symptomatology

    1. Background This is the first documented study on the four medications for the Mangeum symptomatology (亡陰證) in the Soyangin disease category. The four medications, Hyeongbangsabaek-san (荊防瀉白散), Jeoryeongchajeonja-tang (猪苓車前子湯), Hyeongbangjihwang-tang (荊防地黃湯), and Hwalseokgosam-tang (滑石苦蔘湯), were comparatively analyzed on their compositive principles and respective symptomatologies. 2. Methods Previous literature, including classical works on earlier Sasang Constitutional Medicine (different editions of Donguisusebowon and Dongmuyugo), were reviewed for relevant material on the Mangeum medications. 3. Results and Conclusions 1) Hyeongbangsabaek-san (荊防瀉白散) is used in pathologies of early stages of invasion of the internal heat in the lower-most burn-center (locus related to the Kidney system). 2) Jeoryeongchajeonja-tang (猪苓車前子湯) is used when pathogenic dampness-heat invades the lower-most burn-center (locus related to the Kidney system). 3) Hyeongbangjihwang-tang (荊防地黃湯) is used to treat deficient heat involving injury of the lower-most burn-center (locus related to the Kidney system). 4) Hwalseokgosam-tang (滑石苦蔘湯) is used when the lower-most burn-center (locus related to the Kidney system) is in the early stages of exhaustion, expecially accompanied by concomitant dampness-heat. 5) The Mangeum symptomatology (亡陰證) was fully understood only after the completion of the 1901 Sinchuk edition (辛丑本) of Donguisusebowon (東醫壽世保元).

    A study on the schematic organization of the sub-classification system of the Taeeumin symptomatology

Objectives We aimed to propose a sub-classification system for the Taeeumin symptomatology by examining the Taeeumin pathology and symptomatology descriptions appearing in 『Donguisusebowon』. 2. Methods The Gabo Edition and the Sinchuk Edition (the upgraded and revised edition) of 『Donguisusebowon』 were reviewed and examined for relevant information on the Taeeum pathology and symptomatology. 3. Results and Conclusions 1) In the Taeeumin symptomatology, the Exterior disease develops from the basic pathology of Esophagus-Cold and the Interior disease from that of Liver-Heat, eventually progressing to damage of the expirational and dispersive energy of the Lung Sector, the Prime Core Organ or the excessively small organ of the Taeeum constitutional type. The resulting pathology can be broadly defined as the “Lung-Dryness symptomatology”. 2) The case reports introduced in the Exterior disease section, including the Zhang Zhongjing Mahuang-tang treatment,Prolonged-affliction disease treatment, and Exterior disease Pestilential disease treatment, share several points in common. They all arise from the pathology of “weakness in the Lung sector and deficiency in the Exterior sector”, and they can all be assigned to the same symptomatological division that presents with systemic heat and cold intolerance; this symptomatology can be defined as the “Esophagus-Cold symptomatology”, the milder subdivision of the exterior symptomatology. 3) The body of text appearing in the last part of the Interior disease section commonly referred to as the “Taeeumin Conspectus”is in fact not a conspectus when its contents are actually examined. Instead, it can be understood from its pathological and symptomatological descriptions that the passage is explaining the more severe subdivision of the exterior symptomatology that has progressed from Esophagus-Cold to a pathology characterized by damaged expirational and dipersive energy of the Lung Sector. 4) The relocation of the “dry-related pathology” indicates a change in perspective regarding the “Dry-related symptomatology”,which caused the rearrangement of the Interior disease into divisions of Liver-Heat symptomatology that is characterized by fulminant heat pathology and Dry-Heat symptomatology that is also accompanied by Lung-Dryness. 5) The Interior disease Yin-Blood Consumptive symptomatology should be included in the Dry-Heat symptomatology in the pathological scheme. 6) Based on the above, the subdivisions of the Taeeumin symptomatology should be arranged as “Esophagus-Cold symptomatology”and “Lung-Dry-Cold symptomatology” in the Exterior disease and “Liver-Heat symptomatology” and “Dry-Heat symptomatology” in the Interior disease.

    Literature Review on Safety of Pharmacopunctur

    1. Objectives Pharmacopuncture according to Sasang Constitutional Medicine is new field of constitutional medicine. We investigated safety of pharmacopunctures to develop this treatment. 2. Methods We searched for papers in fifteen Korean oriental medical journals and four online searching portal articles. Hepatotoxicity,nephrotoxicity and general toxicity tests were investigated about one hundred thirty kinds of pharmacopunctures. 3. Results Toxicity test about 36 kinds of 130 pharmacopunctures were investigated. 1) Hepatotoxicity and nephrotoxicity test of pharmacopunctures were searched in 11 of 38 Soeumin herbs, 8 of 36 Soyangin herbs, 5 of 44 Taeeumin herbs and 1 of 12 Taeyangin herbs. From among these herbs, toxicity was detected in Radix aconiti lateralis preparata(附子), Fructus alpiniae oxyphyllae(益智仁), Semen crotonis(巴豆), Radix bupleuri(柴胡) and Semen armeniacae amarum(杏仁) pharmacopunctures. 2) General toxicity test of pharmacopunctures were searched in 8 of 38 Soeumin herbs, 9 of 36 Soyangin herbs,6 of 44 Taeeumin herbs and 2 of 12 Taeyangin herbs. from among thes herbs, toxicity was detected in Radix aconiti lateralis preparata(附子), Radix ginseng(人蔘), Semen crotonis(巴豆), Rhizoma seu radix notopterygii(羌活), Radix aralia cordatae(獨活), Semen armeniacae amarum(杏仁) and Radix scutellariae(黃芩) and Cortex acanthopanacis(五加皮) pharmacopunctures. 4. Conclusions From these results, we must decide extraction method, concentration and dose carefully to use the toxic pharmacopuncture. More toxicity test and further study is needed about pharmacopuncture treatment in Sasang Constitutional Medicine.

