        <sec>
          <title>Objectives:</title>
          <p>To identify the factors influencing the turnover intention of registered South Korean dental hygienists and analyze the effect sizes.</p>
        </sec>
        <sec>
          <title>Methods:</title>
          <p>Overall, 54 studies based on criteria of PICO from 1 January 2000 to 11 January 2022 were extracted. The factors related to turnover intention were applied to the ecological system theory and categorized. Subsequently, the effect size of the correlations was comprehensively meta-analyzed by dividing it into protective and risk factors depending on the negative or positive correlation direction.</p>
        </sec>
        <sec>
          <title>Results:</title>
          <p>For the overall effect size, the protective factor (ES<sub>r</sub>=-0.458) was higher than the risk factor (ES<sub>r</sub>=0.352). In the protective factors, organizational commitment and perceived organizational support belonging to the microsystem yielded the largest effect size. Furthermore, as for risk factors, burnout, job stress, work harassment, role conflict, and emotional labor belonging to the microsystem showed a moderate effect size.</p>
        </sec>
        <sec>
          <title>Conclusions:</title>
          <p>Factors belonging to the microsystem demonstrated a large effect size in both protection and risk factors for dental hygienists&#x2019; turnover intention. Additionally, the factor showing the largest effect size was protective factor categorized into a microsystem.</p>

      <title>서론</title>
      <p>치과위생사는 치과임상현장에서 치료와 예방, 교육, 환자관리 및 경영 등 다방면에 걸친 역할을 담당하는 치과의료인력의 구심점이라고 볼 수 있다<xref ref-type="bibr" rid="r001">[1]</xref>. 구강건강과 전신건강과의 유의한 상관성이 밝혀짐으로써<xref ref-type="bibr" rid="r002">[2]</xref> 구강건강증진을 위한 사회적 관심과 요구도가 더욱 높아지고 있다. 따라서 치과위생사 본연의 업무수행에 대한 중요성이 더욱 강조되고 있으나 치과위생사의 잦은 이직으로 인해 치과의료기관에서는 인력의 부족을 호소하는 실정이다<xref ref-type="bibr" rid="r003">[3]</xref>. 치과위생사의 높은 이직률은 전 세계적으로 해결해야 하는 중요한 문제로 대두되고 있으며<xref ref-type="bibr" rid="r001">[1</xref>,<xref ref-type="bibr" rid="r004">4</xref>-<xref ref-type="bibr" rid="r006">6]</xref> 국내에서도 치과위생사의 75.6%가 이직을 경험하는 것으로 확인되고 있다<xref ref-type="bibr" rid="r007">[7]</xref>. 이로 인해 1980년대 후반부터 지금까지 임상현장에서 치과위생사의 인력난은 지속되고 있다<xref ref-type="bibr" rid="r008">[8]</xref>.</p>
