        <sec>
          <title>Objectives:</title>
          <p>The purpose of this study was to systematically analyze and examine the relationship between the number of lost teeth and quality of life (QoL) among elderly.</p>
        </sec>
        <sec>
          <title>Methods:</title>
          <p>Papers published in Korean and English between 2000 and 2020 were searched. Relevant papers were selected by two independent readers according to predefined exclusion criteria. Quality assessment and consistency tests were conducted using ROBIS.</p>
        </sec>
        <sec>
          <title>Results:</title>
          <p>Twelve of the 854 studies were targeted, and the analysis confirmed that tooth loss was related to a decrease in the QoL in 11 studies. Tooth loss negatively affected the QoL among the elderly. However, one study indicated that the QoL and number of teeth were not related. Rather, dry mouth affected the QoL, and cavities caused by oral dryness led to tooth loss. Most of the selected studies indicated that the QoL of elderly individuals with a single tooth was higher than that of those with no teeth.</p>
        </sec>
        <sec>
          <title>Conclusions:</title>
          <p>This study provides strong evidence that tooth loss in the elderly is associated with impairment of QoL.</p>

      <title>서론</title>
      <p>경제 수준 향상과 의학의 발전으로 인해 현대인의 수명은 꾸준히 향상되고 있고, 그 결과로 인하여 노령인구가 증가하고 있는 현상은 전 세계적인 추세이다. 최근 웰빙(well-being)의 개념을 넘어 웰다잉(well-dying)을 말할 정도로 단순히 오래 사는 것이 아니라 주관적인 행복과 만족감을 중요시하는 삶의 질(quality of life, QoL)에 대하여 관심이 많아지고 있다. 삶의 질은 다양한 요인들이 복합적으로 작용하기에 단순한 요인을 특정하여 결론을 내리기가 어렵지만, 그 중 가장 큰 영향을 주는 요인은 건강문제이다<xref ref-type="bibr" rid="r001">[1]</xref>.</p>
      <p>특히 구강건강은 발음과 외모에 관련이 있어 대인관계와 원활한 사회생활에 적지 않은 영향을 미친다. 노인에게 흔히 볼 수 있는 치아 수의 감소나 만성 치주질환 같은 구강건강의 악화는 저작의어 려움과 영양섭취의 저하로 이어져 전신건강에 나쁜 영향을 미칠 수 있으며<xref ref-type="bibr" rid="r002">[2</xref>,<xref ref-type="bibr" rid="r003">3]</xref> 이로 인해 삶의 질이 낮아지는 결과를 초래할 수 있기에 구강건강의 유지는 삶의 질에 매우 중요하다<xref ref-type="bibr" rid="r004">[4]</xref>. 최와 정<xref ref-type="bibr" rid="r005">[5]</xref>의 연구에서 구강건강이 삶의 질에 미치는 영향이 20대에서는 19.8%였던 것이 60대 이상에서는 64.5%로 3배 이상 증가하는데, 이는 연령이 증가할수록 구강건강이 더욱 삶의 질에 밀접한 관련이 있다는 것을 뒷받침한다. 따라서 구강건강과 관련된 삶의 질을 측정하는 것은 전반적인 행복을 측정하는 평가지표가 될 수 있다<xref ref-type="bibr" rid="r006">[6]</xref>.</p>
