        <sec>
          <title>Objectives</title>
          <p>The aim of this study was to investigate the effects of GalGunSeungGi-Tang (GST) on allergic contact dermatitis (ACD) induced by 2,4-dintrochlorobenzone (DNCB)</p>
        </sec>
        <sec>
          <title>Methods</title>
          <p>In this study, The changes of body weight, ear weight, ear thickness, spleen weight, dorsum skin thickness, symptom score by eyesight, histological finding, proliferation rates of splenocyte <italic>in vitro</italic> and <italic>in vivo</italic> are investigated to check effects of GST. The mice are divided into four group; Normal (naive mice) , Control (DW administered), GST-L (GST 500mg/Kg/day administered), GST-H (GST 1,000mg/Kg/day administered).</p>
        </sec>
        <sec>
          <title>Results</title>
          <p>GST inhibited weight of ear significantly (<italic>P</italic> &#x3C; 0.05) and also thickness (<italic>P</italic> &#x3C; 0.01). In addition, There are significant decrease in thickness of dorsum skin and proliferation rates of splenocyte <italic>in vivo</italic> in GST administered group. Finally, GST reduced symptom score and hyperkeratosis, hyperpigmentation, increase of granulocyte and parakeratosis in histological finding.</p>
        </sec>
        <sec>
          <title>Conclusions</title>
          <p>These results suggest that GST can decrease symptoms of ACD.</p>

      <title>I. 緖 論</title>
      <p>알레르기접촉피부염(Allergic contact dermatitis, ACD)은 홍반, 구진, 소수포, 대수포 등의 발진 형태를 취하는 피부염으로 2,4-dintrochlorobenzone (DNCB) 같은 알레르기 항원에 의해 노출되어 증상이 발현된다<sup><xref ref-type="bibr" rid="r001">1</xref></sup>. 이러한 증상은 과민반응 제&#x2163;형인 지연형 과민반응에 의한 결과로 특정 항원이 랑게르한스세포에 의해 포획되고 국소 림프절의 T림프구가 항원에 감작되어 시작한다. 이후 반복적인 항원 노출에 의해 알레르기 반응이 발생하며 이에 T림프구가 축적되어 염증 반응이 발생한다<sup><xref ref-type="bibr" rid="r002">2</xref></sup>.</p>
      <p>韓方에서 알레르기접촉피부염은 漆瘡, 膏藥風, 馬桶廯 등으로 설명하였다. &#x226A;外科大成&#x226B;에서는 漆의 辛熱毒이 미치는 영향에 대해 설명하였고<sup><xref ref-type="bibr" rid="r003">3</xref></sup> 巢元方은 &#x226A;諸病源候論&#x226B;의 &#xFF62;漆瘡候&#xFF63;에서 漆이 인체에 자극물질이 됨을 언급하였다<sup><xref ref-type="bibr" rid="r004">4</xref></sup>.</p>