      <p>이직의도는 현 직장에 대한 불만족으로 직업이나 근무지를 떠나고 싶어 하는 의도를 말하는 것으로<xref ref-type="bibr" rid="r009">[9]</xref> 실제 이직을 예측하는 중요한 변수가 된다<xref ref-type="bibr" rid="r010">[10]</xref>. 그러므로 이직의도에 대한 관련요인을 파악하고 규명하는 것은 이직을 낮추는데 기여할 뿐만 아니라 의료서비스의 질적 향상을 높이는 데에도 영향을 미치고 있다<xref ref-type="bibr" rid="r011">[11]</xref>. 때문에 이직의도에 영향을 주는 요인을 찾기 위해 다양한 방법의 연구와 분석이 시도되고 있다. 메타분석은 선행연구들을 종합적으로 분석하여 결과를 도출하는 방법<xref ref-type="bibr" rid="r012">[12]</xref>으로 최근 다양한 직종의 이직의도 분석에도 이용되고 있다. 국내 간호사를 대상으로 2000년부터 2020년까지 출판된 문헌을 메타분석한 김과 김<xref ref-type="bibr" rid="r013">[13]</xref>은 이직의도와 양의 값을 나타낸 변수를 정서적 소진, 직무스트레스, 경력 안정기를 제시했으며, 음의 값을 나타낸 변수는 조직몰입, 개인-조직 적합성, 경력몰입, 업무몰입, 직무만족도, 직무배태성으로 보고하였다. 호텔리어를 대상으로 이직의 억제요인을 메타분석한 오와 정<xref ref-type="bibr" rid="r014">[14]</xref>은 조직몰입, 절차 공정성, 조직신뢰, 개인-조직 적합성이 이직의도와 효과크기값이 큰 요인이라고 하였다. 이외에도 특정한 단일요인과 이직의도를 메타분석한 연구<xref ref-type="bibr" rid="r015">[15</xref>-<xref ref-type="bibr" rid="r018">18]</xref>들도 다양한 직종에서 상당 부분 이루어지고 있는 실정이다.</p>
      <p>치과위생사의 이직의도에 대해서도 연령이나 학력, 직위, 총경력, 고용형태 등 개인적인 특성 외에도 이직관련 다양한 요인을 고려한 단면분석 결과들이 보고되어 왔다<xref ref-type="bibr" rid="r019">[19</xref>-<xref ref-type="bibr" rid="r023">23]</xref>. 치과위생사 이직의 주된 예측요인으로는 직무스트레스와 직무만족이 많았고<xref ref-type="bibr" rid="r024">[24</xref>-<xref ref-type="bibr" rid="r026">26]</xref>, 조직몰입은 이직의도를 감소시키는 요인으로 분석되고 있었으며<xref ref-type="bibr" rid="r027">[27</xref>-<xref ref-type="bibr" rid="r029">29]</xref> 신규 치과위생사의 경우는 멘토의 기능에 영향을 받고 있었다<xref ref-type="bibr" rid="r030">[30]</xref>. 특히 내부마케팅 요인은 상반된 결과를 나타내 긍정적으로도 작용하였고<xref ref-type="bibr" rid="r031">[31]</xref>, 부정적으로 작용하는 것으로 파악되었다<xref ref-type="bibr" rid="r032">[32]</xref>. 이와 같이 치과위생사의 이직의도와 관련된 연구가 상당수 축적되어 있었으나, 이 연구들을 체계화하여 총체적으로 리뷰하며 메타분석한 연구는 거의 이루어진 바가 없다.</p>
      <p>이에 본 연구는 이직의도와 음의 상관성을 나타내는 보호요인과 양의 상관성을 나타내는 위험요인으로 구분하여 메타분석을 실시하고자 한다. 이를 통해 기존의 연구자 임의로 관련요인을 추출하여 상관성을 분석한 선행연구와 이직의도를 보호하는 요인과 촉진하는 위험요인이 혼재된 선행연구의 한계를 극복하고자 한다. 또한 보호요인과 위험요인의 영향력을 파악하여 치과위생사의 이직의도와 관련된 후속연구와 중재의 방향성을 제시하는데 기초적인 자료를 제시하고자 한다.</p>
    </sec>
    <sec id="s2" sec-type="methods">
