        <sec>
          <title>Objectives</title>
          <p>This study aimed to quantitatively evaluate differences in abdominal palpation responses according to Sasang constitutional type using pressure pain threshold (PPT) and pressure depth (PD).</p>
        </sec>
        <sec>
          <title>Methods</title>
          <p>This retrospective cross-sectional study included 2,503 healthy adults from six Korean medicine hospitals (2020–2024). Sasang constitutional type was classified using the K-PRISM system. PPT (kgf) and PD (mm) were measured at CV4, CV12, and CV14 using a modified digital algometer. Constitutional differences among Tae-Eum, So-Yang, and So-Eum types were analyzed using nonparametric tests and linear mixed-effects models adjusted for sex, age, abdominal fat thickness, examination year, and institutional clustering.</p>
        </sec>
        <sec>
          <title>Results</title>
          <p>In unadjusted analyses, PPT and PD differed significantly across constitutional types at all acupoints. In adjusted models, the main effect of constitution on PPT was not significant overall; however, significant acupoint effects and constitution-by-acupoint interactions were observed. At CV4, So-Yang and So-Eum showed higher adjusted PPT than Tae-Eum, whereas at CV14, So-Eum showed lower PPT. For PD, constitutional differences were significant overall and in females, with So-Eum showing shallower PD than Tae-Eum and So-Yang at CV12 and CV14. These differences were more pronounced in women.</p>
        </sec>
        <sec>
          <title>Conclusions</title>
          <p>Abdominal palpation responses differed by Sasang constitutional type, with patterns varying by acupoint and sex. Quantitative assessment may provide objective supportive information for constitutional diagnosis. Further prospective studies in clinical populations are needed to verify diagnostic utility.</p>