      <p>최근에는 접촉피부염에 관련하여 生地黃<sup><xref ref-type="bibr" rid="r005">5</xref></sup>, 山楂<sup><xref ref-type="bibr" rid="r006">6</xref></sup>, 忍冬藤<sup><xref ref-type="bibr" rid="r007">7</xref></sup>, 玄蔘<sup><xref ref-type="bibr" rid="r008">8</xref></sup>, 馬油<sup><xref ref-type="bibr" rid="r009">9</xref></sup>, 稻草<sup><xref ref-type="bibr" rid="r010">10</xref></sup>, 馬齒莧<sup><xref ref-type="bibr" rid="r011">11</xref></sup>, 苦蔘 및 黃連<sup><xref ref-type="bibr" rid="r012">12</xref></sup>의 단일 약재 연구가 보고되어 있고, 체질의학관점의 처방으로는 少陽人 處方인 荊防敗毒散<sup><xref ref-type="bibr" rid="r013">13</xref></sup>, 凉膈散火湯<sup><xref ref-type="bibr" rid="r014">14</xref></sup>, 太陰人 處方인 熱多寒少湯<sup><xref ref-type="bibr" rid="r015">15</xref></sup>등의 연구결과가 보고되어 있다.</p>
      <p>李濟馬의 &#x226A;東醫壽世保元&#x226B;에서는 太陰人의 病證을 크게 胃脘受寒表寒病과 肝受熱裏熱病으로 나누었다. 陽毒證, 裏熱瘟病, 飮一溲二 등의 證候를 나타내는 肝受熱裏熱病에서도 陽毒證은 面赤斑 斑如錦紋 咽喉痛 唾膿血 등의 증상을 보이는데 李濟馬는 이에 葛根解肌湯을 治方으로 제시하고 大便이 燥澁할 경우 葛根承氣湯을 제시하였다<sup><xref ref-type="bibr" rid="r016">16</xref></sup>.</p>
      <p>葛根解肌湯은 알레르기 자반병<sup><xref ref-type="bibr" rid="r017">17</xref></sup>, 단독<sup><xref ref-type="bibr" rid="r018">18</xref></sup>, 아토피피부염<sup><xref ref-type="bibr" rid="r019">19</xref></sup>, cytokine의 조절<sup><xref ref-type="bibr" rid="r020">20</xref></sup> 등의 다양한 임상례 및 연구가 이루어진 바 있지만, 葛根承氣湯 (GalGunSeungGi-Tang, GST) 은 葛根解肌湯에 비해 허<sup><xref ref-type="bibr" rid="r021">21</xref></sup>의 비만관련연구와 임<sup><xref ref-type="bibr" rid="r022">22</xref></sup>의 太陰人 便秘 치험례 연구를 제외하면 피부질환의 직접적인 연구는 보고되지 않았다.</p>
      <p>이에 저자는 홍반, 구진, 수포 등의 증상을 나타내는 알레르기접촉피부염이 裏熱이 熾盛하여 일어나는 陽毒證의 陽毒發斑의 과정으로 인식하고 葛根解肌湯의 연구<sup><xref ref-type="bibr" rid="r017">17</xref>-<xref ref-type="bibr" rid="r020">20</xref></sup>, 大黃의 淸裏熱효능에 대한 연구결과<sup><xref ref-type="bibr" rid="r023">23</xref>-<xref ref-type="bibr" rid="r024">24</xref></sup>, 葛根에 관한 고찰<sup><xref ref-type="bibr" rid="r025">25</xref></sup>, 大黃에 관한 고찰<sup><xref ref-type="bibr" rid="r026">26</xref></sup>, 葛根의 항알러지 연구<sup><xref ref-type="bibr" rid="r027">27</xref></sup>, 黃芩抽出物의 알레르기접촉피부염에 대한 연구 결과<sup><xref ref-type="bibr" rid="r028">28</xref></sup>를 토대로 葛根承氣湯 또한 알레르기접촉피부염에 효과가 있을 것이라 판단하여 실험을 계획하였다. 이를 확인하기 위해 DNCB에 의해 알레르기접촉피부염이 유발된 생쥐에 葛根承氣湯을 투여하였고 유의한 실험 결과를 얻었기에 보고하는 바이다.</p>
    </sec>
    <sec id="s2" sec-type="methods">
      <title>II. 硏究 및 方法</title>
      <sec id="s2a">
        <title>1. 재료</title>
        <sec id="s2aa">
          <title>1) 방제구성 및 약재구입</title>