      <p>이처럼 구강건강이 삶의 질에 미치는 영향을 평가하기 위하여 구체적인 구강건강관련 삶의 질(oral-health-related quality of life, OHRQoL) 측정 도구<xref ref-type="bibr" rid="r007">[7]</xref>가 개발되어 왔는데, 그 중 가장 보편적으로 사용되는 도구는 Slade<xref ref-type="bibr" rid="r008">[8]</xref>의 Oral health impact profile (OHIP)나 단축형인 OHIP-14<xref ref-type="bibr" rid="r009">[9]</xref>, Geriatric oral health assessment index (GOHAI)<xref ref-type="bibr" rid="r010">[10]</xref>, Euro QoL(EQ-5D)<xref ref-type="bibr" rid="r011">[11]</xref> 이다.</p>
      <p>Marino 등<xref ref-type="bibr" rid="r012">[12]</xref>과 Tsakos 등<xref ref-type="bibr" rid="r013">[13]</xref>의 연구에 따르면 우식치아의 수, 치주낭 깊이와 같은 치주 관련 임상 지표가 OHRQoL의 감소와 강하게 연관되지 않음을 보고하였지만, 치아 우식과 치주질환은 진행적인 과정이며 적절히 치료하지 않으면 치아 상실을 초래한다. 그리고 치아 상실은 위치나 수에 따라 기능적 손상을 일으킬 수 있으며, 이는 궁극적으로 삶의 질에 영향을 미칠 수 있다.</p>
      <p>몇몇 인구 조사에서는 OHRQoL에 대한 영향을 분석하는 통계적 모델에 치아 수를 포함하여 조사하였는데, Ekanayake와 Perera<xref ref-type="bibr" rid="r014">[14]</xref>의 스리랑카 노인을 대상으로 진행한 연구에서는 치아의 수보단 환자의 나이, 성별 또는 문화적 배경과 같은 다른 요소들이 삶의 질에 중요한 역할을 한다는 것을 암시하였다. 그러나 이와 대조적으로 대부분의 다른 연구에서는 상실된 치아의 수가 삶의 질에 강한 상관관계가 있다고 말한다. 예를 들면, 노인의 구강건강 관련 삶의 질을 측정하는 도구인 GOHAI가 좋을수록 저작이 가능한 기능치아를 많이 보유하고<xref ref-type="bibr" rid="r015">[15]</xref>, 자연치의 존재 및 보존이 성취감, 자부심, 더 나은 구강 기능과 같은 전체적인 삶의 질에 기여하며<xref ref-type="bibr" rid="r016">[16]</xref>, 단단한 음식을 씹을 때 문제가 있으면 GOHAI가 낮아진다는 것이다<xref ref-type="bibr" rid="r017">[17]</xref>.</p>
      <p>이와 같이 구강상태와 저작기능 또는 음식섭취의 관계, 구강상태와 구강건강관련 삶의 질의 관계에 대한 국내외 연구들은 많이 보고되어 왔다. 2010년 Gerritsen 등<xref ref-type="bibr" rid="r018">[18]</xref>은 치아 상실과 삶의 질의 관계에 대해 조사하였고, 최근 우리나라에서는 노인의 구강건강과 관련한 삶의 질에 대한 체계적 문헌고찰<xref ref-type="bibr" rid="r019">[19]</xref>이 진행되었지만, 치아 수를 특정하여 삶의 질을 비교한 최근 연구는 찾기 힘들다. 따라서 본 연구의 목적은 최근 20년간의 연구를 종합한 체계적 문헌고찰을 통하여 노인의 치아 개수와 삶의 질을 분석하고자 하며, 본 연구는 노인의 구강건강 관리의 필요성을 뒷받침하는 정책자료의 충분한 근거가 될 것으로 사료된다.</p>