      <title>Ⅰ. 서론</title>
      <p>복진은 한의사가 환자의 복부에서 나타나는 다양한 징후들을 직접 만져보고 관찰하여 종합적으로 평가하는 진단법이다. 복진 시 관찰되는 주요 징후는 복부의 긴장도, 압통, 복피(腹皮)의 두께, 복부의 색깔 및 형태, 복명음(腹鳴音) 등이다1.</p>
      <p>압통은 복진 시 한의사가 복부에서 관찰하는 핵심적인 징후 중 하나로, 실증적 측면이 강한 진찰법으로서 오장육부의 허실(虛實) 등의 병리적 상태를 판단하는 데 중요한 역할을 한다. 압통계(algometer)를 활용하여 압통 역치(Pressure Pain Threshold, PPT)를 측정함으로써 압통에 대한 정량화된 값을 얻을 수 있으며, 이는 진단의 객관성과 신뢰성을 높이는 데 기여한다2. 압통 역치는 통증이나 불편감을 유발하는 최소한의 압력으로, 통증이 없는 압력 자극이 통증을 동반하는 압력 감각으로 변환되는 지점을 측정한다3. 압통 역치는 기능성 소화불량증 환자의 한의학적 복증인 심하비경(心下痞硬) 진단과 깊은 상관관계를 보여 객관적 진단 보조 지표로 활용될 수 있다4. 또한 설사형 과민성 장 증후군에서 압통 역치 값은 치료 전후 복통 시각 통증 척도(VAS)와 유의한 상관관계를 보였다5. 건강한 성인 남녀를 대상으로 한 연구에서도 주관적으로 호소하는 소화 불편감이 정량적인 압통 역치 및 압입 깊이로 설명될 수 있으며, 이는 연령과 체질량지수를 보정한 후에도 유의한 차이를 보였다6.</p>
      <p>사상체질의 장부 특성 및 복진과 관련하여, 소음인은 심하부의 경결(硬結)이나 비만(痞滿), 하복부의 팽만과 구급, 복력이 연약한 것이 특징적이며, 소양인은 흉협고만(胸脇苦滿), 흉협의 경만(硬滿) 또는 결흉(結胸)이 특징적으로 나타나는데, 이는 가슴과 옆구리가 그득하고 단단한 증상을 의미한다. 태음인은 강한 복력 및 복부 표면의 긴장이 특징적이다7. 이러한 체질별 복부 특징들은 임상에서 압통과 연관성이 높아 보이나, 복부 압통의 정도를 정량화한 연구는 아직 미흡한 실정이다.</p>
      <p>이에 본 연구는 사상체질 간 복진 반응의 차이를 정량적 지표인 압통 역치 및 압입 깊이를 이용하여 분석하고, 아울러 이러한 차이가 인구학적·신체적 특성과 독립적으로 유지되는지 검증하고자 하였다.</p>
    </sec>
    <sec sec-type="methods">
      <title>Ⅱ. 연구 방법</title>
      <sec>
        <title>1. 연구 대상</title>
        <p>본 연구는 2020년부터 2024년까지 가천대학교 부속 길한방병원, 동국대학교 일산한방병원, 동신대학교 나주한방병원, 부산대학교 한방병원, 대전대학교 대전한방병원, 세명대학교 부속 충주한방병원에서 수행된 한의건강검진 자료를 이용한 후향적 단면 연구이다. 연구 대상자는 만 19세 이상의 성인으로, 인지 기능에 문제가 없고 연구 참여에 서면 동의한 자로 하였다8. 원자료 수집 단계에서는 의료기관에서 심혈관계 질환(심근경색, 울혈성심부전, 협심증, 부정맥 등), 뇌혈관질환(뇌졸중, 중풍 등), 악성 신생물(암), 정신질환(우울증, 불안장애 등), 관절염(류마티스), 갑상선질환(갑상선기능항진증, 갑상선기능저하증)으로 진단받은 자, 임신 중이거나 임신 가능성이 있는 자, 월경 중인 여성은 제외하였다. 다만 생활습관병(고혈압, 당뇨, 이상지질혈증 등) 유병자는 원자료 수집 대상에 포함하였다. 대상자는 측정 전 3일 이내 과도한 식이조절과 음주를 제한하였고, 측정 당일 흡연, 사우나 또는 목욕, 숨이 차거나 땀이 날 정도의 운동을 제한하였다. 또한 측정 4시간 전에는 물을 포함한 음식 섭취를 금하였다.</p>
