    Medical Minds of Trational Korean Medicine in DongEuiBoGam and Dongyi Suse Bowon􋺸

    1. Objectives This research is purposed to present developmental direction of traditional Korean medicine, through summarizing original medical mind of traditional Korean medicine. 2. Methods It was researched as bibliographic study with chief writings of traditional Korean medicine such as 􋺷 DongEuiBoGam(東醫寶鑑)􋺸 and 􋺷Dongyi Suse Bowon(東醫壽世保元)􋺸 3. Results and Conclusions (1) Huh Jun regarded Three Precious(三要: 精, 氣, 神) as important factors in Morpho-imaginary viewpoints. So he suggested finding out the pathological states according to the states of Three Precious. (2) As Huh Jun recognized that the essence(精) is the origin of the body, he suggested the medical mind of laying emphasis on the essence. And he suggested that circulating of the essence is important to health and longevity, which is different from previous trational medicne laying emphasis on the tonifying of five organs(五臟). (3) Dong-mu suggested diagnostic method that is to find out “Natural Principle of Organ(臟腑性理)” by observation of “Knowing-Acting(知行)” and “Morphological aspect and Temper(體形氣像)”. And he suggested finding out pathological states according to states of “Good Qi of each constitution(體質正氣)”. (4) Dong-mu recognized that the disease should be treated by recovering “Good Qi of each constitution” with both pharmacotherapy and non-pharmacotherapy. So he uggested the medical mind of laying emphasis on “Good Qi of each constitution”, which is different from previous trational medicne laying emphasis on the tonifying of five organs. (5) To be brief, Huh Jun layed emphasis on the essence and Dong-mu layed emphasis on “Good Qi of each constitution”. Emphasizing internal vitality which is applyed to treatment and prevention of disease is the original medical mind of traditional Korean medicine.

    A study on the ‘Dae-Byung’ and ‘Constant Mind'

    1. Objectives This study was to investigate the basic principle of 'Dae-Byung' and constant mined as a characteristic symptoms of mind-body medicine. 2. Methods The concept of constant mind was compared with confucianism literatures such as 'The Great Learning", 'The Discourses of Mencius" and Lee, Jema's writings. and the concept of 'Dae-Byung' was investigated Lee, Jema's medical writings. 3. Results & Conclusions The 'Dae-Byung' is a sasang constitution's own symptom and it can be occurred by discontrolled constant mind. Soyangin's dae-byung is a amnesia, and Taeumin's dae-byung is a palpitation. It is a typical symptoms to represent the characteristics of mind-body medicine. The early recognition of unhealthy condition is the functional disorder of intention, ethereal soul, corporeal soul, and will of four organs. The constant mind is originated 'the constant mind' of 'The Discourses of Mencius' and the righteousness mind of 'The Great Learning'. And the constant mind of Dong-mu was contained the direction of Qi's movement based on nature Qi and emotion Qi.

    A Study on the Clinical Features of the Ordinary Sleeping Patterns of Taeumin Using Logistic Regression

    1. Objectives This study is for discovering the characteristics of the sleep in ordinary symptoms based on the Sasang Constitution. The result of this study could be helpful to understand and to identify the patients as Taeumin by contrast with non-Taeumin. 2. Methods There were 1,229 patients(529 men), who answered the questionnaire about their ordinary sleep patterns. They had been diagnosed, including their Sasang Constitution, by the Sasang Constitution specialist at Bundang Oriental Hospital of Dongguk university. Applying the binary logistic regression analysis to each item about ordinary sleep patterns and the diagnosed Sasang Constitution, we can measure the characteristics and influence of ordinary sleep patterns to the dependent variable(Sasang Constitution). 3. Results As a result of the binary logistic analysis on the observed questionnaire, we found the similar features of the ordinary sleep patterns on Taeumin by contrast with non-Taeumin. In order of influence that identify Taeumin or non-Taeumin, first, Taeumin has a tendency that he will snore well or frequently. Second, Taeumin has a tendency that he will not dream, when he sleeps by contrast with non-Taeumin. 4. Conclusions This study will be used to identify patients as Taeumin or non-Taeumin by means of a reference tool.