    </sec>
    <sec id="s2" sec-type="other">
      <title>본론</title>
      <sec id="s2a">
        <title>1. 연구대상 및 방법</title>
        <sec id="s2aa">
          <title>1) 연구설계</title>
          <p>본 연구는 노인을 대상으로 치아의 개수가 삶의 질과 관련이 있는가를 분석하여 20년간의 연구를 종합한 체계적 문헌고찰 연구이다. Cochrane에서 제시하고 있는 체계적 문헌고찰 가이드라인<xref ref-type="bibr" rid="r020">[20]</xref> 및 Preferred reporting items for systematic reviews and meta-analysis (PRISMA) 지침<xref ref-type="bibr" rid="r021">[21]</xref>에 따라 진행하였다&#x3C;<xref ref-type="fig" rid="f001">Fig. 1</xref>&#x3E;.</p>

          <title>2) 검색전략</title>
          <p>분석에 필요한 연구를 찾기 위해 국내 데이터베이스는 KoreanMed, RISS, 국외 데이터베이스는 Medline, PubMed, CINAHL, Cochrane Library를 이용하였다. 출판 년도는 2000년 1월부터 2020년 1월 사이에 출판된 논문으로 제한하였다.</p>
          <p>노인의 치아 수에 따른 삶의 질의 관계를 확인하기 위해 Patient, Intervention, Comparison, Outcome-Study Design (PICO-SD) 형식에 따라 대상(P)은 노인, 비교군(C)은 치아 개수, 성과(O)는 삶의 질로 구성하였으며, 사전연구검색 및 Medical Subject Headings (MeSH) term을 이용하여 검색어를 확정했다. 국내 데이터베이스의 검색어는 (삶의 질 OR 환자만족도 OR 구강건강관련 삶의 질) AND (자연치아 OR 상실치아 OR 잔존치아 OR 치과 임플란트 OR 치과 보철) AND (노령 OR 노인 OR 고령 OR 고령자), 국외 데이터베이스의 검색어는 (old people OR elderly [MeSH] OR aged [MeSH]) AND (natural tooth OR tooth loss [MeSH] OR remaining tooth OR dental implants [MeSH] OR dental prostheses [MeSH] OR dental prosthesis [MeSH] OR dental implantation [MeSH]) AND (QoL [MeSH] OR life quality [MeSH] OR patient satisfaction OR oral health related QoL)로 검색을 진행하였다. 수집된 연구들은 서지 프로그램 Mendeley (Mendeley Ltd., Elsevier, UK)를 이용하여 관리하였다.</p>
        </sec>
        <sec id="s2ac">
          <title>3) 연구선택 및 배제기준</title>
          <p>대상자는 65세 이상 노인으로 지정하였으며, 선별된 연구가 광범위한 연령층을 대상자로 조사한 경우 65세 이상 대상자의 결과만을 추출하였다. 대상자들의 치아 수를 수량화 하고, 이와 관련한 삶의 질을 측정한 연구를 검색하였고 6개의 데이터베이스에서 849개의 연구가 검색되었으며 검색된 연구의 참고문헌에서 관련성이 높은 5개의 논문을 추출 및 추가하여 총 854개의 연구가 수집되었다. Mendeley에서 제공하는 중복제거 기능을 통해 중복되는 50개의 연구를 제외하였다. 수집된 연구는 제목 및 초록을 평가하여 1차로 선별하였으며 전문(full text)을 분석하여 2차로 선별하였다. 1차 선별에서 737개, 2차선별에서 45개가 제외되어 최종 24개 문헌의 질 평가를 진행하였다. 2차 선별 과정은 2인의 연구자가 독립적으로 진행하였으며 의견이 불일치할 경우 PICO 및 제외 기준에 따라 재검토하여 조율하였다. 연구자 간의 일치도(kpapa index)는 0.81이다.</p>