          <p>본 실험에서 연구 주제로 삼은 葛根承氣湯 (Galgunseunggi-tang, GST) 의 방제 구성과 용량은 &#x226A;東醫壽世保元&#x226B;의 &#xFF62;新定 太陰人病 應用要藥 二十 四方&#xFF63;을 따랐으며 구체적인 처방 내용은 <xref ref-type="table" rid="t001">Table 1</xref>과 같다. 각각의 약재는 동신대학교 부속 목포한방병원에서 구입한 것을 사용하였다.</p>

          <title>2) 동물</title>
          <p>실험을 시작하기 전 동신대학교 실험동물윤리위원회(DSU-2014-053)의 승인을 거쳐 7주령의 雄性 Balb/c 계열의 생쥐를 구매하였다. 실험동물은 동물실험 전문업체 (Samtako, Korea) 에서 구매하여 사용하였으며 사육장으로 반입 후 동물실험시설에 규정에 적합한 사육시설에서 4일 동안 12시간 명암주기(dark/light) 및 실내온도 (24&#xB1;2&#x2103;) 와 습도 (55&#xB1;5 %)를 일정하게 유지시켰고 고형사료와 물을 충분히 공급하였다.</p>
        </sec>
        <sec id="s2ac">
          <title>3) 알레르기접촉피부염 유발 약물</title>
          <p>Acetone과 olive oil을 4대 1로 섞은 혼합액 (이하 AOO액) 에 Sigma 社 (시약 번호 : 237329, USA) 에서 구매한 2,4-dinitrochlorobenzene (DNCB) 을 1 % 비율로 희석하여 혼합용액 (이하 혼합용액) 을 제조한 후 제모한 생쥐의 피부에 도포하여 ACD를 유발하였다.</p>
        </sec>
      </sec>
      <sec id="s2b">
        <title>2. 방법</title>
        <sec id="s2ba">
          <title>1) 시료의 제조</title>
          <p>GST 2첩 분량 88g을 증류수 1,500&#x3396;에 넣고 전기약탕기 (Daewoong, Korea) 로 2시간 동안 煎湯하여 얻어진 추출액을 원심분리기 (Eppendrof, Germany) 에서 5,000rpm에서 15분간 원심분리하여 찌꺼기를 버린 상층액을 감압 농축기 (Eyela, Japan) 에서 감압 농축한 다음 27.34g의 분말을 얻었고 수득률은 30.72 %이었다. 이를 저농도군은 500mg/kg/day 비율로, 고농도군은 1,000mg/kg/day의 농도로 희석하여 해당 실험군에게 투여하였다.</p>
        </sec>
        <sec id="s2bb">
          <title>2) 알레르기접촉피부염 유발 방법</title>
          <p>동물전용 마취제인 zoletile (Virbac, France) 을 완충용액 (PBS) 에 1:100 비율로 희석하여 혼합액을 제조하였다. 생쥐 무게를 20g을 기준으로 하여 100&#x3395;를 복강 주사하여 마취시켰다. 1차 제모한 후 크림형 제모제를 바르고 5-10분 정도 방치하였다. 그 후 제모제를 제거하고 24시간 방치한 후 제모하였다.</p>
          <p>24시간 경과 후 아이프란액 (isoflurane, 중외제약) 호흡마취제를 1.5&#x3396;의 용량으로 마취통에 넣어 잠시 마취시킨 후 등과 귀 부위에 AOO액과 1 % DNCB가 혼합된 용액 (이하 혼합용액) 을 도포하였다. 귀 부위는 pipet (yellow tip) 을 이용하여 1일 1회 3일간 귀 부위에 혼합용액 30&#x3395; 도포하고, 등 부위는 1일 1회 3일간 등 부위에 혼합용액 50&#x3395;을 도포하였다.</p>
        </sec>
        <sec id="s2bc">
          <title>3) 군 분류</title>
          <p>생쥐를 각 군마다 8 마리씩 배정하였으며, 정상군(normal) 은 증류수를 도포한 후 증류수를 투여하였고, 대조군 (control group) 은 DNCB를 도포하여 ACD를 유발한 후 증류수를 투여하였다. 약물투여군은 저농도투여군과 고농도투여군으로 설정하였는데 저농도투여군 (GST-L) 은 DNCB를 도포한 후 500mg/kg/day의 양을 고농도투여군 (GST-H) 은 1,000mg/kg/day양을 투여하였다.</p>
        </sec>
        <sec id="s2bd">
          <title>4) 체중 측정</title>
