    Effects of Dipsaci Radix on Muscle Atrophy of MCAO Rats

    Abstract
    Objectives : This study evaluated the effects of Dipsaci Radix on hindlimb muscle atrophy of stroke-model rats. Methods : Stroke-model was established in Sprague-Dawley rats using by the temporal occlusion of the middle cerebral artery(MCAO). Water-extract of Dipsaci Radix was administer orally, once a day, for 4 weeks. Then effects of Dipsaci Radix treatment were observed with changes of infarct size, neurological score, muscle fiber type and size, MyoD expression by immunohistochemistry and Western blotting. Results : 1. Dipsaci Radix treatment improved the aggravation of neurological score of MCAO rats, but it was not effective on infarct size induced by MCAO. 2. Dipsaci Radix treatment increased the ratio of muscle fiber type-I, MyoD expression and positive cells in soleus muscle of the contralateral hindlimb of MCAO rats. 3. Dipsaci Radix treatment was not effective on the muscle fiber size in soleus muscle of the contralateral hindlimb of MCAO rats. 4. Dipsaci Radix treatment reduced MHC-II expression in gastrocnemius muscle of the contralateral hindlimb of MCAO rats. Conclusions : These results suggest that Dipsaci Radix has a protective effect on muscle atrophy and motor disfunction of stroke patients.

    A Study on the Effect of Gwanjul-bang(Guanjie-fang)-3 in Rheumatoid Arthritis

    Abstract
    Objectives : Gwanjul-bang(Guanjie-fang)-3 (GJB-3) is a prescription used clinically to treat rheumatoid arthritis. The purpose of this study is to investigate the immunity responses of GJB-3 to the human fibroblast-like synoviocytes isolated from patients with rheumatoid arthritis. Methods : Experimental groups were divided into 6 groups ; The hFLS were stimulated with hrTNF-α(100 ng/ml) and hrIL-1β(10 U/ml). Afterwards, They were treated with medium (CT), methotrexate (10㎍/ml, MTX) and GJB-3 (1, 10, 100㎍/ml) for 6 hours. The cells were harvested following the further incubation with [3H]-thymidine reagent for 4 hours. The WT group was cultured without cytokine stimulation. Results : 1. The proliferation of hFLSs was significantly reduced in 100㎍/ml dose. 2. The mRNA expression of TNF-α and the production of ROS was dose-dependently reduced in hFLS. 3. The mRNA expression of IL-1β, IL-6, IL-8, ICAM-1, NOS-Ⅱ, MMP-3 were reduced in high concentration. 4. The production of NO was significantly reduced in over 10㎍/ml dose. 5. The mRNA expression of TIMP-1 was dose-dependently increased. Conclusions : We found out that GJB-3 is considered to be effective in treating rheumatoid arthritis.

    The Influence of Bee Venom on Immune-modulation in Rheumatoid Arthritis

    Abstract
    Objectives : In the present study we hypothesized that the change of cytokine profiles can be identified after bee venom treatment and that is associated with modulation of immune-activating cells which have pathogenic role in rheumatoid arthritis. Methods : T cells and monocytes were isolated from human peripheral blood. Isolated monocytes were further induced to differentiate into dendritic cell(DC). Isolated T cells and differentiated DC were activated with phorbol 12-myristate 13-acetate(PMA) and lipopolysaccharide(LPS), respectively. Bee venom(BV) added to each experiment group. Intracytoplasmic stain of T cells and morphologic observation, surface molecule analysis of DC were performed before and after cellular activation. Results : Pro-inflammatory cytokine was reduced whereas anti-inflammatory cytokine was increased with BV treatment in T cells. In DC, morphologic maturation and surface molecule increase by LPS was also inhibited by BV. Conclusions : BV may be an effective treatment in rheumatoid arthritis through its immune-modulating effect.