          <p>연구 문헌의 제외 기준은 다음과 같다. 1) 암 및 전신질환 환자가 대상인 경우 2) 대상자가 65세 미만인 경우 3) 틀니 사용자인 경우 4) 원문을 찾을 수 없는 경우 5) 삶의 질을 측정하지 않은 경우 6) 치아 수를 조사하지 않은 경우이다.</p>
        </sec>
        <sec id="s2ad">
          <title>4) 질 평가</title>
          <p>질 평가는 Penny 등<xref ref-type="bibr" rid="r022">[22]</xref>이 개발한 Risk of bias in systematic reviews (ROBIS)를 이용하였다. 도구에 제시되어 있는 평가항목은 4가지가 있으며 연구선정 자격 기준(study eligibility criteria), 연구의 식별 및 선택(identification and selection of studies), 데이터 수집과 연구에 대한 평가(data collection and study appraisal), 종합 및 결론(synthesis and findings)으로 나뉜다. 질 평가의 등급은 High concern, Low concern, Unclear concern 3가지가 있으며 Low concern는 &#x2018;편향된 결과를 도출할 것 같지 않음&#x2019;을, High concern는 &#x2018;편향된 결과를 산출할 가능성이 있음&#x2019;을, Unclear concern는 &#x2018;편향의 위험을 판단하기 위해 보고된 정보가 불충분&#x2019;을 나타낸다. 두 평가자가 전문을 읽고 질 평가를 시행하였다. 최종 결과가 Low concern으로 나온 연구들을 채택하여 분석을 진행하였으며 두 연구자가 부여한 등급이 다를 경우 논의를 거쳐 합의된 등급으로 결정하였다. 최종 분석 연구는 12개의 연구가 선택되었다&#x3C;<xref ref-type="fig" rid="f002">Fig. 2</xref>&#x3E;.</p>

          <title>5) 데이터 추출 및 분석</title>
          <p>선택된 12개의 연구는 두 연구자가 전문을 읽어 대상자의 일반적 특성(표본 수, 연령), 치아의 개수, 대상자의 수량화 된 삶의 질 지수를 추출하였으며 제1저자명, 출판 년도, 연구 설계를 조사하였다. 추출된 데이터는 표로 정리하여 연도, 지역, 독립변수, 측정도구, 결과를 비교하여 분석하였다.</p>
        </sec>
      </sec>
      <sec id="s2b">
        <title>2. 문헌의 일반적 특성</title>
        <p>12개의 논문 중 2000년부터 2010년 사이에 출판된 논문은 2편, 2010년부터 2020년 사이에 출판된 논문은 10편이었으며, 한국에서 4편, 네덜란드, 일본, 브라질에서 2편, 폴란드와 스페인에서 1편씩 진행되었다.</p>
        <sec id="s2ba">
          <title>1) 연구대상</title>
          <p>연구 대상은 지역사회 주민 및 노인 복지시설 이용자를 대상으로 진행된 연구가 10편, 정부에서 조사한 국민건강영양조사 자료가 2편이었다. 대상자 수는 세 구간으로 나누었으며 대상자 수가 200명 이하인 논문은 4편, 201명 이상 400명 이하의 논문은 3편, 401명 이상인 논문은 5편이며 총 연구대상자는 26,368명이다.</p>
        </sec>
        <sec id="s2bb">
          <title>2) 연구에 사용된 독립변수</title>