          <p>실험 시작 전, 1주 경과 후, 2주 경과 후 그리고 희생시키기 하루 전에 전자저울 (CAS SV-02, Korea)을 사용하여 측정하여 기록하였다.</p>
        </sec>
        <sec id="s2be">
          <title>5) 귀 및 비장 무게의 측정</title>
          <p>실험이 모두 끝난 후 희생시킨 생쥐의 귀를 직경 5mm의 둥근 절편으로 뚫어 귀의 무게를 측정하였고 비장은 희생시킨 생쥐를 개복하여 비장 전체를 적출한 후 미량 저울 (OHAUS, USA) 을 이용하여 측정하였다.</p>
        </sec>
        <sec id="s2bf">
          <title>6) 귀 및 등 피부 두께의 측정</title>
          <p>희생시킨 생쥐의 등과 귀의 피부를 절제한 후 이들의 직경을 디지털 캘리퍼(Mitutoyo, Japan) 를 이용하여 측정하였다.</p>
        </sec>
        <sec id="s2bg">
          <title>7) 등과 귀 피부의 임상적 관찰</title>
          <p>각 군별 생쥐의 등 부위를 디지털 카메라 (Samsung, Korea) 로 촬영하고 이를 기초로 등과 귀 부위에 발생한 ACD 정도를 5점 척도로 수치화하였고 아래와 같은 기준으로 판정하였다.</p>

        </sec>
        <sec id="s2bh">
          <title>8) 피부 조직학적 관찰</title>
          <p>피부 조직을 관찰하기 위해 H &#x26; E 염색 (Hematoxylin &#x26; Eosin stain) 을 시행하였다. 희생시킨 생쥐의 등 부위 피부 조직을 적출한 후 10 % 포르말린에 고정하고 이를 파라핀에 포매하였다. 이후 미세절단기(Sakura, Japan) 를 이용하여 4&#x339B; 두께로 박절하여 슬라이드에 부착시키고 탈수와 투명 과정을 거쳐 흐르는물에 침전시키고 파라핀을 제거한 후 조직 절편을 Hematoxylin (SIGMA, USA) 과 Eosin (SIGMA, USA) 으로 염색하고 탈수 및 투명과정을 거쳐 100배율의 광학현미경으로 관찰하였다.</p>
        </sec>
        <sec id="s2bi">
          <title>9) 비장세포 증식율 <italic>in vitro</italic>실험</title>
          <p>ACD 유발 약물 및 GST 등을 투여하지 않은 상태의 생쥐에서 비장에서 추출한 비장세포 (spleenocyte) 의 부유액을 RPMI 1640 배지로 희석하고 96 well plate에 1.0&#xD7;106 cells/&#x3396; 농도로 나누고 LPS 10&#x3395;와 각각의 농도로 희석된 GST을 첨가하고 24시간 배양한다. 이후 Ez-cytox 15&#x3395;를 각 well에 첨가하고 24시간 정도 배양한 후 흡광도를 Microplate Reader로 450nm에서 측정하였다.</p>
        </sec>
        <sec id="s2bj">
          <title>10) 비장세포 증식율 <italic>in vivo</italic>실험</title>
          <p>ACD 유발 약물 및 GST 등을 투여가 미치는 비장세포 증식율을 관찰하기 위하여 약물 투여를 모두 마친 생쥐에서 비장을 적출한 뒤 비장세포 부유액을 RPMI 1640 배지로 희석하고 96 well plate에 1.0&#xD7;106 cells/&#x3396; 농도로 나눈 다음 LPS 10&#x3395;를 첨가하여 배양한다. 이후 Ez-cytox 15&#x3395;를 각 well에 첨가하고 2시간 정도 배양한 후 well의 흡광도를 Microplate Reader로 450nm에서 측정하였다.</p>
        </sec>
      </sec>
      <sec id="s2c">
        <title>3. 통계 분석</title>
        <p>테이터를 수집한 후 실험 결과에 대한 통계적 분석은 SPSS ver. 21 (IBM, U.S.A.) 를 사용하였고, <italic>P</italic>값이 0.05 미만인 경우 유의한 것으로 인정하였다. 실험 결과는 평균과 표준편차 (mean&#xB1;SD) 로 표기하였으며, 통계기법은 one-way ANOVA로 검정하였으며, 사후검정은 tukey test를 이용하였다.</p>
      </sec>
    </sec>
    <sec id="s3" sec-type="results">