      <title>연구방법</title>
      <sec id="s2a">
        <title>1. 연구설계</title>
        <p>본 연구는 국내 치과위생사의 이직의도와 관련된 요인에 대해 연구한 개별연구들을 대상으로 양적 연구결과인 상관계수 효과크기를 메타분석한 서술적 기술연구이다.</p>
      </sec>
      <sec id="s2b">
        <title>2. 문헌 선정기준 및 배제기준</title>
        <p>본 연구를 위해 분석대상 문헌의 선정은 PICO(Participants, Intervention, Comparisons, Outcome)를 기준으로 하였고, 대조군(Comparisons)은 선정에 해당하지 않아 제외하였다. 1) 2000년 1월 1일부터 2022년 1월 11일까지 출판된 한국 치과위생사(Participants)를 대상으로 한 문헌 2) 이직의도에 영향을 미치는 관련요인(Intervention)에 대한 문헌 3) 이직의도(Outcome)를 종속변수로 한 문헌 4) 상관계수값이 제시된 문헌이었다. 배제기준은 1) 치과위생사 이외의 직원을 포함한 문헌 2) 전문을 구할 수 없는 문헌 3) 효과크기를 구할 수 없는 질적 연구나 문헌 연구 4) 특정 하위요인만 상관계수가 제시된 문헌 5) 한국어 또는 영어 이외의 언어로 된 문헌이었다.</p>
      </sec>
      <sec id="s2c">
        <title>3. 문헌 선정 및 자료 수집방법</title>
        <p>본 연구는 PRISMA(Preferred Reporting Items for Systematic Reviews and Meta-Analysis) 지침에 따라 수행하였다&#x3C;<xref ref-type="fig" rid="f001">Fig. 1</xref>&#x3E;.</p>

        <p>문헌 검색은 의학분야 전자데이터베이스(OVID Medline &amp; Pubmed), 유럽의학분야전자데이터베이스(Excerpta Medica dataBASE, EMBASE), 누리미디어(DBpia), 대한의학학술지편집인협의회(KoreaMed), 한국학술정보(Korean Studies Information Service System, KISS), 한국교육학술정보원(Research Information Sharing Service, RISS), 한국과학기술정보센터(NDSL), 국회도서관(Nanet), 국립중앙도서관(NLK), 한국의학데이터베이스(Korean Medical Database, KMbase), 를 이용하여 2000년 1월부터 2022년 1월까지 발행된 문헌을 모두 검색하였다. 검색식은 &#x2018;치과위생사&#x2019; AND(&#x2018;이직의도&#x2019; OR &#x2018;이직&#x2019;)으로 하였고 해외논문 검색은 MEDLINE에서 확인된 MeSH어를 통해 검색하였으며 (&#x2018;dental hygienists&#x2019; [MeSH] AND &#x2018;Korea&#x2019; [MeSH]) AND (&#x2018;turnover&#x2019; OR &#x2018;intention&#x2019; [MeSH] OR &#x2018;quitting&#x2019; [MeSH] OR &#x2018;leave&#x2019; OR &#x2018;occupation&#x2019; [MeSH] OR &#x2018;employment&#x2019;)로 검색하였다.</p>
        <p>이를 통해 국내 석·박사 학위논문과 학술지 논문 등을 포함하여 총 1,064편의 문헌이 검색되었다. 이후 서지반출 프로그램 엔드노트(EndNote 20)를 이용하여 중복된 781편의 문헌을 제외하여 총 283편을 선정하였다. 본문을 확인하여 기준에 부합하지 않는 50편과 결과변수로 이직의도를 보고하지 않은 113편, 상관분석을 실시하지 않은 31편, 효과크기를 구할 수 없는 질적연구 등 13편, 치과위생사가 아닌 직종이 포함된 10편, 통계치 오류로 값을 신뢰할 수 없는 4편, 관련요인의 하위요인만 상관분석을 시행한 4편, 동일한 주제에 대하여 공통의 데이터를 이용한 3편, 대상자를 남자만으로 한정한 1편을 제외하여 최종 54편을 선정하였다.</p>
      </sec>
      <sec id="s2d">
        <title>4. 자료의 처리 및 분석</title>