          <p>본 연구에서 분석한 연구들은 공통적으로 연령, 성별 및 대상자의 치아 수를 조사하였으며 잔존 치근 및 제 3대구치는 치아 수에서 제외하였다. 치아 개수를 범주로 제시한 논문이 5편, 평균값을 제시한 논문 3편, 치아가 존재하는 대상자와 무치악의 대상자를 비교한 연구가 2편, 질적연구가 1편이었다. 그 외 독립변수로는 생활환경, 건강상태, 일상활동, 치아 보존상태, 구강상태, 지역, 수입, 교육, 생활 상태, 보철 사용 유무, 구강 상태 자가진단, 만성질환, 노쇠지수, 흡연, 보유 질병 수, 우울, 마지막 치과 방문, 충치, 민족적 배경, 보험, 상실 치아, 틀니, 하루 칫솔질 횟수, Body mass index (BMI 지수), 음주, 신체 활동, 당뇨, 고혈압, 치통, 전치부 상실, 구치부 상실, 치과 방문, 자가 칫솔질 가능 여부, 복용 약물, 구강건조, 음식 섭취 능력, 운동, 배우자 유무가 있었다.</p>
        </sec>
        <sec id="s2bc">
          <title>3) 측정도구</title>

          <p>OHIP는 구강건강상태가 개인의 안녕과 삶의 질에 미치는 영향을 평가하기 위해 개발된 도구이다. 기능적 제한, 신체적 통증, 정신적 불안, 신체적 장애, 정신적 장애, 사회적 장애, 불이익으로 7영역 총 49개의 항목으로 구성되어 있으며 5점 척도로 평가하여 점수가 높을수록 일상생활과 삶의 질에 미치는 영향이 높은 것을 의미한다.</p>
          <p>SF-36는 36개의 질문으로 구성된 삶의 질 설문 도구이며, 두 개의 신체적, 정신적 건강요약 지표로 표현할 수 있고, 8개의 영역점수로도 표현할 수 있다. SF-36의 8개 차원은 신체 기능, 신체 역할, 통증, 일반적 건강, 정신 건강, 정서적 역할, 사회 기능, 활력으로 구성되어 있다.</p>
          <p>SF-12는 SF-36의 항목을 반영하여 재구성한 도구로 SF-36과 같이 8개의 영역점수로 표현할 수 있고, SF-36의 측정을 짧은 설문을 통해 최소 90%의 정확도로 재현한 도구이다. 응답은 5점 척도로 매겨지며 각 항목의 점수를 합산하여 총 점수를 도출하고, 총 점수가 높을수록 낮은 삶의 질을 나타낸다.</p>
          <p>GOHAI는 환자의 구강 기능 문제를 측정하고 구강 질환으로 인한 심리 사회적 영향을 평가하기 위해 개발된 도구이며 구강 질환의 영향에 대한 세 가지 영역을 반영하는 12개의 항목으로 구성되어 있다.</p>
          <p>EQ-5D는 삶의 일반적 품질을 평가하는 도구이며, 운동, 자기관리, 일상 활동, 통증/불편, 근심/우울의 5개 영역으로 구성되어 있다.</p>
          <p>질적연구방법이 시행된 연구<xref ref-type="bibr" rid="r016">[16]</xref>는 대상자를 인터뷰하는 방식으로 진행하였다.</p>

      </sec>
      <sec id="s2c">
        <title>3. 연구결과</title>
        <p>SF-36을 이용해 삶의 질을 측정한 Akifusa 등<xref ref-type="bibr" rid="r026">[26]</xref>의 연구와 SF-36을 재구성한 지표인 SF-12을 이용한 Mack 등<xref ref-type="bibr" rid="r027">[27]</xref>의 연구 모두 일상생활 수행능력(activities of daily living)을 측정했으며 치아 수가 적을 경우 삶의 질을 측정하는 지표 중 하나인 신체지수(physical index)를 악화시킨다고 하였다. OHIP-14를 이용한 Guimarães 등<xref ref-type="bibr" rid="r039">[39]</xref>의 연구에서도 치아 상실은 삶의 질에 부정적인 영향을 미친다고 하였으며, 구치보다는 전치를 상실할 경우에 삶의 질이 더 감소한다고 보고하였다.</p>