      <title>III. 結 果</title>
      <sec id="s3a">
        <title>1. 체중 변화</title>
        <p>실험기간 동안 4회에 걸쳐 체중 변화를 관찰하였으며 실험 시작 전을 0 week로 설정하였다. 각각 1주 후, 2주 후에 측정하였고 희생시키기 전을 3week로 설정하고 체중을 측정하여 변화를 관찰하였다. 결과를 보면 2 week에서 GST 투여군 모두가 control군에 비해 유의하게 (<italic>P</italic> &#x3C;0.01) 감소하였다가 희생 직전에 회복하였다 (<xref ref-type="table" rid="t002">Table 2</xref>, <xref ref-type="fig" rid="f001">Fig 1</xref>)</p>

      <sec id="s3b">
        <title>2. 귀 무게의 변화</title>
        <p>희생된 생쥐의 귀를 일정한 크기로 잘라낸 후 무게를 측정하여 비교하였다. 그 결과 control군에 비해서 고용량의 葛根承氣湯을 투여한 GST-H군 (<italic>P</italic> &#x3C; 0.05)에서 유의하게 귀 무게가 감소된 것이 관찰되었다(<xref ref-type="table" rid="t003">Table 3</xref>, <xref ref-type="fig" rid="f002">Fig 2</xref>).</p>

        <title>3. 비장무게의 변화</title>
        <p>희생시킨 생쥐의 비장을 적출하여 무게를 측정하여 비교하였다. 그 결과 normal군에 비해서 다른 군들이 모두 유의하게 증가하였으나 control 군과 GST를 투여한 실험군 사이에서는 유의한 차이를 보이지는 않았다 (<xref ref-type="table" rid="t004">Table 4</xref>, <xref ref-type="fig" rid="f003">Fig 3</xref>).</p>

        <title>4. 귀 두께의 변화</title>
        <p>희생시킨 생쥐의 양쪽 귀를 오려낸 다음 두께를 측정한 후 차이를 비교하였다. 실험결과 control군에 비해서 GST-L군(<italic>P</italic>&#x3C; 0.01)과 GST-H군(<italic>P</italic>&#x3C; 0.01)이 모두 유의하게 귀 두께가 감소됨이 관찰되었다 (<xref ref-type="table" rid="t005">Table 5</xref>, <xref ref-type="fig" rid="f004">Fig 4</xref>).</p>

        <title>6. <italic>In vitro</italic>에서의 비장세포 증식율 변화</title>
        <p>아무런 처치를 하지 않은 생쥐의 비장 세포를 추출하여 총 7개의 표본을 만들어 LPS 5 &#x338E;/&#x3396;을 첨가한 후 GST 추출물의 농도에 따라 변화하는 비장세포의 증식율을 관찰하였다. 실험 결과 투여하는 농도가 증가됨에 따라 증식율이 0 (LPS+) 군에 비해 모든 군에서 증가하는 경향은 보였으나 통계적으로 유의한 수준은 아니었다 (<xref ref-type="table" rid="t007">Table 7</xref>, <xref ref-type="fig" rid="f006">Fig 6</xref>).</p>

        <title>7. <italic>In vivo</italic>에서의 비장세포 증식율 변화</title>
        <p>실험을 마친 후 희생된 생쥐에서 적출한 비장에서 비장세포를 분리하여 증식율을 측정하였다. 비장세포의 증식을 위해 LPS 5 &#x338E;/&#x3396;을 첨가하였고 normal군을 100 % 로 환산하여 control군 및 실험군과 비교하였다. 실험 결과 GST를 투여한 군에서 모두 control 군에 비해 감소하는 경향이 보였으며 GST-L 군에서 유의하게 (<italic>P</italic> &#x3C; 0.05) 감소하는 것이 관찰되었다 (<xref ref-type="table" rid="t008">Table 8</xref>, <xref ref-type="fig" rid="f007">Fig 7</xref>).</p>

        <title>8. 피부증상의 임상적 관찰</title>