    Assessment of the Clinical Efficacy of the Health Index in the Sasang Constitutions : Short Form-36 Based Study

    1. Objectives Theoretically Healthy Condition of four constitution is based on intact state of four fundamental factors - digestion, urination, perspiration, defecation. Each one of these four factors is specifically related to its respective constitution. We made SHI(Sasangin Health Index) 10 items from four factors, and examined clinical efficacy of the four factors as index of health status in four constitution. 2. Methods and Procedures We offered QSCCⅡ, SHI and SF-36 questionnaires to adults who had a periodic medical examination. 233 adults were classified to one of four constitution. We compared means of 8 scale scores of SF-36, and computed the correlation coefficient between 8 scale scores of SF-36 and 10 item scores of SHI in four constitution. 3. Results In Soyangin, stool volume and frequency are weakly correlated with health status(P<0.05). In Soeumin, appetite is weakly correlated with health status(P<0.05). In Taeumin, there is no correlation between perspiration and health status. 4. Conclusions 2 factors - digestion, defecation - had clinical efficacy as index of health status.

    The Clinical Study on Ordinary Symptoms and Pathological Symptoms of Stroke In-patients based on Sasang Constitutional Medicine

    1. Objectives The purpose of this study is to investigate constitutional ordinary and pathological symptoms differences among each constitutional groups of stroke in-patients. 2. Methods 101 stroke in-patients(Soyangin(SY) na)=52, Taeumin(TE) n=39, Soeumin(SE) n=10) hospitalized at Kyung-Hee Oriental medical center from Nov. 2003 to Sept. 2004 were investigated through questionnaire which consists of 18 parts, 289 questions(156 questions concerned ordinary symptoms and 133 pathological symptoms). The answers were analyzed statistically in order to find the questions which represent the significant differences among each constitutional groups. ( a) number of patients) 3. Results As for the ordinary symptoms, majority of the Soyangin group replied that they experienced "frequent dreams during sleep" and "scarce constipation". Majority of the Taeumin group affirmed to "dark redness of face", "dry nose", "low intolerance of heat", "good appetite and digestion", and "frequent overeating experiences". Majority of the Soeumin group affirmed to "pale face", "insomnia", "frequent constipation", "poor appetite and digestion", "small amount of diet", "slow eating", "fond of warm or hot water", and "easily tired(especially after sweating)". As for the pathological symptoms, majority of the Taeumin group affirmed to "frequent optic fatigue", "frequent tinnitus", "frequent bitter mouth", "fond of cold water", "flatulent", "stuffiness in the navel area", "irregular stool habits", "frequent constipation", "large amount of sweat", and "frequent rigidity of neck". Majority of the Soeumin group affirmed to "bad appetite", "fond of warm or hot water", "drink small amount of water", "sensitive during sleep" and "frequent heavy head". 4. Conclusions Through this study, We found statistically significant questions distinguishing the characteristics of each constitutional group.

    A Study on the Sasang Constitutional Diagnosis by Perceptual Voice Analysis

    1. Objectives This study was performed by means of perceptual evaluation of the voices of Sasang Constitution. 2. Methods 73 female subjects were classified by means of 3 kinds of Questionnaire papers(QSCCⅡ, QSCCI, Sasang Pattern Identification Questionnaire). So they were categorized into 3 groups, 23 Soyangin, 28 Taeumin and 22 Soeumin. 73 voice samples were presented three times to a group of 5 judges. The time interval between ratings was 14 days. The four goals of this study were to evaluate the intraobserver reliability between each rating, to evaluate the interobserver reliability, to evaluate the reliability between the each rating and Questionnaire result and to make the notion of the consensus of Sasang Constitution's Voice. 3. Results & Conclusions The intraobserver reliability between the first and second rating showed significance statistically among all observers. And the intraobserver reliability between the second and third rating showed significance except one observer. The interobserver reliability among the three ratings showed significance statistically except one to two observers in the first rating and other one to another one in the second rating. In the reliability between the each rating and Questionnaire result, one in the first rating, other one in the second rating and another two in the third rating showed significance. To make the notion of the consensus of Sasang Constitution's Voice, classification into 4 categories was made: clear/hoarse, high/low, fast/slow, powerful/powerless. The voice of Soyangin group was classified as powerful and fast, and that of Taeumin group as powerful, hoarse and low and that of Soeumin group as powerless and slow.