    The Effects of Magye-um(Magui-yin) on Obese Rat Induced by High Fat Diet

    Abstract
    Objectives : This experimental study was designed to investigate the effects of Magye-um(Magui-yin) on change of weight, serum and in obese rat induced by high fat diet. Methods and Materials : Experimental groups were divided into 3 groups; normal group, control group and sample group for 10 separate rats as experiment. We fed normal diet to normal group for 8 weeks, high fat diet and administered normal saline 448 mg/kg(1 time/1 day) to control group for 8 weeks, high fat diet and administered Magye-um(Magui-yin) extract 448mg/kg(1 time/1 day) to sample group for 8 weeks. Magye-um(Magui-yin) was investigated effects of serum contents and size of epididymis fat cells Results : 1. Glucose level in blood serum of sample group decreased as compared to control group with statistical significance(p<0.01). Insulin level in blood serum of sample group was increased as compared to control group with statistical significance(p<0.05). 2. Total cholesterol, triglyceride and LDL-cholesterol level in blood serum of sample group decreased as compared to control group with statistical significance(p<0.001) and phospholipid and free fatty acid level in blood serum of sample group decreased as compared to control group with statistical significance(p<0.005). HDL-cholesterol level in blood serum of sample group was not different from that of control group statistically. 3. Leptin level in blood serum of sample group was increased as compared to control group with statistical significance(p<0.005). 4. T3 and T4 level in blood serum and mean size of epididymis fat cells of sample group decreased as compared to control group with statistical significance(p<0.001). Conclusions : It is suggested that Magye-um(Magui-yin) has some effects to reduce the obesity induced by high fat diet in rats.

    Hair Growth Promoting Effect of Saengbal-eum(Sh?ngf?-y?n) Application on Hair-Removed C57BL/6 Mouse

    Abstract
    Objectives : Saengbal-eum(Shēngf&agrave;-yǐn) is a hair care product which is composed of eight plant extracts used in oriental medicine. This study was carried out to investigate effect of Saengbal-eum(Shēngf&agrave;-yǐn) on hair regrowth in a shaving model of C57BL6 mice. Materials and Methods : Five-week-old mice were acclimated for 1 week under 21~23℃, 40~60% relative humidity, and 12h of a light/dark cycle before beginning experiment. There were four experimental groups including normal saline(control), a positive control of 3% Minoxidil(MXD), and 30%, 100% Saengbal-eum(Shēngf&agrave;-yǐn) extracts(sample-I, II) for 12 female mice. The test compounds were topically treated once a day for 14 days. The hair regrowth was photographically and histologically determined during the experimental period of 14 days. Revelation of TGF-β1 and EGF were also determined using a immunohistochemistry. Results : The topical application of 3% MXD and Saengbal-eum(Shēngf&agrave;-yǐn) in female mice promoted hair regrowth earlier and faster than the control groups. Concentrations of hairs and thick-hair ratio in 3% MXD and Saengbal-eum(Shēngf&agrave;-yǐn) groups were promoted than control group. There was not significant difference of hair regrowth, concentration of hair, and thick-hair ratio between 3% MXD and Saengbal-eum(Shēngf&agrave;-yǐn) groups. TGF-β1 was not revealed in control, 3% MXD and Saengbal-eum(Shēngf&agrave;-yǐn) groups. EGF was strongly positive in out root sheat of some thick hair of 30%, 100% Saengbal-eum(Shēngf&agrave;-yǐn) group, but negative in control and 3% MXD group. Conclusions : These results suggest that Saengbal-eum(Shēngf&agrave;-yǐn) has hair growth promoting activity and it can be for treatment for alopecia. And these effects relate to EGF revelation of hair roots.