        <p>희생시키기 전 등과 귀 피부에서 관찰되는 임상양상을 5점 척도로 수치화하여 관찰하였다. Normal 군의 등 피부에서는 특별한 변화를 관찰 할 수 없었고, control군에서 관찰된 중등도 이상의 혈흔, 홍반, 낙설 및 착색이 GST-L 및 GST-H군에서는 완화되었음이 관찰되었다 (<xref ref-type="table" rid="t009">Table 9</xref>, <xref ref-type="fig" rid="f008">Fig 8</xref>-<xref ref-type="fig" rid="f009">9</xref>).</p>

        <title>9. 조직형태학적 소견 (Histological finding)</title>
        <p>피부조직에 H&#x26;E 염색(Hematoxylin &#x26; Eosin stain)을 실시한 결과, control군 (B) 은 epidermis의 두께가 과형성, 확장 되어, 그 주변에 과각화, 색소침착, 과립증가, 부전각화증이 정상군에 비하여 현저하게 증가하였다. GST-L군 (C) , GST-H군 (D) 모두 정상군에 가깝게 epidermis의 두께가 줄어들었고, 세포변형과 각화 증상 또한 감소되었다 (<xref ref-type="fig" rid="f010">Fig 10</xref>).</p>

      <title>IV. 考 察</title>
      <p>알레르기접촉피부염의 원인으로는 옻나무 접촉피부염이 대표적인데 옻나무 수액성분인 urushiol 의 pentadecylcatechol에 감작되어 있는 사람에게 발현된다. 다른 원인물질로는 옻나무와 유사한 성분을 지닌 은행, 망고, 캐슈넛 등이 있으며 식물류로는 국화, 앵초, 금속류로는 니켈, 크롬, 그 밖에 화장품, 고무가죽 제품, 접착제 등의 원인이 될 수 있다. 이러한 물질들은 일상생활에서 흔히 접할 수 있는 것들이므로 반복적으로 노출될 경우 지속적이고 반복적으로 접촉피부염을 유발할 수 있다<sup><xref ref-type="bibr" rid="r030">30</xref></sup>.</p>
      <p>서양의학에서의 알레르기접촉피부염의 치료는 피부염을 유발하는 원인물질을 찾아내는 것이 필수적이며 다른 물질로 항원대치하는 것이 중요한데 일상생활에서 흔히 접하거나 생활상 대치하는 것이 어려운 경우가 많아 이는 실제 적용하는데 어려움이 있다. 국소병변의 경우 크게 급성기와 아급성 및 만성기로 나누는데, 급성기에는 습포제를 이용하여 병변의 진물을 중단시키고 국소 스테로이드를 도포한다. 아급성기와 만성기에는 국소도포제를 사용하여 피부를 윤활하게 하고 국소 스테로이드 연고를 사용하는데 간혹 국소 스테로이드 제제에 포함된 방부제가 감작원으로 역할을 하여 증상을 심화시킬 수 있다. 전신병변의 경우는 스테로이드제의 전신 치료를 병행하고 소양증을 완화시키기 위해 항히스타민제를 사용하기도 한다. 다만, 스테로이드의 사용은 소아나 임신 및 수유부에게는 주의를 기울여야 하며 1-3주 이상 사용하게 되면 복용 중단 후 최고 1년까지 뇌하수체-부신피질의 억제가 나타날 수 있고 고혈압, 고혈당, 체중증가, 수면장애 등의 여러 부작용이 나타날 수 있으므로 점진적 감량에 주의해야 한다<sup><xref ref-type="bibr" rid="r031">31</xref></sup>. 따라서 이를 대체할 수 있는 한의학적 치료의 필요성이 증가하고 있음을 인식할 필요가 있다.</p>
      <p>한의학적으로 알레르기접촉피부염은 대개 漆瘡으로 인식되었는데 吳謙은 &#x226A;醫宗金鑑&#x226B;의 &#xFF62;漆瘡&#xFF63;, &#xFF62;狐尿刺&#xFF63;에서 알레르기접촉피부염과 유사한 증상을 설명하였다. &#xFF62;漆瘡&#xFF63;에서 &#x201C;漆瘡感受漆毒生, 腠理不密腫焮紅, 初發覺癢後如疹, 皮破流水更兼疼...宜韭菜汁調三白散塗之, 內服化斑解毒湯. 忌浴熱水... 生蟹黃調塗患處尤效.&#x201D;이라 하여 홍반, 각설, 소양감, 구진, 진물 등 알레르기접촉피부염의 임상증상과 비슷한 증상을 설명하고, 조리법 및 외치처방과 내복처방을 제시하였다. 또한 &#xFF62;狐尿刺&#xFF63;에서는 &#x201C;狐尿刺生手足間, 悶腫焮痛紅紫斑, 螳螂精尿流積毒...初起紅紫斑點, 肌膚乾燥, 悶腫焮痛, 不眠...內宜黃連解毒湯, 外以蒲公英連根濃煎溫洗...按癰疽腫瘍門.&#x201D;이라 하여 &#x226A;鍼灸大成&#x226B;, &#x226A;外臺秘要&#x226B;, &#x226A;聖濟總錄&#x226B; 등에서도 狐尿刺에 대해 인식하고 있음을 언급하였고 사마귀의 독에 의하여 일으키는 피부염의 증상을 설명한 후 외치법과 내치복약을 제시하였다<sup><xref ref-type="bibr" rid="r032">32</xref></sup>.</p>