    A Study on the Sasang Constitutional Characteristics of Obese Middle-Aged Women

Objectives This research is to ascertain relations between the Sasang Constitution and characteristics of the eating attitudes, mental state, body composition, various examination results of obese middle-aged women. 2. Methods The subjects were 31 female patients whose BMI(Body Mass Index) was over 25kg/m2 and age was 35~55. It was analysed statistically the results of EAT(The Eating Attitudes Test), DEBQ(Dutch Eating Behavior Questionnaire), BDI(Beck Depression Inventory), STAI(State-Trait Anxiety Inventory), body composition analyser, biochemistry, abdominal ultrasonography, mammography which was surveyed from them. 3. Results and Conclusions 1)There was no relation between the Sasang Constitution and the eating attitudes, eating behaviors. But Taeumin got statistically lower score than Non-Taeumin in 23rd question for emotional eating and 33rd question for environmental eating of DEBQ 2)On the mental state, Taeumin got statistically lower score than Non-Taeumin in BDI, STAI-S 3)On the body composition, Taeumin got statistically higher value than Non-Taeumin in BMI. Soyangin got statistically lower value than Non-Soyangin in BMI, left upper limb water, soft lean mass, body water, trunk water. 4)On the various examinations, Taeumin got statistically lower value than Non-Taeumin in serum albumin. Soyangin got statistically lower value in BMI and higher frequency in gallstone, breast nodule than Non-Soyangin. 5)It was thought that Taeumin's obesity is managed by controling physical factor such as excessive 'Gathering Qi(吸聚之氣)' rather than eating attitudes and psychological factor.

    The Effects of Use of Sasang-Bang with Western Medicine on Liver and Kidney Function

    1. Objectives The Purpose of this study was to evaluate whether use of Sasang -Bang with western medication may injury the liver and the kidney function. We clinically studied the change Liver function test and BUN/Cr in patients who were hospitalized in Hana oriental Medical center for more than 1 month. 2. Methods The subjects were 33 patients admitted in Hana Oriental Medical center. All patients had been checked Liver Function Test and BUN/Cr three times every 2 weeks. 3. Results Aspartate Aminotransferase(AST), Alanine Aminotransferase(ALT), Gamma-Glutamyl transferase(GGT), Total billirubin, BUN, Creatine didn't change, compared with previous 2 weeks and 4 weeks. 4. Conclusions This study suggests that even though there are few toxic Sasang-Bang, in general, herbal medications with western medications which are prescribed by oriental medicine doctors should be carefully administered not to hurt liver and kidney function.

    A Study about Sasang Constitutional difference on Autonomous Function after Acupuncture stimulation

    1. Objectives This paper is intended to find out Sasang Constitutional difference on Autonomous Function after Acupuncture stimulation in healthy subjects. 2. Methods of Research Nineteen healthy volunteers(Soyangin 6, Taeumin 7, Soeumin 6) had acupunctured perpendicularly into the Hapkok point(LI4) and the Taechung point(Liv3). Heart Rate Variability, Mean Heart Rate and Blood Pressure were checked 4 times(before and after Acupuncture stimulation) to evaluate Autonomous Function. 3. Results & Conclusions The conclusions were as follows 1. There was no statistically significant Sasang Constitutional difference on sympathetic activity and parasympathetic activity before Acupuncture stimulation. 2. Acupuncture stimulation caused a statistically significant increase in the parasympathetic activity in the whole subject. 3. After Acupuncture stimulation, there was a statistically significant increase in the parasympathetic activity in Soeumin and a tendency of increase in the parasympathetic activity in Soyangin. But there was a tendency of increase in the Sympathetic activity in Taeumin.

    The Antimicrobial Activity of Bojungikgi-tang, Gwakhyangjeonggi-san, and Gunggwihyangso-san to Klebsiella pneumoniae 289 extracted from Allergic Rhinitis Patients

    1. Objectives Although the incidence of allergic rhinitis is increasing continually, Sasang Constitutional Medicine is showing favorable treatment outcomes. This study was aimed to test the antimicrobial effects of Bojungikgi-tang, Gwakhyangjeonggi-san, and Gunggwihyangso-san which are being applied to allergic rhinitis of Soeumin. 2. Methods Klebsiella pneumoniae 289 was extracted from allergic rhinitis patients. Bojungikgi-tang, Gwakhyangjeonggi-san, and Gunggwihyangso-san were boiled down and concentrated, and the substances obtained were tested for the effects on stability to temperature and pH and on antimicrobial activity 3. Results When measured the level of antimicrobial activity, the inhibition zones of Bojungikgi-tang, Gwakhyangjeonggi-san, and Gunggwihyangso-san were found 11.42, 11.07, and 10.63mm, respectively. The levels of MIC were 15㎕, 20 ㎕, and 10 ㎕ in that order. The antimicrobial activities were increased 13.88% in Bojungikgi-tang, 16.63% in Gwakhyangjeonggi-san, and 10.72% in Gunggwihyangso-san. There was no influence of pH to antimicrobial activity of all three substances. 4. Conclusions Bojungikgi-tang, Gwakhyangjeonggi-san, and Gunggwihyangso-san had a high level of antimicrobial activity to Klebsiella pneumoniae 289 even at a low concentration.