        <sec id="s2da">
          <title>1) 자료의 코딩</title>
          <p>선정된 문헌을 정리하기 위하여 연구자 3인이 합의 후 코딩매뉴얼을 만들었고 항목은 일련번호, 저자명, 제목, 출판유형, 출처, 출판년도, 표본 수, 지역, 독립변인, 종속변인, 상관계수, 측정도구 등으로 구성하였다. 두 명의 연구자가 코딩하고 다시 세 명의 연구자가 검토하여 이상치나 확인이 필요한 부분은 충분한 논의를 통해 의견을 일치시켰다.</p>
          <p>먼저 관련요인은 상관의 방향을 고려하여 이직의도를 완충시키거나 예방하는 부적 상관의 보호요인과 증가시키는 정적 상관의 위험요인으로 크게 분류하였다. 다음으로 각 요인의 의미와 특성에 맞게 생태체계이론<xref ref-type="bibr" rid="r033">[33]</xref>을 적용하여 관련요인을 유목화하였다. 생태체계이론은 개인이 처한 상황을 생태학적으로 이해하는 관점에서 체계적인 접근을 해야 할 필요성이 있다는 이론에 기반하고 있다<xref ref-type="bibr" rid="r033">[33]</xref>. 생태체계이론은 성격과 기질을 포함한 &#x2018;개인&#x2019;을 중심으로 개인과 직접적인 상호작용을 통해 나타나는 감정과 경험 등의 요인을 포함한 &#x2018;미시체계&#x2019;가 둘러싸고 있다. 이러한 미시체계를 연결하는 근무환경을 &#x2018;중간체계&#x2019;로 정의하고, 소속은 되어 있으나 직접 관여되지는 않는 &#x2018;외체계&#x2019;와 광범위한 사회문화적 특성으로 개인에게 영향을 미칠 수 있는 &#x2018;거시체계&#x2019;로 분류하고 있다.</p>
        </sec>
        <sec id="s2db">
          <title>2) 포함된 문헌의 질 평가</title>
          <p>문헌의 질은 Law 등<xref ref-type="bibr" rid="r034">[34]</xref>이 개발한 Critical review form을 이용하였다. 제시한 평가 항목은 양적 연구의 연구목적과 문헌고찰, 설계, 표본과 표본 수 산정, 측정변수의 신뢰도와 타당도, 중재방법(중재 설명 여부, 중재 오염, 중복 중재 여부), 분석방법 및 통계의 유의성, 대상자 탈락 기록, 결론 및 의의로 총 15개 항목이었다. 각 항목에 관해 연구자 2인이 독립적으로 질을 평가하였으며, 일치하지 않는 항목은 재검토하여 일치시켰다. 표본크기의 정당화 항목에서 17편(31%)의 문헌이 표본수의 산정 근거를 제시하고 있었다. 측정도구의 신뢰도는 53편(98%), 타당도는 51편(94%)의 문헌이 제시하였다. 분석방법은 53편(98%)의 문헌이 제시하였으며 대상자 탈락 기록에 대해서는 48편(89%)의 문헌이 제시하고 있음을 확인하였다. 위 다섯 항목을 제외한 항목에 대해서는 모든 문헌이 보고하고 있어 연구 보고의 질이 높은 것으로 확인되었다.</p>
        </sec>
        <sec id="s2dc">
          <title>3) 출판편향</title>
          <p>54편의 문헌에 대한 출판편향을 검증하기 위해 Funnel plot<xref ref-type="bibr" rid="r012">[12]</xref>을 이용하여 평가하였다. 시각적으로 비대칭이 있다고 판단될 경우 Trim-and-Fill<xref ref-type="bibr" rid="r035">[35]</xref> 방법으로 연구 편수를 추가하여 대칭화하였다.</p>
        </sec>
        <sec id="s2dd">
          <title>4) 효과크기의 산출</title>
          <p>본 연구에서 R 4.1.3 프로그램을 이용하였으며 효과크기 산출은 상관계수 값(Correlation coefficient, r)을 표준화하기 위해 Fisher&#x2019;s Z로 변환<xref ref-type="bibr" rid="r012">[12]</xref>하여 계산하고 이를 다시 상관계수로 전환하여 효과크기(Effect sizes of correlation coefficient, 이하 ES<sub>r</sub>)를 해석하였다. 공통된 척도를 통해 계산된 효과크기는 원자료 유형에 상관없이 같은 결과로 해석할 수 있다<xref ref-type="bibr" rid="r036">[36]</xref>. 효과크기의 해석은 0.10보다 작으면 &#x2018;작은 효과&#x2019;, 0.30 정도면 &#x2018;중간 효과&#x2019;, 0.50 이상이면 &#x2018;큰 효과&#x2019;로 설명하였다<xref ref-type="bibr" rid="r037">[37]</xref>. 유의성 판단은 효과크기 값이 95% 신뢰구간에 &#x2018;0&#x2019;이 포함되었는지를 통해 확인하였다<xref ref-type="bibr" rid="r037">[37]</xref>.</p>