      <p>알레르기접촉피부염에 대해서는 단일 약재<sup><xref ref-type="bibr" rid="r005">5</xref>-<xref ref-type="bibr" rid="r012">12</xref></sup> 및 체질의학처방<sup><xref ref-type="bibr" rid="r013">13</xref>-<xref ref-type="bibr" rid="r015">15</xref></sup>, 그 외에 황<sup><xref ref-type="bibr" rid="r033">33</xref></sup>, 변<sup><xref ref-type="bibr" rid="r034">34</xref></sup>, 안<sup><xref ref-type="bibr" rid="r035">35</xref></sup>, 박<sup><xref ref-type="bibr" rid="r036">36</xref></sup>의 처방연구가 있었으나 사상체질의학의 관점에서 太陰人의 肝受熱裏熱病의 형태로 인식하여 연구한 것은 김<sup><xref ref-type="bibr" rid="r015">15</xref></sup>의 연구가 유일하다. 또한 김<sup><xref ref-type="bibr" rid="r015">15</xref></sup>의 연구는 肝受熱裏熱病의 燥熱證에 사용하는 처방으로 肝受熱裏熱病의 陽毒發斑과 관련한 알레르기접촉피부염의 연구는 부족하였다.</p>
      <p>陽毒證은 피부증상 외에도 咽喉痛, 微惡寒發熱, 目疼, 鼻乾 등의 熱證이 뚜렷한데<sup><xref ref-type="bibr" rid="r016">16</xref></sup> 이는 背顀表病이나 長感病 등의 胃脘受寒表寒病이나 肝受熱裏熱病의 燥熱證보다 접촉피부염의 증상에 더욱 유사하다고 판단하여 淸裏熱하면서 解肌하는 治法을 고려하였다.</p>
      <p>이를 바탕으로 저자는 본 연구에서 DNCB를 이용하여 알레르기접촉피부염을 유발시켜 체중의 변화, 귀와 비장의 무게 변화, 귀와 등 피부의 두께 변화를 측정하였고 등과 귀의 피부를 육안적으로 관찰하였으며, 5점 척도를 기준으로 증상에 따라 점수를 부여하였고, 조직병리학적으로 피부를 관찰하고, <italic>in vivo</italic> 및 <italic>in vitro</italic>에서 비장세포 증식율을 측정하였다.</p>
      <p>GST의 투여로 인한 생쥐의 체중 변화를 살펴보면, 3주째 체중 측정 결과에서는 각 군의 생쥐의 체중은 큰 차이가 없었으나 2주째의 GST-L군과 GST-H군의 체중이 Normal군과 Control군에 비해 다소간 감소한 것을 관찰하였는데 (<xref ref-type="fig" rid="f001">Fig 1</xref>), 이는 기존의 최<sup><xref ref-type="bibr" rid="r037">37</xref></sup>의 연구의 결과로 미루어 보아 GST 처방에 포함된 大黃의 영향으로 볼 수 있으며 이에 대한 후속연구가 필요할 것으로 사료된다.</p>
      <p>알레르기접촉피부염은 염증반응으로 인해 edema, 면역세포침윤, spongiosis, keratinocyte 및 fibroblast 증식으로 인한 조직 비후가 일어나는데 이로 인해 피부의 두께 및 무게의 증가가 일어날 수 있다<sup><xref ref-type="bibr" rid="r038">38</xref></sup>. 알레르기접촉피부염이 유발된 생쥐의 귀 무게의 변화를 살펴보면 Control군에 비해 GST-H군의 귀의 무게가 유의하게 감소한 것을 관찰하였다. Control군의 귀 무게의 변화는 국소 부위의 염증으로 인한 부종과 무게의 증가로 판단할 수 있으며 GST 투여군의 귀의 무게 감소가 GST가 귀의 염증 반응을 억제할 수 있음을 알 수 있다 (<xref ref-type="fig" rid="f002">Fig 2</xref>).</p>
      <p>비장은 림프기관 중 가장 큰 기관으로 B 및 T림프구, 그 외에 수많은 형질세포를 함유하고 있다<sup><xref ref-type="bibr" rid="r029">29</xref></sup>. 이러한 비장세포의 증식 혹은 무게의 증가는 림프구의 증가와 더불어 세포성 면역 및 체액성 면역 반응을 항진시킬 수 있음을 보여주는데<sup><xref ref-type="bibr" rid="r039">39</xref></sup> 비장무게의 변화를 살펴보면 Normal군에 비해 Control군의 무게가 증가하였으나 GST군은 Control군과 유의한 차이를 보이지 못하였다 (<xref ref-type="fig" rid="f003">Fig 3</xref>).</p>