        <p>본 연구의 최종 분석 대상자는 원자료 중 사상체질 진단, 신장·체중 및 체성분 분석, 복부 압통 측정이 모두 수행된 자로 하였다. 또한 분석의 동질성을 확보하기 위하여 최근 1개월 이내 약물 복용력이 없고, 최근 3년 이내 주요 질환의 병력이 보고되지 않은 건강인을 분석에 포함하였다. 사상체질 분포에서 태양인은 표본 수가 적어 통계적 비교의 안정성을 확보하기 어렵다고 판단하여 체질별 비교 분석에서는 제외하였다.</p>

      <sec>
        <title>2. 신장 · 체중 및 체성분 측정</title>
        <p>신장과 체중은 자동 신장 · 체중계(BSM370, InBody Co., Ltd., Seoul, Korea)를 이용하여 측정하였다. 대상자는 기기 위에 올라 정자세를 유지한 상태에서 약 10초간 대기하였으며, 신장과 체중은 자동으로 기록되었다. 체성분은 인바디(BWA 2.0, InBody Co., Ltd., Seoul, Korea)를 이용하여 측정하였다. 대상자는 침대에 누운 자세에서 5분간 안정을 취한 후 4점 전극 방식을 이용하여 체성분을 측정하였으며, 측정 결과는 자동으로 기록되었다.</p>
      </sec>
      <sec>
        <title>3. 복부 압력 통증 역치 및 압입 깊이 측정</title>
        <p>복부 압통 측정은 전자식 압통계(digital algometer, FDIX 50, Wagner, USA)를 개량한 복진용 압통 장치를 사용하여 수행되었다. 압입 깊이는 복부 가압 시 전자식 압통계의 수직 이동 거리를 장치 상단에 부착된 레이저 센서(OWRB4040 AAS1, Welotec, Germany)로 측정하였다. 측정자는 대상자의 우측에 위치하여 복부 표면의 해당 혈자리에 복진용 압통 장치를 수직으로 접촉시킨 후, 일정한 속도(kg/cm²/s)로 압력이 증가하도록 가압하였다. 대상자가 통증을 느끼는 시점에 직접 버저를 누르도록 하였으며, 버저가 눌리는 시점의 압력 통증 역치(kgf)와 압입 깊이(mm)는 측정 프로그램을 통해 자동으로 기록되었다. 측정자는 버저음을 확인하는 즉시 가압을 중단하였다. 대상자는 충분히 이완된 상태에서 양측 무릎을 편 채 앙와위로 누워 5분간 휴식한 후 측정을 시행하였다. 측정 부위는 상복부, 중복부, 하복부를 대표하는 경혈인 거궐(巨闕, CV14), 중완(中脘, CV12), 관원(關元, CV4)이었다.</p>
      </sec>
      <sec>
        <title>4. 사상체질 분류</title>
        <p>대상자의 사상체질 분류를 위해 한국한의학연구원에서 개발한 사상체질 통합 분석 시스템(K-PRISM)을 사용하였다. 본 시스템은 안면, 음성, 체형, 설문 등 4가지 영역의 생체 데이터를 수집하여 통합적인 체질 확률을 산출한다. 안면 데이터 수집은 일정한 조도와 배경 조건이 갖춰진 환경에서 대상자의 정면 및 측면 사진을 촬영하였으며, 시스템은 촬영된 이미지에서 안면 윤곽 및 이목구비의 형태학적 특징점을 자동으로 추출하여 분석에 활용하였다. 음성 데이터는 외부 소음이 제어된 독립된 공간에서 고감도 마이크를 통해 5개 단모음('아, 에, 이, 오, 우')과 지정된 표준 문장을 발성하도록 하여 체질별 음향학적 변수들을 추출하였다. 체형 분석은 숙련된 측정자가 줄자를 이용하여 대상자의 주요 8개 부위(이마, 목, 겨드랑이, 가슴, 늑골, 허리, 장골, 곡골) 둘레를 직접 측정하여 시스템에 입력하였다. 아울러 대상자의 심리적 성향과 평소 생리적 특성을 파악하기 위해 시스템 내에 내장된 사상체질 진단 설문지(KS-15)를 작성하게 하였다. 최종적으로 수집된 4가지 영역의 데이터는 통합 알고리즘에 적용되어 태양인, 태음인, 소양인, 소음인 각각에 대한 확률값으로 산출되었으며, 산출된 네 체질 확률 중 최댓값을 나타내는 체질을 해당 대상자의 최종 사상체질로 분류하였다. 체질 확률값 자체는 분석 가중치로 반영하지 않았다.</p>