        <p>하나 이상의 치아가 있는 대상자와 무치악의 대상자를 비교한 Yoshida 등<xref ref-type="bibr" rid="r023">[23]</xref>과 Andrade 등<xref ref-type="bibr" rid="r031">[31]</xref>은 무치악 상태의 대상자보다 치아가 하나라도 존재하는 대상자의 삶의 질이 더 높다고 말하고, Mack 등<xref ref-type="bibr" rid="r027">[27]</xref>은 남아있는 치아 개수가 9개 이하일 경우 삶의 질 감소에 더 큰 영향을 미친다는 연구결과를 얻었으며, 김 등<xref ref-type="bibr" rid="r041">[41]</xref>과 박 등<xref ref-type="bibr" rid="r045">[45]</xref>은 평균 잔존 치아수가 20개 이상인 경우에 높은 삶의 질 점수를 가진다고 보고하였다.</p>
        <p>Visscher 등<xref ref-type="bibr" rid="r034">[34]</xref>은 완전 자연 치아 환자와 고정성 보철물을 장착하고 있는 환자 사이엔 삶의 질에는 차이가 없지만, 고정성 보철물 장착 환자와 가철성 보철물 장착 환자를 비교하면 전자의 그룹이 상당히 더 나은 삶의 질을 보인다고 하였으며 보철치아를 포함하는 기능치아의 수는 노인의 삶의 질에 긍정적인 영향이 있음을 확인하였다. 또한, 김 등<xref ref-type="bibr" rid="r035">[35]</xref>은 치아의 상실을 감소시킴으로써 의치 장착을 지연시키는 것이 구강건강 삶의 질을 유지시키는 방안이라고 하였으나, Paredes 등<xref ref-type="bibr" rid="r040">[40]</xref>은 치아의 수는 삶의 질과 관련이 없다는 이전 결과들과는 다른 결과를 얻었고, 오히려 노인들이 섭취하는 약으로부터 나타나는 구강건조증이 삶의 질과 관련이 있다고 기술하였다. 한편 양 등<xref ref-type="bibr" rid="r038">[38]</xref>은 삶의 질이 치아 수와 연관이 있지만 치아 통증과 더 밀접한 관련이 있다고 보고하였다.</p>
        <p>Niesten 등<xref ref-type="bibr" rid="r016">[16]</xref>은 질적연구방법을 이용해 연구를 진행하였으며 무작위로 선정된 주간보호시설 및 주거지원시설에서 참여자를 선정하였다. 참여자들은 자연치아를 갖는 것은 삶의 질에 긍정적으로 기여한다고 답하였으며 성취와 자부심, 통제력, 온전함, 구강 기능, 외모와 편안함에도 영향을 미친다고 하였다. 결과적으로 Paredes 등<xref ref-type="bibr" rid="r040">[40]</xref>은 남아있는 치아의 수는 삶의 질과 관계가 없다고 보고하였으며, 이를 제외한 나머지 11개의 연구에서 치아 수가 많을수록 삶의 질이 상승한다고 보고하였다&#x3C;<xref ref-type="table" rid="t002">Table 2</xref>&#x3E;.</p><table-wrap id="t002" position="float"> <label>Table 2.</label> <caption> <title>Systematic review of included studies</title> </caption> <table rules="groups" frame="hsides"> <thead> <tr valign="middle"> <th align="left">Author(year) / Region</th> <th align="center">Sample size (mean age and age range)</th> <th align="left">Design</th> <th align="left">Independent variables</th> <th align="center">Dependent variables</th> <th align="left">Results</th> </tr> </thead> <tbody> <tr valign="top"> <td align="left">Yoshida et al.       <title>결론</title>
      <p>본 연구는 노인의 치아의 개수가 삶의 질에 미치는 영향을 알아보기 위해 2000년 1월부터 2020년 1월까지 국내외에 발표된 연구 중 12편을 선정하여 체계적인 분석을 시행하였고, 11개의 연구에서 치아의 수가 많을수록 삶의 질이 높다는 결과를 얻었다.</p>