      <p>부종 및 염증으로 인한 피부 두께의 감소율을 확인하여 GST의 효능을 확인하고자 귀 두께의 변화와 등 두께의 변화를 측정한 결과 두 측정결과 모두 Control군에 비해서 GST 투여군이 유의하게 두께가 감소하였다 (<xref ref-type="fig" rid="f004">Fig 4</xref>, <xref ref-type="fig" rid="f005">5</xref>). 이러한 결과로 보아 피부 두께의 증가를 GST가 효과적으로 억제하고 있다는 것을 알 수 있다.</p>
      <p>GST의 투여에 따른 비장세포의 증식률을 확인하고자 아무런 처치를 하지 않은 생쥐의 비장세포의 증식율을 관찰한 결과, 약간의 증식이 일어났지만 유의한 수준은 아니었다 (<xref ref-type="fig" rid="f006">Fig 6</xref>). 하지만 실험이 끝난 후 희생시킨 쥐의 비장세포의 증식율을 관찰한 결과, Control군과 GST 투여군을 비교한 결과 GST를 투여한 군에서 모두 Control군에 비해 감소하는 경향을 보였다 (<xref ref-type="fig" rid="f007">Fig 7</xref>). 비장무게 변화 (<xref ref-type="fig" rid="f003">Fig 3</xref>) 와 비장세포 증식률의 실험 결과를 분석하면 GST는 비장무게에 직접적인 변화를 주지는 못하였으나, 비장세포의 증식율을 억제함으로써 면역반응의 항진을 억제하는 것을 알 수 있다.</p>
      <p>각 군마다 실험 전과 실험 후의 귀와 등피부의 증상을 5점 척도로 하여 수치화한 결과, GST-L군과 GST-H군 모두 유의하게 증상이 완화되었음을 관찰하였다. 측정결과 GST의 투여는 알레르기접촉피부염의 증상을 유의성 있게 감소시켰음을 알 수 있다 (<xref ref-type="fig" rid="f008">Fig 8</xref>).</p>
      <p>피부조직에 Hematoxylin &#x26; Eosin (H&#x26;E) 을 실시하여 피부조직의 관찰을 시행한 결과 Control군은 피부의 두께가 두꺼워지고 색소가 침착되고 과립구의 증가, 각화가 진행된 것을 확인할 수 있다. GST-L군의 일부에서는 피부의 두께가 줄어든 것을 확인할 수 있고 세포변형 및 각화증상의 감소 등을 확인할 수 있다. GST-H군 또한 Control군에 비하여 각화증상, 과립구 증가, 피부 두께 감소를 확인하였다.</p>
      <p>이상과 같은 결과를 볼 때 葛根承氣湯이 알레르기접촉피부염으로 인한 피부의 염증반응을 억제하여, 알레르기접촉피부염에 의한 증상을 효과적으로 감소시켰음을 알 수 있다.</p>
    </sec>
    <sec id="s5" sec-type="conclusions">
      <title>V. 結 論</title>
      <p>葛根承氣湯(GST)이 DNCB에 의하여 유발된 알레르기접촉피부염(ACD)에 미치는 영향을 관찰한 결과 다음과 같은 결론을 얻었다.</p>
      <p><list list-type="simple"><list-item><p>1. 생쥐의 체중은 GST-L, GST-H군에서 일시적으로 감소하였지만, 실험 후 측정 결과 실험 전 체중으로 회복하였다.</p></list-item> <list-item><p>2. 생쥐의 귀 및 등 피부의 두께는 GST-L, GST-H군에서 유의하게 감소하였다.</p></list-item><list-item><p>3. 생쥐의 귀의 무게는 GST-H군에서 유의하게 감소하였다.</p></list-item><list-item><p>4. 생쥐의 비장의 무게는 Control군과 GST투여군 사이에 유의한 차이는 없었다.</p></list-item><list-item><p>5. 생쥐의 귀와 등 피부를 육안적으로 관찰한 결과, GST 투여군의 피부 증상은 현저히 감소하였으며 5점 척도의 수치 또한 감소하였다.</p></list-item><list-item><p>6. <italic>in vitro</italic> 상, Control군에 비하여 GST 투여군은 유의한 변화를 보이지 않았다.</p></list-item><list-item><p>7. <italic>in vivo</italic> 상, Control군에 비하여 GST-L군의 비장세포 증식율은 유의하게 감소하였다.</p></list-item><list-item><p>8. 조직형태학적 소견상 GST 투여군의 과각화, 색소침착, 과립증가, 부전각화증 등의 증상이 감소하였다.</p></list-item></list></p>