    The Using Status and Effectiveness of Sasang Constitutional Herbal Prescription

    1. Objectives We aimed to derive using status of Sasang Constitutional herbal prescription and good effective one in specific disease. 2. Methods We sent Questionnaire to 463 oriental medical doctor listed Korea Sasang Constitutional medicine from 2009years. 10 to 11 and got back 191 questionnaire, which consisted of sex, age, clinical duration, working type, using status and effectiveness of Sasang Constitutional herbal prescription. 3. Results - Digestive tract disease was especially most effective field, and circulatory, the endocrine, dermatology, psychopathy,urogenital disease went in order in western disease. - Spleen disease was especially most effective field, and the heart and circulatory, neuropsychiatry, lung, liver,psychopathy, gynecology disease went in order in oriental medicine pathology. - Chungshimyenja-tang was effective to neuropsychiatry and heart disease in Taeeumin, Hyangsayangwi-tang to disease related digestive system, Palmulgunja-tang to gynecology disease in Soeumin, Dockhalgiwhang-tang to diet disorder, Yanggeuksanwha-tang to heart and psychologic disease in Soyangin. 4. Conclusions Constitutional prescription effected on special pathological symptom support weaken organ, and these result will help to make treatment standard and support clinical study of constitutional medicine.

    A Study on Job and Stress of Staffs in Clinical Research of Oriental Medicine

Objectives In this research, job and stress of the staffs who is working on clinical trial of oriental medicine involved in the project of Korea Institute of Oriental Medicine (KIOM) was identified. And an efficient way to enhance the working ability of clinical trial of oriental medicine was developed by recognizing the relationship of job satisfaction, organizational commitment and job performance with their stress. 2. Methods A survey was conducted on 18 staffs in 12 hospitals who is working in the project of The construction of constitutional information collecting system for the scientification of Sasang Constitution. The collected data was analyzed with computer software of SPSS 17.03. Results The research results are as follows: 1. The staffs of clinical trial of oriental medicine responded that they want to receive the rewards for their work with a salary, and they felt that through clinical demonstration followed by SOP and front face photograph are the most difficult works in clinical trial of oriental medicine. 2. Permanent workers showed higher organizational commitment than temporary workers in the relationship of job satisfaction, organizational commitment and job performance with general characteristics(p=0.026). 3. In the relationship of job stress with general characteristics, the workers who are higher than college graduates had dissatisfaction in salary with the highest frequency(p=.004), and the workers whose monthly salary is higher than 2 millions won showed the lowest job stress(p=.021). 4. In the relationship of job satisfaction, organizational commitment and job performance with job stress, as job apprehension stress is increased, job performance is dramatically decreased(p=.027). And as improper salary stress is increased, job satisfaction and organizational commitment is dramatically decreased (p=.018, p=.050). 4. Conclusions It was clearly evidenced that job satisfaction, organizational commitment and job performance is closely related with stress of the staffs who is working on clinical trial of oriental medicine. And it is highly recommended that the improvement of working condition and the decrease of job stress can enhance the working ability of them.

    Relations between the Questionnaire and Health State in each Sasang Constitution based on the Oriental medical doctors󰡑 diagnoses

    1. Objectives We report the relation between the questionnaire and the health state on the Sasang Constitution. 2. Methods We carried out a clinical test, in which 2 Oriental medical doctors participated to diagnose health state in each Sasang Constitution with 239 subjects. The subjects were divided into 3 subgroups, which are allocated to healthy group, poor health group and disease group. 3. Results We extracted 4 questions for Taeeumin, 5 questions for Soyangin and 3 questions for Soeumin from the whole 98 questions respectively. Soyangin and Taeeumin share a same question that shows significant difference by health status. For the statistical analysis, we used a Chi-square test. As a result, we found out that Taeeumin showed different distributions of answers in direct/indirect characteristic,perspiration, frequent urination, regular defecation. According to the health status Soyangin showed the different distribution of the answers in color of urination, frequent urination, mouth dryness, abnormal perspiration, discomfort of head. Soeumin showed the different distribution of the answers in cold and chilly feel of abdomen, digestion status and cause of digestion problems when body condition gets worse. 4. Conclusions This is the first work which reports the relatedness between Questionnaire and Health State in each Sasang Constitutions by an objective clinical test. We found out that several questions have different distributions by health status in each Sasang Constitution. These extracted questions are categorized not only by Wansilmoobyung(완실무병),but also by other symptoms and body status.