        </sec>
        <sec id="s2de">
          <title>5) 이질성 평가와 분석모형</title>
          <p>선정한 문헌 간의 이질성을 알아보기 위하여 Higgins의 I2통계량<xref ref-type="bibr" rid="r036">[36]</xref>, Cochrane의 Q통계량<xref ref-type="bibr" rid="r038">[38]</xref>을 이용하여 확인하였다. Q통계량은 개별 연구결과들이 같은 모집단에서 추출된 것인지를 파악하는 것이다. 동질성 검증결과 <italic>p</italic>값이 0.1보다 작은 경우 문헌 간에 통계적인 이질성이 존재한다는 의미이다. I<sup>2</sup>통계량은 총 분산에 관한 실제 연구의 효과크기 분산의 비율이고 25% 미만이면 작고, 25-75%이면 중간, 75% 이상이면 이질성이 큰 것으로 해석하였다<xref ref-type="bibr" rid="r039">[39]</xref>. 이를 토대로 연구결과들이 이질적인 경우에는 랜덤효과 모델을 이용하였고, 이질성이 낮은 경우는 고정효과 모델을 이용하였다.</p>

      </sec>
    </sec>
    <sec id="s3" sec-type="results">
      <title>연구결과</title>
      <sec id="s3a">
        <title>1. 분석대상 문헌의 특성</title>

        <title>2. 출판편향의 검증</title>
        <p>54편의 개별 문헌들을 대상으로 출판편향을 확인한 결과, 전체적인 분포가 좌측으로 편향된 것을 확인하였다. Trim-and-Fill<xref ref-type="bibr" rid="r035">[35]</xref> 방법으로 출판편향을 재검증하였고, 14편의 문헌을 추가하였다. 효과크기는 보정 전 0.429(95% CI=0.374~0.480)에서 보정 후 0.496(95% CI=0.445~0.544)으로 확인되었다&#x3C;<xref ref-type="fig" rid="f002">Fig. 2</xref>&#x3E;. 보정 이후 효과크기의 변화가 10% 이상으로 나타나 출판편향이 있는 것으로 확인되었다.</p>

        <title>3. 효과크기</title>
        <sec id="s3ca">
          <title>1) 이직의도 보호요인의 상관계수 효과크기</title>
          <p>이직의도와 관련된 보호요인의 전체 효과크기는 &#x2013;0.458로 중간 값을 나타냈다. 보호요인은 I<sup>2</sup> 값이 95.7%로 높았고, 동질성 검증 결과 Q=961.59(<italic>p</italic>&#x3C;0.001)로 나타나 연구의 효과크기가 이질적이라고 볼 수 있으므로 랜덤효과 모형으로 분석하였다. 개인의 자아존중감을 제외한 관련요인은 95% 신뢰구간에서 통계적으로 유의한 효과를 가진 것으로 나타났다. 가장 많이 보고된 요인은 직무만족으로 27편이었고, 효과크기 값이 가장 큰 요인은 조직몰입(ES<sub>r</sub>=-0.595)이었다. 생태체계별 효과크기를 분석한 결과, 미시체계(ES<sub>r</sub>=-0.492), 중간체계(ES<sub>r</sub>=-0.377), 외체계(ES<sub>r</sub>=-0.346), 개인(ES<sub>r</sub>=-0.272) 순으로 나타났다. 