    The Short-term Efficacy of Acupuncture for Chronic Low Back Pain :Randomized Sham Controlled Trial

    Abstract
    목적 : 만성요통에 있어서 침치료의 효과에 대해 연구하고자 하였다. 방법 : 포함기준은 연령 20세 이상의, 3개월 이상 된 요통을 가진 환자를 대상자로 하였고, 척추종양, 감염, 신경학적 결손과 같은 척추질환은 대상에서 제외시켰다. 단순 요추부 X-선 사진과 CT를 촬영한 후, 전체 57명의 환자 중 50명을 선정하여 각각 25명씩, 수기침을 처치받는 군과 표피자극침을 처치받는 군으로 무작위 배정하였다. 수기침 군은 20분 동안 총 14개의 경혈에 得氣를 이용한 처치를 받았고, 표피자극침 군은 20분 동안 총 14개의 비경혈점에 표피자극만의 처치를 받았다. 침치료는 한 주에 3번씩, 4주 동안 시행되었다. 처치 전과 처치 2주 후, 4주 후에 VAS(Visual Analogue Scale), RDQ(Roland Disability Questionnaire), PGA(Patient Global Assessment)를 측정하였고, 처치 전과 처치 4주 후에 DT(Digital Thermography)를 측정하였다. 자료는 Repeated Measure ANOVA와 ITT를 이용하여 분석하였고, P<0.05인 경우를 통계적으로 유의하다고 보았다. 결과 : 총 3명의 탈락자가 있었는데 수기침 처치 군에서 1명, 표피자극침 처치 군에서 2명이 있었다. 수기침 처치 군에서, 처치 전, 처치 2주 후, 4주 후의 VAS 점수는 통계적으로 유의한 감소를 보였고(19.24±19.74, P=0.000), RDQ 점수 또한 감소하였으나 통계적으로 유의하지 않았다(1.16±4.85, P>0.05). 표피자극침 처치 군에서 처치 전, 처치 4주 후의 VAS 점수는 통계적으로 유의한 감소를 보였으나(15.75±20.73, P=0.000), 처치전, 처치 2주 후의 VAS 점수의 감소는 통계적으로 유의하지 않았고(P>0.05), RDQ 점수 또한 감소하였으나 통계적으로 유의하지는 않았다(1.84±5.25, P>0.05). 수기침 처치군과 표피자극침 처치군 간에, VAS와 RDQ 점수의 차이는 없었다. 각 군의 시기별 DT 측정치와 PGA 점수는 통계적으로 유의한 차이가 없었고 두 군 간의 비교에 있어서도 마찬가지 결과를 보였다. 총 3명의 대상자(수기침 처치군에서 1명, 표피자극침 처치군에서 2명)는 처치 후의 경미한 부작용으로서 피로감을 호소하였으나, 심각한 부작용을 호소하는 대상자는 없었다. 결론 : 단기간의 침치료는 만성요통에 있어 효과적이고 안전한 치료방법이다. 그러나 본 연구에서 침치료는 표피자극침 처치에 비해 뛰어난 효과를 보여주지는 않았다. 앞으로 다 기관에서 추후 추적검사를 포함한, 엄격한 임상 연구가 진행되어야 할 것으로 사료된다.

    Satisfaction Questionnaire, VAS and ODI Scale of Chuna Manual Therapy in Patients with Low Back Pain(LBP)

    Abstract
    Objectives : The purpose of this study was to identify and evaluate factors influencing the level of satisfaction of patients with LBP who have undergone Chuna Manual Therapy. Methods : This study was conducted from April 2006 to October of the same year, at Department of Rehabilitation at the Sang-Ji University Oriental Medical Hospital, ninety-four patients who had been treated using Chuna Manual Therapy participated in the questionnaire. The participants were surveyed using the questionnaire of Dimatteo and Hays which was modified to serve the purpose of this study by the author. Both the Visual Analogue Scale(VAS) and Oswestry Disability Index(ODI) were examined by the questionnaire. The collected data was analyzed by a t-test and ANOVA according to the purpose of this study. Results : There was no difference in statistic values in LBP patients' satisfaction according to age, religion, schooling, income, diagnosis, duration of disease, cost, the number of kinds of Chuna Manual Therapy used, the time required, waiting time, the number of other medical treatments, but there were differences according to sex, marital status, occupation, recurrence and the period of Chuna Manual Therapy(p<0.05). The LBP patients were satisfied with the doctor's bedside manners, but were dissatisfied with their own occupations and family life. There was no difference between the statistical values of the LBP patients' Visual Analogue Scale(VAS) and Oswestry Disability Index(ODI) according to sex, marital status, occupation, recurrence and the period of Chuna Manual Therapy(p<0.05). Conclusions : And there were differences according to sex, marital status, occupation, recurrence, the period of Chuna Manual Therapy. Therefore, we should consider these factors in treatment resulting in treatment for LBP patients leading to more careful access to effective LBP treatment.