      </sec>
      <sec>
        <title>5. 통계 분석</title>
        <p>연속형 변수는 평균과 표준편차 또는 중앙값과 사분위수 범위(IQR, interquartile range)로, 범주형 변수는 빈도와 백분율로 제시하였다. 사상체질에 따른 인구학적 특성 및 측정 특성의 차이를 비교하기 위하여 범주형 변수는 카이제곱 검정(Chi-square test) 또는 Fisher의 정확 검정(Fisher's exact test)을 사용하였고, 연속형 변수는 Shapiro-Wilk 검정을 통해 정규성을 평가한 후 정규성을 만족하는 경우 일원분산분석(one-way ANOVA)을, 정규성을 만족하지 않는 경우 Kruskal- Wallis 검정을 사용하였다.</p>
        <p>체질별 비교 분석에서 태양인은 표본 수가 충분하지 않아 분석에서 제외하였다. 이는 태양인의 낮은 유병률로 인한 표본 불균형 문제를 방지하고 통계 모델의 수렴 안정성과 결과의 신뢰도를 제고하기 위함이다. 최종 분석은 태음인, 소양인, 소음인을 대상으로 수행하였다. 체질별 차이가 성별에 따라 달라지는지를 평가하기 위해 남녀를 층화하여 동일한 분석을 각각 수행하였다. 각 혈위의 압입 깊이 및 압통 역치에 일부 결측이 존재하여, 변수별로 해당 측정값이 존재하는 대상자만 포함한 완전 사례 분석(complete-case analysis)을 수행하였다. 이에 따라 Table 2, 3의 체질별 및 성별 층화 표본 수는 혈위 및 변수에 따라 상이하며, 각 분석 결과는 해당 변수에서 관측값이 있는 표본을 대상으로 해석하였다. 압통 역치 및 압입 깊이에 대한 체질간 분포 차이를 탐색하기 위해, 각 혈위별로 Kruskal-Wallis 검정을 수행하였으며, 사후 분석으로 Bonferroni 보정을 적용한 Dunn 검정을 실시하였다. 비모수 분석 결과는 중앙값과 IQR로 제시하였다.</p>
        <p>주요 분석으로 복부 3개 혈위에서 반복 측정된 압통 역치와 압입 깊이는 각각 종속변수로 하는 선형 혼합효과모형(linear mixed-effects model)을 별도로 적합하였다. 고정효과로는 체질, 측정부위, 체질과 측정부위 간 상호작용항을 포함하였고, 성별, 연령, 복부 지방 두께, 측정연도를 공변량으로 추가하여 보정하였다. 동일 대상자 내 반복 측정으로 인한 상관 구조를 반영하기 위해 대상자 수준의 랜덤 절편을 포함하였다. 또한 다기관 및 다년도 자료 수집에 따른 군집 구조를 고려하여 기관 수준과 기관-연도 조합에 대한 랜덤 절편을 추가하여 기관 간 변동 및 기관 내 연도 간 변동을 모형에 반영하였다. 고정효과 계수의 제시는 해석 편의를 위해 압통 역치 모형과 압입 깊이 모형 모두에서 태음인과 관원(關元, CV4)을 기준범주로 설정하였다. 기준범주의 설정은 계수의 표시 방식에만 영향을 주며, Type III 검정에 따른 전체 효과, 상호작용 효과 및 추정 주변평균 기반의 사후 비교 결과에는 영향을 주지 않는다. 모형 모수는 제한최대우도법(restricted maximum likelihood)으로 추정하였으며, 고정효과의 유의성은 Satterthwaite 자유도 근사법을 적용한 Type III 분산분석으로 평가하였다. 사후 비교는 추정된 주변평균(estimated marginal means)을 기반으로 다중비교 보정을 적용하여 수행하였다. 보정 평균과 군 간 비교는 주변평균 추정 방법을 이용하여 산출하였으며, 체질간 쌍별 비교 결과는 평균 차이(Δ)와 표준오차(standard error, SE), 95% 신뢰구간으로 제시하였다. 다중 비교 보정은 Tukey 방법을 적용하였다. 선형 혼합모형 결과는 보정 평균과 95% 신뢰구간으로 제시하였다.</p>