이직의도와 중간 이상 효과크기를 나타낸 보호요인을 살펴보면, 미시체계의 조직몰입(ES<sub>r</sub>=-0.595), 조직후원인식(ES<sub>r</sub>=-0.566), 직무만족(ES<sub>r</sub>=-0.497), 개인의 자기성장성(ES<sub>r</sub>=-0.423), 중간체계의 보상(ES<sub>r</sub>=-0.417), 외체계의 내부마케팅(ES<sub>r</sub>=-0.346), 중간체계의 작업환경(ES<sub>r</sub>=-0.339), 업무범위(ES<sub>r</sub>=-0.331), 미시체계의 전문직업성(ES<sub>r</sub>=-0.331), 개인의 행복지수(ES<sub>r</sub>=-0.321), 미시체계의 대인관계(ES<sub>r</sub>=-0.320), 임파워먼트(ES<sub>r</sub>=-0.312) 순이었다&#x3C;<xref ref-type="table" rid="t002">Table           <title>2) 이직의도 위험요인의 상관계수 효과크기</title>
          <p>이직의도와 관련된 위험요인의 전체 효과크기는 0.352로 중간 값을 나타냈다. 위험요인의 I<sup>2</sup> 값은 88.5%로 높았으며, 동질성 검정에서 Q=286.21(<italic>p</italic>&#x3C;0.001)로 유의한 수준이었다. 효과크기가 이질적이었으므로 랜덤효과 모형으로 효과크기를 산출하였다. 미시체계의 직무만족과 외체계의 내부마케팅을 제외한 나머지 요인은 95% 신뢰구간에서 통계적으로 유의성이 확인되었다. 가장 많이 보고된 요인은 직무 스트레스로 13편이었고, 가장 높은 상관성을 나타내는 요인은 소진(ES<sub>r</sub>=0.464)이었다. 생태체계별 효과크기를 분석한 결과 미시체계(ES<sub>r</sub>=0.388)가 가장 높았고, 거시체계(ES<sub>r</sub>=0.259), 중간체계(ES<sub>r</sub>=0.213), 개인(ES<sub>r</sub>=0.142), 외체계(ES<sub>r</sub>=0.106) 순으로 나타났다. 중간 이상의 효과크기를 나타내는 위험요인은 미시체계의 소진(ES<sub>r</sub>=0.464), 직무스트레스(ES<sub>r</sub>=0.427), 직장 내 괴롭힘(ES<sub>r</sub>=0.335), 역할갈등(ES<sub>r</sub>=0.322), 감정노동(ES<sub>r</sub>=0.307) 순이었다&#x3C;<xref ref-type="table" rid="t003">Table 3</xref>&#x3E;.</p><table-wrap           <p>포함된 개별연구들에서 보고하고 있는 주요한 범주에 따라 효과크기를 산출하였다. 출판 연도에 따른 효과크기는 모두 95% 신뢰구간에서 통계적으로 유의하였으며 2016-2021년(ES<sub>r</sub>=0.421), 2011-2015년(ES<sub>r</sub>=0.312), 2000-2010년(ES<sub>r</sub>=0.311) 순으로 나타났다. 출판유형은 학위논문(ES<sub>r</sub>=0.402)가 학술지 논문(ES<sub>r</sub>=0.333)보다 효과크기가 크게 나타났다. 연구대상 지역은 비수도권 지역(ES<sub>r</sub>=0.386)이 가장 높았으며 수도권(ES<sub>r</sub>=0.315), 수도권과 비수도권 혼재(ES<sub>r</sub>=0.299) 순으로 나타났다. 전국은 ES<sub>r</sub>=0.409로 효과크기가 중간이었으나, 95% 신뢰구간에서 통계적으로 유의하지 않았다. 측정도구별로는 주<xref ref-type="bibr" rid="r047">[47]</xref> (ES<sub>r</sub>=0.452), 서와 고<xref ref-type="bibr" rid="r042">[42]</xref> (ES<sub>r</sub>=0.404), Michaels과 Spector<xref ref-type="bibr" rid="r040">[40]</xref> (ES<sub>r</sub>=0.321) 의 도구 순으로 확인되었다&#x3C;<xref ref-type="table" rid="t004">Table 4</xref>&#x3E;.</p>

      <sec id="s3d">
        <title>4. 이직의도 관련 전체 요인의 순위</title>

      <title>총괄 및 고안</title>
      <p>본 연구는 임상치과위생사의 이직의도와 관련요인을 생태체계이론에 적용하여 유목화하였고, 상관의 방향에 따라 보호요인과 위험요인으로 구분하여 상관계수 효과크기를 파악하였다. 또한, 주요 범주형 분석을 통하여 출판연도, 출판유형, 근무지역, 이직의도 측정도구에 따른 요인들의 상관계수 효과크기를 분석하였다.</p>
