    A Study on the Soeumin’s Watery Diarrhea with Little Residue

    Objectives: The purpose of this study is to examine the meaning of the Soeumin’s Watery diarrhea with little residue(下利淸水). Methods: We examined the Watery diarrhea with little residue and Daeseunggi-tang(大承氣湯) in 『Sanghanlun(傷寒論)』, and considered the quotation and the treatment of the Watery diarrhea with little residue in 『Donguisusebowon(東醫壽世保 元)』. Afterward, we considered the meaning of the Soeumin’s Watery diarrhea with little residue by comparing the view of Junggyeong Jang and Jema Lee. Results: and Conclusion 1. Jaricheongsu Saeksuncheong(自利淸水 色純靑) is an expression about the aspect of the Watery diarrhea in 『Sanghanlun』. Especially, Saeksuncheong(色純靑) means there is no residue in Watery diarrhea, and is an expression to distinguish from Haricheonggok(下利淸穀). 2. Haricheongsu(下利淸水) is the Watery diarrhea with little residue in 『Donguisusebowon』. It is caused by the coldness of the Soeumin and should be understood separately from Harihwangsu(下利黃水). 3. Considering the characteristics of the Soeumin, Jema Lee divided the aspects of the Watery diarrhea according to turbidity. Then, he suggested Gwangyebujaijung-tang(官桂附子理中湯) in the warm method(溫法), Seed of Crotonis Fructus(巴 豆) in the diarrhea method(下法), and Gangchulgwanjung-tang(薑朮寬中湯) in the warm method after using the diarrhea method as the treatment of the Watery diarrhea with little residue, and aimed to treat the coldness of the Soeumin.

    Predictive Models for Sasang Constitution Types Using Genetic Factors

    Objectives: Genome-wide association studies(GWAS) is a useful method to identify genetic associations for various phenotypes. The purpose of this study was to develop predictive models for Sasang constitution types using genetic factors. Methods: The genotypes of the 1,999 subjects was performed using Axiom Precision Medicine Research Array (PMRA) by Life Technologies. All participants were prescribed Sasang Constitution-specific herbal remedies for the treatment, and showed improvement of original symptoms as confirmed by Korean medicine doctor. The genotypes were imputed by using the IMPUTE program. Association analysis was conducted using a logistic regression model to discover Single Nucleotide Polymorphism (SNP), adjusting for age, sex, and BMI. Results & Conclusions: We developed models to predict Korean medicine constitution types using identified genectic factors and sex, age, BMI using Random Forest (RF), Support Vector Machine (SVM), and Neural Network (NN). Each maximum Area Under the Curve (AUC) of Teaeum, Soeum, Soyang is 0.894, 0.868, 0.767, respectively. Each AUC of the models increased by 6~17% more than that of models except for genetic factors. By developing the predictive models, we confirmed usefulness of genetic factors related with types. It demonstrates a mechanism for more accurate prediction through genetic factors related with type.

    Research Trends in Articles of Sasang Constitutions Studies on Children and Adolescents

    Objectives: The aim of this study was to review the current Sasang Constitutions studies on children and adolescents and to provide basic data for developing in a better direction of research. Methods: Research articles on children and adolescents of the Sasang Constitutions studies were extracted from six databases. Published year, published journals, types of studies were classified. Subjects of research and clinical methods for the Sasang type classification were summarized, especially in clinical studies. Results: A total of 26 selected research articles were published from 1990 to 2013 and 14 articles (54%) have been published in Journal of Sasang Constitutional Medicine and Journal of Pediatrics of Korean Medicine. 96% of the total researches were based on clinical studies. Psychological and physical characteristics by Sasang Constitutional type accounted for most of the clinical research studies subjects. But these studies were not performed with diagnostic tools only for children and adolescents. Discussion: In this study, the necessity of developing methods of Sasang constitutional type classification for children and adolescents was confirmed. If accompanied by additional research based on this study, it is expected to be used in diverse fields with active collaboration or education.

    Comparative Study on Frequent Disease Patterns and Prescriptions by Three Societies of Korean Medicine for Gastrointestinal Disease

    Objectives: The aim of this study is to analyze the therapeutic direction and characteristics of gastrointestinal diseases by three societies of Korean medicine by comparing the frequent disease patterns and prescriptions of them. Methods: The experts of the three societies of Sasang constitutional medicine, Hyungsang medicine and Sanghan-GeumGwe medicine discussed and confirmed a list of clinical data to make use for diagnosis. Then, clinical data on 200 patients suffering from gastrointestinal symptoms were collected. The each experts diagnosed appropriate disease patterns and decided prescriptions according to the method of their affiliated society. Results: and Conclusions The three different societies’ experts showed the similarity in the sense of categorizing disease patterns focusing on the specific physiology and pathology of the patients, while the each societies have different diagnosis systems. Treating chronic gastrointestinal patients, they treated the mental or psychological symptoms of the patients as well as their physical problems. Based on this study, it is necessary to develop a systematic and precise research model to objectively and statistically analyze the correlation of disease patterns and prescriptions by different societies.