        <p>성별에 따른 체질 효과의 이질성을 탐색하기 위해, 위의 주요 분석 모형을 남성과 여성으로 각각 층화하여 동일한 모형 구조로 재적합하였다. 층화 분석에서는 성별을 공변량에서 제외하였으며, 그 외 고정효과 구조 및 랜덤효과 구조는 전체 모형과 동일하게 유지하였다. 다만 성별 층화 후 소양인 남성 및 소음인 남성 등 일부 셀의 표본 수가 상대적으로 제한적이었으므로, 성별 층화 분석은 전체 분석을 보완하는 탐색적 분석으로 해석하였다. 모든 통계 검정은 양측 검정으로 수행하였으며, 유의수준은 0.05로 하였다. 모든 통계 분석은 R version 4.5.0 (R Foundation for Statistical Computing, Vienna, Austria)을 사용하여 수행되었다.</p>
      </sec>
    </sec>
    <sec sec-type="results">
      <title>Ⅲ. 연구 결과</title>
      <p>전체 원자료는 총 12,840명이 포함되어 있었다. 사상체질 진단 결과가 있는 대상자는 8,518명이었으며, 이 중 체성분 자료가 확보된 경우는 8,425명, 복진기 측정 자료가 있는 경우는 4,605명이었다. 최근 1개월 이내 약물 복용력이 없고 3년 이내 병력이 보고되지 않은 건강인은 총 2,503명이었다. 이 중 사상체질 분포에서 빈도가 극히 적어 통계적 분석이 어려운 태양인(n=58, 2.3%)을 제외한 2,445명을 최종 분석 대상으로 하였다. 따라서 기술통계 및 전체 경향 분석은 건강인 2,503명을 대상으로, 사상체질에 따른 복부 압통 역치 및 압입 깊이 비교 분석은 태양인을 제외한 2,445명을 대상으로 수행하였다.</p>
      <sec>
        <title>1. 일반적 특성</title>
        <p>총 2,503명 중 태양인 58명, 태음인 1,296명, 소양인 622명, 소음인 527명이 포함되었다. 성별 분포는 전체적으로 여성의 비율이 높았으며, 특히 소양인(86.0%)과 소음인(82.2%)에서 여성 비율이 두드러졌다. 연령 중앙값은 태양인 49.0세(IQR 33.0–55.0), 태음인 43.0세(IQR 30.5–53.0), 소양인 44.0세(IQR 33.0–53.0), 소음인 49.0세(IQR 36.0–57.0)로 체질 간 유의한 차이를 보였다. BMI 중앙값은 태음인이 26.1 kg/m²(IQR 24.3–28.1)로 가장 높았으며, 소양인 21.8 kg/m²(IQR 20.6–23.5), 소음인 20.1 kg/m²(IQR 19.0–21.5) 순으로 낮았다(Table 1).</p>

        <title>2. 체질 별 관원(關元, CV4), 중완(中脘, CV12), 거궐(巨闕, CV14) 혈위의 압통 역치 및 압입 깊이</title>
        <p>태양인을 제외한 2,445명 대상자에서 체질에 따른 복부 혈위별 압통 역치와 압입 깊이를 비교한 결과, 관원(關元, CV4), 중완(中脘, CV12), 거궐(巨闕, CV14)혈 모두에서 체질 간 유의한 차이를 보였다.</p>
        <p>관원(關元, CV4)에서는 소양인의 압통 역치 중앙값이 6.9 kgf(IQR 5.0–8.7)로 태음인 6.1 kgf(IQR 4.2–8.4)에 비해 유의하게 높았다. 중완(中脘, CV12)에서는 소양인 5.8 kgf(IQR 4.4–7.5)과 태음인 5.6 kgf(IQR 3.9–7.4)이 소음인 5.2 kgf(IQR 3.8–6.8)보다 유의하게 높은 역치를 보였으며, 소양인은 태음인보다도 다소 높은 경향을 나타냈다. 거궐(巨闕, CV14)에서는 소양인 5.8 kgf(IQR 4.5–7.2)과 태음인 5.8 kgf(IQR 4.2–7.7)이 소음인 5.0 kgf(IQR 3.8–6.7)보다 유의하게 높은 압통 역치를 보였다.</p>