    The Effects of Gunyuljejo-tang on the CCl4-induced Liver Damage in Rats

    1. Objectives This study was carried out to investigate the effects of Gunyuljejo-tang on the CCl4-induced Liver Damage in Rats. 2. Methods Sprague-Dawley rats were devided into 5 experimental groups : Normal, NS+CCl4(Solid extract of CCl4 injection group after Normal Saline feed), GYJJT+CCl4(Solid extract of CCl4 injection group after Gunyuljejo-tang feed), CCl4+NS(Normal Saline feed group after CCl4 injection), CCl4+GYJJT(Solid extract of Gunyuljejo-tang feed group after CCl4 injection). Biochemical assays for serum enzyme activities such as AST, ALT, ALP, BUN, Creatinine, Uric Acid, Total Protein, Albumin, Total Cholesterol, Triglyceride, Glucose, and mRNA Revelation of Cytochrome p450 and activities such as LPO, GSH, GST, Glutathione Reductase, Glutathione Peroxidase, SOD, Catalase, Hydroxyproline, and β-Glucuronidase were performed. 3. Results ① GYJJT+CCl4 showed lower revelation of Cytochrome p450. ②GYJJT+CCl4 showed higher GSH activity than NS+CCl4, CCl4+GYJJT showed higher GSH activity than CCl4+NS injection significantly. ③GYJJT+CCl4 showed higher GST activity than NS+CCl4. CCl4+GYJJT showed higher GST activity than CCl4+NS significantly. ④GYJJT+CCl4 showed higher Glutathione Peroxidase activity than NS+CCl4, CCl4+GYJJT showed higher Glutathione Peroxidase activity than CCl4+NS significantly. ⑤ CCl4+GYJJT showed higher SOD activity than CCl4+NS significantly. ⑥ CCl4+GYJJT showed higher Catalase activity than CCl4+NS significantly. ⑦ GYJJT+CCl4 showed lower Hydroxyproline than NS+CCl4 significantly, CCl4+GYJJT showed higher Hydroxyproline than CCl4+NS significantly. ⑧GYJJT+CCl4 showed higher β-Glucuronidase activity than NS+CCl4, CCl4+GYJJT showed higher β -Glucuronidase activity than CCl4+NS significantly. 4. Conclusions Gunyuljejo-tang has the recovering effects on the CCl4-induced Liver Damage significantly.

    A Clinical Case Study of Low Back Pain Caused by the Herniated Lumbar Disc in Soeumin patient

Objectives The purpose of this study is to give an account of the effects of Korean herbal medicine therapy which is based on the Sasang Constitutional Medicine for the patient who has suffered from low back pain caused by the Herniated Lumbar Disc. 2. Methods Consulting her sungjeong(性情), Chehyungkisang(體形氣像) and QSCCⅡ, we have diagnosed her as Soeumin(少 陰人), and have prescribed Hyangbujapalmul-tang(香附子八物湯). 3. Results After the treatment, she felt her symptoms, low back pain, headache, and general condition, improved. 4. Conclusions This paper describes the process of management and changes of symptoms. We hope this paper useful to rehabilitatable treatment for low back pain caused by the Herniated Lumbar Disc through Sasang Constitutional Medicine.

    1. Objectives As we advance to senior society the incidence rate of heart disease such as atrial fibrillation grows higher. These heart disease can also be treated by Sasang constitutional medical diagnosis and treatment. 2. Methods There is one case-report of the patient who has severe atrial fibrillation and was treated with Sasang constitutional medicine and Westen medicine as well. 3. Results The patient had shown remarkable clinical effects, especially when Sasang Constitutional Medical treatment was practiced. 4. Conclusions Thus we report the healing process and result of this atrial fibrillation.