    The Effects of Taeeumjowi-tang Extract Granule on Metabolic Syndrome Risk Factors with Obesity: A Single Group, Prospective, Multi-Center Trial

    Objective: The purpose of this study is to study the effects of Taeeumjowi-tang extract granule on metabolic syndrome with obesity in Taeeumin. Methods: We studied 206 subjects who diagnosed metabolic syndrome with obesity and classified Taeeumin by a Sasang constitution analysis tool. The subjects were administered by Taeeumjowi-tang for 12 weeks. We investigated the changes of body mass index, abdominal circumference, the number of risk factors for metabolic syndrome, lipid, fasting blood sugar, Hemoglobin A1c, free fatty acid, insulin, and blood pressure. Results: Body mass index and abdominal circumference, systolic and diastolic blood pressure were significantly reduced after 4 weeks, 8 weeks, and 12 weeks of administration compared to before administration. Triglyceride significantly decreased after 8 weeks and 12 weeks of dosing compared to before taking. Low density lipoprotein cholesterol, fasting blood sugar were significantly increased after 8 weeks of administration compared to before administration. High density lipoprotein cholessterol was decreased significantly after 4 weeks of administration compared to before administration. The number of risk factors for metabolic syndrome, Hemoglobin A1c increased significantly after 12 weeks of administration compared to screening. Conclusions: In this study we knew that Taeeumjowi-tang extract granule is very efficient in metabolic syndrome, especially in obesity and hypertension, triglyceride, insulin resistance.

    A Case of Panic Disorder of Soeumin Concurrently Treated with Psychotherapy Based on Sasang Constitutional Medicine

    Objectives: The purpose of this case study was to report that a patient with Panic Disorder concurrently treated with psychotherapy based on Sasang Constitutional Medicine was able to discontinue the drugs. Methods: The patient was diagnosed as Soeumin and consulted according to her Sasang constitution while being administered with herbal medicines. The psychotherapy applied in the consultation was to reduce excessive expectations towards her family members or situations. Results: and Conclusions Anxiety of the patient was alleviated and the patient was able to discontinue the drugs during the treatment period. And no panic attack or adverse effect of the discontinuation of the drugs was observed during the rest of the time of 7 months and 4 months later after the termination of the treatment.

    A Case-Report of Soyangin with Acute Urinary Retention and Lower Urinary Tract Symptoms Treated by Yukmijihwang-tanggagambang

    We report 1 case about Yukmijihwang-tanggagambang that was considered by Sasang constitution that was used for an elderly Soyangin patient with acute urinary retention, lower urinary tract symptoms and other symptoms. We treated an elderly Soyangin patient who suffered from acute urinary retention, lower urinary tract symptoms and other symptoms, who had history of benign prostatic hyperplasia. We considered his Sasang constitution and prescribed Yukmijihwnag-tanggagambang mainly. The improvement of his symptoms were evaluated by presence of symptoms, times of urination and visual analogue scale. After treatment that was been Yukmijihwang-tanggagambang mainly, patient’s symptoms were disappeared or improved. And there was not any adverse effect. This study shows that using Yukmijihwang-tanggagambang that was considered by Sasang constitution can be effective treatment for an elderly Soyangin patient’s acute urinary retention, lower urinary tract symptoms and other symptoms.

    A Case Report of Patient with Recurrent Aphthous Stomatitis and Pompholyx Treated with Modified Palmulgunja-tang

    Introduction: This study is the first case report to observe a significant improvement of clinical symptoms of both Recurrent Aphthous Stomatitis(RAS) and pompholyx treating with modified Palmulgunja-tang for Soeum-type. Case presentation: A 37-year-old female patient complained of pain by ulcers on her mouth and tongue, and of itching caused by vesicles on her palms and soles was diagnosed with RAS, pompholyx, and, especially moderate pattern of ‘Congestive Hyperpsychotic symptomatology’ according to the theory of Sasang Constitutional Medicine(SCM). During inpatient and outpatient treatment, she took modified Palmulgunja-tang, and was treated with acupuncture, moxibustion, and cupping therapy. The effectiveness of these therapies was assessed with subjective Neumeral Rating Scale(NRS) and Dyshidotic Eczema Area and Severity Index(DASI). Conclusion: After the treatment according to the theory of SCM, NRS, and DASI score about RAS and pompoholyx was decreased, and the general condition of the patient also has improved.