        <p>여성에서 관원(關元, CV4), 중완(中脘, CV12), 거궐(巨闕, CV14) 모두에서 소양인이 태음인보다 높은 압통 역치를 보였다. 관원(關元, CV4), 거궐(巨闕, CV14)에서 소양인은 소음인보다 높은 압통 역치를 보였다. 남성에서는 관원(關元, CV4)과 중완(中脘, CV12)에서 체질 간 차이가 유의하지 않았으나, 거궐(巨闕, CV14)에서는 소양인(7.1 kgf(IQR 5.7–8.7))과 태음인(7.1 kgf(IQR 5.3–9.2))이 소음인(5.8 kgf(IQR 4.5–7.4))보다 유의하게 높은 압통 역치를 나타냈다(Table 2).</p>

        <p>압입 깊이 역시 관원(關元, CV4), 중완(中脘, CV12), 거궐(巨闕, CV14) 혈위에서 체질에 따라 유의한 차이를 보였다. 중앙값 기준으로 관원(關元, CV4)에서 태음인과 소양인은 각각 36.0 mm(IQR 31.0–42.0), 36.0 mm(IQR 31.0–42.0)로 유사하였으나, 소음인은 34.0 mm(IQR 30.0–40.0)로 유의하게 낮았다. 중완(中脘, CV12)에서도 태음인 35.0 mm(IQR 31.0–40.0), 소양인 34.0 mm(IQR 30.0–40.0), 소음인 33.0 mm(IQR 28.0–37.0) 순으로 감소하였으며, 태음인과 소양인에 비해 소음인이 유의하게 얕은 압입 깊이를 보였다. 거궐(巨闕, CV14)에서도 태음인(34.0 mm, IQR 29.0–40.0)과 소양인(33.0 mm, IQR 29.0–38.0)에 비해 소음인(31.0 mm, IQR 24.0–35.0)의 압입 깊이가 유의하게 낮았다.</p>
        <p>성별로 분석한 결과, 여성에서는 거궐(巨闕, CV14), 중완(中脘, CV12), 관원(關元, CV4)혈 모두에서 체질 간 압입 깊이에 유의한 차이가 관찰되었으며, 공통적으로 소음인이 태음인 및 소양인보다 얕은 압입 깊이를 나타냈다. 남성에서는 관원(關元, CV4)과 중완(中脘, CV12)에서 체질 간 유의한 차이가 없었으나, 거궐(巨闕, CV14)에서는 소음인이 태음인 및 소양인보다 유의하게 낮은 압입 깊이를 보였다(Table 3).</p>

        <p>압통 역치에 대한 선형혼합효과모형 분석 결과, 체질의 주효과는 전체 및 여성에서 유의하지 않았으나, 남성에서 유의하였다. 혈위 효과와 사상체질×혈위 상호작용은 모두 유의하였다(모두 p&lt;0.001). 이는 사상체질에 따른 압통 역치 차이가 혈위마다 다른 방향과 크기로 나타남을 의미하며, 구체적인 양상은 혈위별 사후분석에서 다뤘다. 공변량 중에서는 성별과 연령이 유의한 영향을 보였으며, 복부 지방 두께도 전체 및 여성 분석에서 유의한 변수로 확인되었다. 반면 측정연도는 유의한 영향을 보이지 않았다. 혈위별로는 관원(關元, CV4)이 중완(中脘, CV12)과 거궐(巨闕, CV14)보다 높은 압통 역치를 보였다 (Table 4).</p>
        <p>혈위별 보정 평균을 이용하여 체질 간 압통 역치를 비교한 결과, 전체 분석에서 관원(關元, CV4)에서는 소양인이 태음인보다 유의하게 높은 압통 역치를 보였다. 중완(中脘, CV12)에서는 체질 간 유의한 차이가 관찰되지 않았다. 반면 거궐(巨闕, CV14)에서는 태음인과 소양인이 소음인보다 높은 압통 역치를 보였으며, 태음인도 소양인보다 높은 경향을 보였다(Table 5).</p>
        <p>성별 층화 분석에서는 여성에서 체질 간 차이가 보다 뚜렷하게 나타났다. 여성에서는 관원(關元, CV4)에서 소양인과 소음인이 태음인보다 높은 압통 역치를 보였고, 중완(中脘, CV12)에서는 소양인이 소음인보다 높은 값을 보였으며, 거궐(巨闕, CV14)에서는 태음인이 소음인보다 높은 압통 역치를 보였다. 남성에서는 대부분의 혈위에서 체질 간 유의한 차이가 관찰되지 않았으나, 거궐(巨闕, CV14)에서 태음인이 소양인 및 소음인보다 높은 압통 역치를 보였다(Table 5).</p>