      <p>치아를 가지고 있는 대상자와 무치악인 대상자를 비교한 Yoshida 등<xref ref-type="bibr" rid="r023">[23]</xref>의 연구와 Andrade 등<xref ref-type="bibr" rid="r031">[31]</xref>의 연구 모두 무치악인 대상자보다 하나라도 치아가 존재하는 대상자의 삶의 질이 더 높았는데, Brennan 등<xref ref-type="bibr" rid="r048">[48</xref>-<xref ref-type="bibr" rid="r050">50]</xref>의 연구결과와 비슷하였다. 특히 구강건강영역 중에서 치아 상실은 악관절 이상 및 발음의 어려움으로 일상생활활동에도 영향이 있기에 가장 심각한 부분 중 하나라고 말할 수 있다<xref ref-type="bibr" rid="r051">[51</xref>,<xref ref-type="bibr" rid="r052">52]</xref>. 노인들은 치아의 상실로, 생물학적 필요를 충족시켜주는 영양소 불균형의 발생뿐만 아니라 식사에서 느끼는 행복도 빼앗기게 된다<xref ref-type="bibr" rid="r053">[53]</xref>. 신 등<xref ref-type="bibr" rid="r054">[54]</xref>은 에너지, 단백질, 인, 철, 티아민, 니아신 등 모든 영양소에서 무치악군, 1~19개의 자연치아군, 20개 이상의 자연치아군 간의 영양소 섭취에 유의한 차이가 있었다고 하였고, 영국과 호주에서 실시한 삶의 질 비교 연구<xref ref-type="bibr" rid="r055">[55]</xref>에서도 치아상실은 삶의 질에 영향을 미치는 중요한 요소라고 발표된 바 있다.</p>
      <p>Visscher 등<xref ref-type="bibr" rid="r034">[34]</xref>은 완전 자연 치아 환자와 고정성 보철 환자 사이엔 삶의 질 차이가 없지만, 고정성 보철물 환자와 가철성 보철물 환자를 비교하면 전자의 그룹이 상당히 더 나은 OHRQoL를 보이고, 보철치아를 포함하는 기능치아의 수는 노인의 삶의 질에 긍정적인 영향이 있음을 확인하였다. 윤<xref ref-type="bibr" rid="r056">[56]</xref>은 기능치 비율이 89% 이상인 경우 구강건강 뿐 아니라 일반적 건강상태, 정신적 건강상태, 신체적 건강상태에서도 자가건강평가 수준이 높다고 말하여 기능치아가 삶의 질에 긍정적인 영향이 있다는 Visscher 등<xref ref-type="bibr" rid="r034">[34]</xref>의 연구를 뒷받침한다. 허 등<xref ref-type="bibr" rid="r015">[15]</xref>은 가철성 의치를 사용하지 않고 고정성 보철물로 수복한 경우, 저작불편과 발음 문제를 적게 느껴 GOHAI는 높게 나타난다고 한다. 의치 사용 만족도 조사에서 만족은 27.8%, 보통은 32.9%, 불만족이 39.2%로 가철성 보철물인 의치에 대하여 만족하지 못하는 노인의 비율이 높게 조사되는 것으로 보아 가철성 보철물을 사용하는 노인일수록 자신의 구강건강에 대해 나쁘다고 인식하고 있으며, 삶의 질도 떨어진다는 Locker 등<xref ref-type="bibr" rid="r057">[57]</xref>의 연구와 같은 결과를 보인다. 이는 김<xref ref-type="bibr" rid="r035">[35]</xref>의 상실치아를 감소시켜 의치 장착을 지연시키는 것이 구강건강 삶의 질을 유지시키는 방안이라고 주장하는 연구와 일맥상통하다.</p>
      <p>그러나 대부분의 연구결과와 다르게, Paredes 등<xref ref-type="bibr" rid="r040">[40]</xref>의 연구에서는 치아의 수가 삶의 질과 관련이 없다고 말한다. 오히려 노인들이 섭취하는 약으로 인해 나타나는 구강건조증이 삶의 질과 관련이 있다고 기술하였는데, 이 연구는 대상자의 수가 30명으로 매우 적어 삶의 질이 치아 수와 아주 관련이 없다고 일반화하기는 어렵다. 