    Potential Ability of Traditional Korean Medical Tourism and Economic Valuation of Sasang Constitutional Typology Diagnosis from Foreigner's Perspective

    1. Objectives : This study aimed to provide practical insights on the development of traditional Korean medical tourism and maximize the satisfaction of foreign patients. The main objectives of this study were also to evaluate Sasang Constitutional Medicine(S.C.M.) diagnosis cost from foreigners’ perspectives. 2. Methods : For this study, the data were collected from tourists to Korea between January 3, 2010 to February 20,2010. The sample used in this study was mainly composed of Japanese, Chines and US/Canada, because they are the major national visitor origin group to Korea. The primary data were collected by questionnaire survey using a standardized instrument with a convenience sampling procedure, and efforts of the interviews involved an on-site, self-administrated questionnaire to those sitting or visiting tourist attractions. The total size of the sample was 276 of which 275 showed no missing value against the factors needed for final analysis and were for analysis as an effective sample. SPSS 13.0for window was used to analyze the collected data on which descriptive statistics and the Limited Dependent(LIMDEP)8.0 program was used in estimating the willingness-to-pay(WTP) for traditional Korean medical tourism through the Contingent Valuation Method(CVM). A dichotomous choice question form of a CVM was applied to estimating the value. 3. Results and Conclusion 1. There was significant statistical relationship between attributes when considering medical tourism destination and socio-demographic profiles such as age group, nationality, educational level and occupation. 2. In the level of recognition on Korean medicine, north Americans showed low level of recognition compared to other nationalities. In addition, in terms of age group, 20's and 30's showed low level of recognition on Korean medicine,3. In relation to the level of interest on Korean medicine, Japanese respondents had higher interesting level on Korean medicine. Also singles had higher level of interest. 4. Regarding preferred Korean medical treatment program, there found significant statistical relationship between Korean medical treatment program and socio-demographic characteristics. 5. The estimating result of the logit model showed that the variables affecting the WTP for Sasang Constitutional medicine diagnosis were offered price and potential ability of medical tourism destination. 6. In relation to WTP for Sasang Constitutional medicine diagnosis, Japanese respondents showed a WTP of USD 62.69 and Chinese respondents showed USD 57.09. On the other hands, respondents from north America and other countries showed a WTP of USD 65.50. In conclusion, from this study, the results found that the opportunities in tradition Korean medical tourism are immense and the possible rewards are numerous. It is time to continue to promote ‘Sasang Constitutional medicine’ and make the Korean medical tourism programs considering on nationalities, age group, gender and WTP.

    A Case Report of Soeumin Diagnosed to Pleural Effusion

Objectives This purpose of this case is to report that a soeumin patient diagnosed as pleural effusion was treated with Sibimiguanjung-tang and then her symptoms and result of chest X-ray and R-lab improved 2. Methods We diagnosed the patient as Soeumin-Taeum-byung(少陰人太陰病). So we treated her with Sibimiguanjung-tang 3. Results After we treated the patient with Sibimiguanjung-tang, her symptoms and result of chest X-ray and R-lab improved 4. Conclusions This case study show an efficient result of using Sibimiguanjung-tang in Soeumin diagnosed to pleural effusion

    A Case Study of Taeumin Patient with Gastro-esophageal reflux disease(GERD) who Treated Successfully with Yeoldahanso-tang(Reduohanshao-tang)

Objectives This case study reports a Taeumin patient with Gastro-esophageal reflux disease who had suffered from chest pain,heartburn, acid regurgitation and dyspepsia who improved after Yeoldahanso-tang(Reduohanshao-tang) medication. 2. Methods This patient had started treatment with Yeoldahanso-tang(Reduohanshao-tang), three times per day for three months and assessed the changes of the main symptoms such as chest pain, heartburn and acid regurgitation from baseline to post-treatment using a questionnaire with visual analogue scale(VAS). Electrogastrography(EGG) was also performed for assessment of gastric function from baseline to post-treatment. 3. Results After the treatment, symptoms of GERD such as chest pain, heartburn, and acid regurgitation are almost disappeared. The patient don't need to take the proton pump inhibitor therapy. There was significant increase of % bradygastria parameters in fed EEG after treatment with Yeoldahansotang(Reduohanshao-tang). And significant increase of the power ratio after treatment was shown (20.5), compared with initial EGG(0.0). 4. Conclusions This results show Yeoldahanso-tang(Reduohanshao-tang) can be used to treat GERD with Dry-heat symptom of Taeumin. This results suggest that Sasang Constitutional treatment for GERD patients who has a chronic condition with highly recurrence rate, might be a candidate for a therapeutic agent . Also effects of Yeoldahanso-tang(Reduohanshaotang)on GERD need further studies such as clinical trials.