        <p>보정 평균 결과를 종합하면, 소음인은 중완(中脘, CV12)과 거궐(巨闕, CV14)에서 상대적으로 낮은 압통 역치를 보인 반면, 관원(關元, CV4)에서는 오히려 높거나 유사한 수준을 나타내 혈위에 따라 상반된 양상을 보였다. 이러한 차이는 여성에서 비교적 뚜렷하였으며, 남성에서는 변동성이 크고 체질 간 차이가 제한적으로 나타났다.</p>

        <p>혈위별 주효과 비교에서는 하복부 기준 혈위인 관원(關元, CV4)에 비해 상복부 방향으로 이동할수록 압입 깊이가 감소하는 패턴이 관찰되었다. 전체 분석에서 중완(中脘, CV12)은 관원(關元, CV4)에 비해 평균 0.92 mm 얕았으며, 거궐(巨闕, CV14)은 평균 2.76 mm 더 얕은 압입 깊이를 나타냈다(Table 6). 이러한 혈위별 경향은 남성에서도 대체로 유사하게 나타났으나, 여성에서는 CV12와 CV4 사이의 차이가 통계적으로 유의하지 않아 혈위 간 기울기가 상대적으로 완만하게 나타났다. 사상체질과 혈위 간의 상호작용항은 소음인의 압입 깊이 차이가 관원(關元, CV4)보다 중완(中脘, CV12) 및 거궐(巨闕, CV14)에서 더 두드러지는 혈위 특이적 양상을 시사하였다.</p>
        <p>각 혈위별 보정 평균을 비교한 결과(Table 7), 관원(關元, CV4)에서는 체질 간 유의한 차이가 관찰되지 않았다. 중완(中脘, CV12)에서는 태음인과 소양인이 소음인보다 유의하게 깊은 압입 깊이를 보였고, 거궐(巨闕, CV14)에서도 동일하게 태음인과 소양인이 소음인보다 더 깊은 압입 깊이를 나타냈다. 성별 층화 분석에서는 여성에서 이러한 경향이 뚜렷하였으며, 남성에서는 거궐(巨闕, CV14)에서만 소양인이 소음인보다 깊은 압입 깊이를 보였다(Table 7).</p>

      <title>Ⅳ. 고찰</title>
      <p>본 연구에서는 사상체질에 따른 복부 압통 역치와 압입 깊이의 차이가 관원(關元, CV4), 중완(中脘, CV12), 거궐(巨闕, CV14)에서 동일한 방향으로 나타나는 것이 아니라 혈위에 따라 상이하게 나타났으며, 이러한 경향은 성별, 연령, 복부 지방 두께, 측정연도 및 기관 효과를 보정한 이후에도 일부 유지되었다. 특히 압통 역치에서는 체질의 전체 주효과보다 체질과 혈위 간 상호작용이 두드러졌고, 압입 깊이에서는 소음인이 중완(中脘, CV12)과 거궐(巨闕, CV14)에서 상대적으로 얕은 반응을 보였다. 이러한 차이는 여성에서 보다 명확하였으며, 남성에서는 일부 혈위를 제외하면 제한적으로 관찰되었다. 이는 사상체질에 따른 복진 반응의 차이가 단순한 체격 차이에 국한되지 않고, 특정 복부 부위에서 선택적으로 발현될 가능성을 시사한다.</p>
      <p>이러한 결과는 사상체질 진단이 체형, 용모, 음성, 성정 및 소증 등을 종합하여 이루어진다는 기존 사상체질의학의 진단 원칙과 일정 부분 부합한다9,10. 이러한 점을 고려하면, 본 연구에서 관찰된 소음인의 낮은 압통 역치와 얕은 압입 깊이는 소음인의 복력 약화와 예민한 복부 반응이라는 문헌적 기술과 연결하여 해석할 수 있다. 또한 태음인과 소양인에서 일부 혈위의 압통 역치 또는 압입 깊이가 상대적으로 높게 나타난 결과는 각 체질의 복부 긴장도와 충실도의 차이가 정량 지표로 반영된 것으로 볼 수 있다. 다만 이러한 해석은 체질별 복부 반응의 경향을 설명하는 수준으로 이해해야 하며, 특정 체질의 병리 상태를 직접적으로 단정하는 근거로 확대해서는 안 된다.</p>