    A Clinical Study on the Lumbar Stability of the Patients with Degenerative Lumbar Intervertebral Disc

    Abstract
    Objectives : A Structural factor of the lumbar spine determines stability of lumbar spine, and instability of lumbar spine makes degenerative changes on lumbar spine. The purpose of this study is to analyze structure alignment in the patients with Lumbar Intervertebral Disc(IVD) degeneration(Deg.). Also, I would like to make materials for prognosis judgement by observing clinical procedure. Methods : We analyzed the lateral view of lumbar spine checked at erect position on 212 patients who had been diagnosed as IVD Deg. by Magnetic Resonance Imaging. We investigated the Ferguson's angle, Lumbar lordotic angle, L4-5 IVD angle, L5-S1 IVD angle on X-ray film. We applied conservative Korean medicine treatment to 87 among 212 patients, and examined clinical procedure by Visual Analogue Scale(VAS). Results and Conclusions : 1. Ferguson's angle of 212 patients with IVD Deg. was smaller than normal range. Patients with L5-S1 IVD Deg. or herniation of nucleus pulposus(HNP) showed smaller Ferguson's angle than those without L5-S1 IVD Deg. or HNP. Angle of male was smaller than that of female. 2. Lumbar lordotic angle of female was bigger than that of male. Patients with L5-S1 IVD Deg. or HNP showed smaller Lumbar lordortic angle than those without L5-S1 IVD Deg. or HNP. 3. Patients with L4-5 IVD Deg. or HNP showed smaller L4-5 IVD angle than those without L4-5 IVD Deg. or HNP. 4. Patients with L5-S1 IVD Deg. or HNP showed a little smaller L5-S1 IVD angle than those without L5-S1 IVD Deg. or HNP. 5. Patients with angles beyond normal range indicated more effective result than those with normal range of angles. And patients with more effective result for the low back pain showed smaller Ferguson's angle and Lumbar lordotic angle. Ferguson's angle and Lumbar lordotic angle exert negative influence on the VAS of low back pain decrement(by regression analysis).

    Effect of Comprehensive Oriental Medical Therapies on the Rehabilitation of Post-stroke Patients

    Abstract
    연구목적 : 한의학적 종합치료가 중풍 후유증환자의 기능회복에 미치는 영향을 평가한다. 연구방법 : 인천지역에 거주하는 중풍 후유증 환자 29명을 대상으로 5주간의 한약투여와 4주간 매주 1회 침, 동씨침, 동기침법, 첩대요법, 약침요법을 시행하고, 시행전후의 BDI, SS-QOL, SSF, BMI, BBS, MFT, glucose, SGOT, SGPT, total cholesterol, triglyceride의 전후 결과를 비교하였다. 결과 : BBS와 MFT, glucose가 유의하게 증가하였고, total cholesterol은 유의하게 감소하였다. 나머지 항목과 예후인자에 따른 평가에는 유의성이 없었다. 결론 : 한의학적인 종합치료가 말초적 기능회복의 관점에서 만성중풍환자의 상지기능 및 하지기능의 회복과 Total cholesterol의 수치감소에 유의성 있는 효과를 보였다. 지속적인 연구를 통한 보다 우수하고 안전한 프로토콜의 개발이 필요하다.