복용 약물의 수가 많을수록 구강건조증이 심하다고 보고한 박과 류<xref ref-type="bibr" rid="r058">[58]</xref>의 연구와, 타액분비의 저하로 입맛의 변화, 의치 유지의 어려움, 통증, 말하기, 수면장애를 일으켜 구강건강 관련 삶의 질을 떨어뜨린다는 Turner와 Ship<xref ref-type="bibr" rid="r059">[59]</xref>의 연구와 일치하는 결과지만, 타액의 분비가 잘 이루어지지 않으면 타액내 포함된 소화효소, pH, Urea, Ammonia, 칼슘과 인 등의 무기물질과 Lysozyme, Peroxidase, Immunoglobulin 등의 항 세균 작용을 하는 물질 등이 우식병소로 진행되는 법랑질의 탈회과정을 억제할 수 없어, 약 복용에 따른 타액 분비량의 감소는 치아 우식 및 치아 상실로 이어질 수 있기에<xref ref-type="bibr" rid="r060">[60]</xref> 장기적으로 바라보면 본 연구의 결과와 같은 관련성을 시사하는 소견이라고 할 수 있다.</p>
      <p>구강건강 관련 삶의 질에 영향을 미치는 요인으로는 치아 수 이외에도 여러가지 다른 변수가 있다. Alfaro와 Ahluwalia<xref ref-type="bibr" rid="r061">[61]</xref>의 연구에서는 스스로 자신을 보살피는 능력이 높을수록 구강건강 행위에 긍정적이므로 일상생활 수행능력 또한 구강건강 관련 삶의 질에 중요한 변수라 말한다. 이 등<xref ref-type="bibr" rid="r017">[17]</xref>과 Catovi&#x107; 등<xref ref-type="bibr" rid="r062">[62]</xref>도 일상생활수행 능력이 구강건강 영향지수에 직접적인 영향을 미치며, 일상생활수행이 어려운 노인일수록 구강건강이 좋지 않았다는 결과를 보였다.</p>
      <p>신 등<xref ref-type="bibr" rid="r063">[63]</xref>은 노인들의 구강건강 관련 삶의 질이 교육의 수준과 관련이 있다고 하였고 고졸 이상의 학력을 가진 응답자에 비해 무학력자의 구강건강 관련 삶의 질이 14.9배 떨어진다고 말했다. 교육의 정도가 직접 관련된 것은 아니지만 교육과 관계 있는 소득이나 건강상태, 사회적 관계 수준을 반영한 결과로 해석된다. 특히 치과 치료비의 지불 능력에서 차이를 나타내며 경제적 요인의 영향이 크다고 보고된 바 있다<xref ref-type="bibr" rid="r064">[64]</xref>. 낮은 경제적 능력은 치과 치료비 지불에 대한 두려움이 되고, 이는 필요한 치과 치료를 방해하기에 치아의 수명이나 치주의 상태는 걷잡을 수 없이 나빠지게 된다. 제때에 적절한 치과 치료를 받지 못하면 결국 치아의 상실까지 이어질 수 있기에 구강건강관련 삶의 질에 관련한 모든 각각의 변수는 연관이 있다고 말해도 과언이 아니다.</p>
      <p>본 연구는 체계적 문헌고찰을 통해 치아 상실에 따른 삶의 질에 대한 결론을 얻으려고 노력하였다. 대부분의 연구에서 치아의 수가 많을수록 삶의 질은 높아진다고 말하지만, 각 연구마다 삶의 질 척도가 다르고 비교한 치아 개수의 구간도 다르기에 치아의 수에 따라 노인의 삶의 질이 정비례한다고 말하기 어렵다. 또한 전치부와 구치부를 나눠서 비교한 연구가 많지 않아 저작에 중요한 역할을 하는 구치부의 상실이나 심미적으로 중요한 전치부의 상실에 따라, 치아의 수와 삶의 질에 대한 결과는 달라질 수 있다. 양 등<xref ref-type="bibr" rid="r038">[38]</xref>의 연구에 따르면 치아상실도 중요하지만 치아통증과 삶의 질은 조금 더 밀접한 관련이 있다고 하는 것으로 보아 대상자의 치아 통증 유무도 배제할 수 없는 부분이다. 향후 노인의 삶의 질에 영향을 주는 다른 요인들을 추가하여 비교한 연구가 필요할 것으로 보인다.</p>
      <p>이러한 한계에도 불구하고 대부분의 연구 결과가 노인의 치아 수 감소와 삶의 질 하락에 대하여 일관성이 있었으며, 치아 수에 관련한 삶의 질을 비교한 최근 연구를 찾기 힘들다는 것을 고려할 때 평균수명이 점점 늘어나는 현대 노인들의 구강건강 관련 삶의 질을 높이기 위한 기초자료가 될 수 있고, 노인들을 위한 구강건강관련 프로그램 수립 시에도 방향을 제시할 수 있을 것이라 기대한다.</p>