    A Case Report of Taeyangin Yulkyuk

    1. Objectives Yulkyuk is the interior syndrome of Taeyangin and it's chief symptom is dysphagia. Lee Je-Ma prescribed Mihudeungsikjang-tang for Taeyangin Yulkyuk. In this case we diagnosed a woman's chief complaints as Taeyangin Yulkyuk, prescribed Mihudeungsikjang-tang and experienced the effectiveness and reproduction of Mihudeungsikjang-tang against Yulkyuk. 2. Methods To treatment Taeyangin Yulkyuk we used Mihudeungsikjang-tang 3. Results Mihudeungsikjang-tang is effective against Taeyangin Yulkyuk and curled-up tongue, dry vomiting, dizziness are also the symptoms of Yulkyuk. 4. Conclusions We have many difficulties to diagnose and treatment Taeyangin constitution because there have been few clinical study about Taeyangin yet. Further study on Taeyangin constitution is more needed.

    A Case Study of the Taeumin Bronchiectasis, Wrong Diagnosis as Soyangin

    1. Objectives The purpose of this study was to investigate the effects of administration of Taeumjowi-tang on the Taeumin bronchiectasis 2. Methods Consulting her Cheyungkisang(體形氣像) and QSCC, we had diagnosed her as Taeumin and treated her with Taeumjowi-tang(太陰藐胃湯). 3. Results After the treatment, she felt her symptoms, cough, sputum, dyspnea, dizziness, general weakness, flushing sign, improved. 4. Conclusions We have concluded that these effects must be useful in the clinical study of the Sasang constitutional treatment on Bronchiectasis.

    A Case Study of One Patient Who Has Diabetic Gangrene in Foot and Finger Due to Diabetes

    1. Objectives Besides Diabetes Mellitus, its complications are matter of concerns as well. Its following complications are cardiovascular disease, glomerulopathy, retinopathy, neuropathy and diabetic gangrene. These complications can cause overall damage in major organs. In traditional oriental medicine it has been perceived that gangrene is mainly caused by "hwa(火)" but under sasang constitutional medicine its has been diagnosed as Taeumin superficial-disease(太陰人 表病) and Taeumin Jowiseungchung-tang(藐胃升淸湯) was prescribed. As a result, significal improvments was noted and thefore we report the outcome. 2. Methods We have given Taeumin Jowiseungchung-tang(太陰人 藐胃升淸湯) to a patient for treating diabetes complications. 3. Results This patient was treated with Sasang Constitional Medicine and acupuncture for about three weeks. After treatment, ulcer and whole body edema were improved considerbly. 4. Conclusions We have dianosed the patient as Taeumin superficial-disease(太陰人表病) by undergoing several diagnostic method used in sasang constutional medicine. Therefore we have given Taeumin Jowiseungchung-tang(太陰人藐胃 升淸湯) to the patient and has obtained satisfactory results in curing several diabetic complications especially ulcer due to diabetes.

    Clinical Cases about Facial Palsy by Using Yeoldahanso-tang(熱多寒少湯)

    1. Objectives Facial Palsy(Bell's palsy) is a common disease in oriental medicine. In Sasang Constitutional Medicine, Taeumin have a basic condition by Ganyeol as their inherent symptomatic phamacology. This case is a study about Taeumin's facial palsy in Ganyeol conditions. 2. Methods In treatment of facial palsy, especially in acute stage, We prescribe Yeoldahanso-tang(熱多寒少湯) when a patient with facial palsy is diagnosed as Taeumin by their own characters. 3. Conclusions This case-study shows an efficient results by using Yeoldahanso-tang(熱多寒少湯) in treatment of facial palsy compared with other known treatments.

    A Case Study of Chronic Cough of Taeumin throuth the Change Process of Sweating

    1. Objectives In this study, it is experienced that the process of sweating is important in chronic cough of Taeumin. In this case report, the patient has been simproved throuth reaction of sweating improvement. In『Dongyi Soose Bowon』, The process of sweating change is described in detail. "Hamkeylhae" is similar to chronic cough in『Dongyi Soose Bowon』, it is cough that is hard to spit. The chronic cough of Taeumin is related to the pathology of Liver and Lung, sweating condition is also related to condition of Lung in Sasang constitutional medicine. So the study for the process of sweating is needed. 2. Methods One patient diagnosed as chronic cough was practiced with Sasang constitutional medicine. And We observed sweating change process during treatment. 3. Results As sweating condition improved, chronic cough also decreased, but not improved, not decreased. 4.