    The Influences of Chuna(shoulder traction) Therapy for Shoulder Pain and Range of Movement in Hemiplegic Patients After Stoke

    Abstract
    Objectives : The purpose of this study was to investigate the efficacy of the Chuna therapy on shoulder for patients with chronic stroke. Methods : The Chuna therapy on shoulder was applied to 26 hemiplegic patients who had been treated by general rehabilitation therapy after stroke for 2 weeks. The other 24 hemiplegic patients had only general rehabilitation therapy during the same period. Outcomes were assessed by visual analogue scale, medical research council scale, passive range of movement, meridian-electromyograph, and subluxation grade. Results : The patients exhibited significant improvements on the visual analogue scale and passive range of movement but did not show significant results in medical research council scale, meridian-electromyograph and subluxation grade meaningful differences between sample and control group. Conclusions : The Chuna therapy on shoulder may be an efficacious method of improving passive range of movement and pain of the affected shoulder of patients after stroke.

    The Effect of Oriental Medicine on Growth of Children and Study on BMD as a Growth Measurement

    Abstract
    Objectives : The aim of the present study was to evaluate the effect of oriental medicine therapy on the growth of children, and to assess the relation of BMD(Bone Mineral Density) and the stature growth during the period of bone growth. Methods : We measured height and weight of the children, percentile of the growth by using the growth curve, BMD and bone age by using Osteoimages Plus. We treated the children with acupuncture, herbal acupuncture, herbal medication, physical therapy. Then we measured difference of height and percentile change of the children. Results and Conclusions : 1. According to sexual type, percentile change of the growth is higher in the female. 2. According to age group, percentile change of the growth is higher in period Ⅱ(Male;11-14, Female;10-12). 3. According to treatment group, percentile change of the growth is higher in acupuncture and herbal-acupuncture, herbal-medication treated group. 4. According as BMD increases, BMI(Body Mass Index) were ascending significantly.

    A Study on the Correlation between the Patterns of the 7-zone-diagnostic System and the Clinical Parameters

    Abstract
    Objectives : The purpose of this study is relation Oriental Medicine and Western medicine with the data from VEGA-DFM 722 and the clinical parameters. Methods : This study was carried out with the data from some clinical parameters. We made three groups according to the Factor AA form of VEGA-DFM 722. The Factor AA forms of three groups are that the red bar graphs of zone 1, 2 and 3 are lower than the normal range. The Factor AA form of Group A is that the red bar graphs of zone 4, 5, 6 and 7 are located at the normal range. The Factor AA form of Group B is that the red bar graphs of zone 4, 5, 6 and 7 are lower than the normal range. The Factor AA form of Group C is that the red bar graphs of zone 4, 5, 6 and 7 are higher than the normal range. After the data from clinical parameters to correspond with conditions of each group were selected, the data from clinical parameters among three groups analyzed statistically. Results and Conclusions : The values of Albumin, Red blood cell count, Hemoglobin, Triglyceride, Thyroxine of Group C are lower than them of Group A and B. These results mean that group B has low energy. To conclude, it is thought that the more red bar graphs of zone 4, 5, 6 and 7 are low, the more the volume of energy of a living body is small.

    A Clinical Study of Patient with Lumbar Herniation of Intervertebral Disc Combined with Functional Scoliosis by Treating Ah-Shi Points of the Muscles that Induce Unilateral Pelvic Obliquity

    Abstract
    Objectives : Lumbar herniation of intervetebral disc occasionally induces fuctional scoliosis. If the patient show unilateral pelvic obliquity, treating related muscles' tenderness can reduce pain coming from lumbar herniation of intervertebral disc and also improve functional scoliosis, unilateral pelvic obliquity. Methods : We treated the Ah-Shi Points by acupuncture, moxibustion, and herbal treatment. Several physiological and radiological tests, Visual Analogue Scale(VAS) of low back and hip pain, Cobb's angle for measuring the state of scoliosis, apparent length gap of both legs for checking unilateral short leg, etc. were used for assessment. Results : After treatment, we confirmed these improvement:VAS from 10 to 1~3; Cobb's angle from 15° to 12°; apparent length gap of both legs from 1.5cm to 1.25cm. Conclusions : These results suggest treating muscles' tenderness that worsen the unilateral pelvic obliquity, is effective for reducing pain symptoms from lumbar herniation of intervertebral disc and also functional scoliosis.