      <p>본 연구의 결과는 정량적 복진 지표 및 통증 과민성을 다룬 선행 연구들의 보고와 전반적으로 일관된 경향성을 나타낸다. 압통 역치는 디지털 압통계를 이용하여 비교적 객관적으로 측정할 수 있으며, 반복 측정에서도 양호한 재현성을 보인 바 있다11. 또한 건강인을 대상으로 한 대규모 연구에서는 압통 역치의 성별 특이 기준값이 제시되었고, 압박 속도와 측정 조건의 표준화가 재현성 확보에 중요하다고 보고되었다12. 한편 중완(中脘, CV12) 가압에 대한 초음파 연구에서는 위와 위 주변 조직, 특히 위 후벽에 압력이 집중될 때 통증이 발생한다고 하였으며, 기능성 소화불량 환자에서는 식후 위 팽창 과민성이 식후 증상 중증도와 관련되는 것으로 보고되었다13,14. 따라서 본 연구에서 중완(中脘, CV12)과 거궐(巨闕, CV14)에서 체질 차이가 상대적으로 뚜렷하게 관찰된 결과는 상복부 압박 반응의 해부학적·기능적 의미를 시사하는 소견으로 이해할 수 있다.</p>
      <p>아울러, 성별에 따른 반응성의 차이는 본 연구 결과가 지닌 임상적 의미를 구체화하는 근거이다. 본 연구에서는 여성에서 압통 역치와 압입 깊이 모두 체질 간 차이가 보다 뚜렷하게 관찰된 반면, 남성에서는 일부 혈위를 제외하고 유의한 차이가 제한적이었다. 이는 건강인을 대상으로 한 기존 연구에서 압통 역치의 기준값이 성별에 따라 다르게 제시된 점과, 여성에서 임상통 및 실험통에 대한 민감도가 전반적으로 더 높게 보고된 점과도 부합한다12,15. 따라서 향후 복진의 정량화 또는 체질별 기준값 설정에서는 성별 층화가 필수적으로 고려되어야 한다. 아울러 본 연구 결과는 복진에서 사용되어 온 ‘아프다’, ‘깊다’, ‘얕다’와 같은 주관적 표현을 kgf와 mm의 수치로 전환하였다는 점에서 의미가 있으나, 현 단계에서는 체질 진단의 독립적 판정 기준이라기보다 체질별 복부 반응의 경향을 보조적으로 제시하는 지표로 해석하는 것이 타당하다.</p>
      <p>본 연구의 장점은 대규모 다기관 건강인 자료를 이용하여 복부 압통 역치와 압입 깊이를 동시에 분석하였고, 혼합효과모형을 적용하였다는 데 있다. 반면 후향적 단면 연구라는 설계상 체질과 복진 반응 간 인과관계를 규명할 수 없고, 연구 대상이 건강인 중심이므로 실제 임상 환자군으로의 일반화에는 제한이 있다. 또한 압통 역치와 압입 깊이는 대상자의 주관적 통증 인지 시점에 영향을 받으며, 태양인은 표본 수 부족으로 분석에서 제외되었다. 특히 성별 층화 분석에서 남성 소양인 및 소음인 표본 수가 상대적으로 제한되어 남성 층화 결과의 검정력과 추정 안정성에는 한계가 있으므로, 해당 결과는 탐색적으로 해석할 필요가 있다. 또한 본 연구는 임맥 상의 관원(關元, CV4), 중완(中脘, CV12), 거궐(巨闕, CV14)만을 측정하여 상복부, 중복부, 하복부의 수직적 복진 반응을 평가하였으므로, 소양인의 흉협고만이나 태음인·소음인의 대장 병리와 관련될 수 있는 천추(天樞, ST25), 기문(期門, LR14) 등 측복부 및 흉협부 혈위의 반응은 반영하지 못하였다. 체질 분류 역시 K-PRISM의 확률 기반 분류에 의존하였으므로 잠재적 오분류 가능성을 완전히 배제할 수 없다. 특히 최종 체질 확률이 서로 근접한 대상자에서는 소양인과 소음인 간 분류 불확실성이 결과의 방향과 크기에 영향을 미쳤을 가능성이 있으며, 본 연구에서는 체질 확률값을 이용한 가중 분석이나 고확률군 제한 민감도 분석을 수행하지 못하였다. 따라서 향후 연구에서는 천추(天樞, ST25), 기문(期門, LR14) 등 추가 혈위를 포함하여 체질별 복진 반응의 부위 특성을 보다 포괄적으로 평가하고, 체질 확률 상위군에 대한 민감도 분석, 체질 확률을 반영한 가중모형, 기능성 소화불량이나 과민성 장증후군 등 복증이 뚜렷한 환자군을 포함한 전향적 연구를 통해 체질과 질환의 상호작용을 검증하고, 성별 및 혈위별 기준값, 재현성, 진단 성능을 함께 제시하는 방향으로 발전시킬 필요가 있다.</p>