      <p>선정된 문헌은 2011년 이후 출판된 문헌이 80%로 대다수를 차지하여 2010년 이전에 출판된 문헌보다 4배 증가하였다. 이는 치과위생사의 이직이 사회적인 문제로 인식되어 점차 증가한 결과로 사료된다. 대다수의 연구에서 국외 연구자가 개발하고 국내 연구자에 의해 수정 보완한 측정도구를 사용하고 있었고, 국내에서 개발하거나 치과위생사 업무 특성을 단독으로 고려하여 측정한 연구는 없었다. 따라서 치과위생사의 이직의도를 정확하게 파악하는 측정도구의 개발이 요구되었다. 이직의도 관련 요인은 34개였고, 2편 이상에서 다루고 있는 25개 요인을 메타분석한 결과 보호요인은 17개, 위험요인은 10개였다. 생태체계별 관련요인은 미시체계가 가장 큰 비중을 차지하고 있었다. 이는 연구자들이 치과위생사와 가장 밀접한 환경을 이직의 원인으로 예측하여 활발하게 조사한 결과가 반영된 것으로 생각된다.</p>
      <p>보호요인들의 전체 값을 메타분석한 결과는 중간 효과크기(ES<sub>r</sub>=-0.458)로 나타나 이직의도에 일정한 영향을 미치고 있음을 알 수 있었다. 보호요인 중 이직의도와 가장 높은 상관성을 나타낸 생태체계는 미시체계의 조직몰입(ES<sub>r</sub>=-0.595)과 조직후원인식(ES<sub>r</sub>=-0.566)은 치과위생사 이직을 방지하는데 가장 효과적인 요인으로 관리될 필요가 있었다. 많은 선행연구<xref ref-type="bibr" rid="r048">[48</xref>-<xref ref-type="bibr" rid="r051">51]</xref>에서도 조직몰입은 이직의도에 영향을 미치는 중요한 요소로 확인된 바 있다. 따라서 조직의 목표를 치과위생사의 목표에 반영할 수 있도록 조직몰입을 촉진하는 다양한 전략을 개발하고 관리하는 것이 효과적일 것으로 사료되었다. 치과위생사의 공헌에 대한 인정과 복지로 지원하는 조직후원을 높이는 것이 이직감소에 중요할 것으로 평가되었다. 미시체계의 직무만족(ES<sub>r</sub>=-0.497)과 개인의 자기성장성(ES<sub>r</sub>=-0.423), 중간체계의 보상(ES<sub>r</sub>=-0.417)과 외체계의 내부마케팅(ES<sub>r</sub>=-0.346)등도 중요한 요인으로 고려되어 이직의도의 중간 효과크기를 나타냈다. 이중 관련요인으로 가장 많이 보고된 요인은 직무만족으로 Turner 등<xref ref-type="bibr" rid="r052">[52]</xref>은 개인의 기량 활용성과 치과의사의 긍정인식이 높으면 직무만족도가 높아진다고 하였다. 하지만 체계적 고찰연구를 한 Reinders 등<xref ref-type="bibr" rid="r053">[53]</xref>은 치과의사가 치과위생사의 자율적 업무수행에 관한 태도가 부정적이라고 분석하였는데, 이에 대해 문 등<xref ref-type="bibr" rid="r054">[54]</xref>은 치과위생사의 업무나 능력, 교육과정에 대한 치과의사들의 인식이 부족하기 때문이라고 하였다. 따라서 치과위생사의 직무만족과 이직률을 낮추기 위해서는 치과위생사의 교육과정과 업무역량에 대해 치과의사와 공유하는 것이 중요하다고 생각되었다. 또한 지속적인 자기성장을 할 수 있도록 하며, 이에 따른 내, 외부적인 지원과 적절한 보상도 역시 이직감소에 도움이 될 것으로 사료되었다.</p>
      <p>위험요인의 전체 효과크기는 0.352로 중간 효과크기를 나타냈으며, 이중 가장 영향력이 높은 요인은 소진(ES<sub>r</sub>=0.464)이었다. 간호사를 대상으로 한 메타분석<xref ref-type="bibr" rid="r055">[55</xref>-<xref ref-type="bibr" rid="r057">57]</xref>에서도 소진이 주요한 위험요인으로 확인된 바 있다. 소진과 이직의도를 낮추기 위해서는 조직 내 팀워크를 강하게 결속시키는 것이 중요하므로<xref ref-type="bibr" rid="r058">[58]</xref> 개방적인 의사소통을 통하여 조직구성원이 상호 협력하도록 지지할 필요가 있다. 두 번째로 유의한 관련요인은 직무스트레스로 많은 선행연구<xref ref-type="bibr" rid="r022">[22</xref>,<xref ref-type="bibr" rid="r059">59</xref>,<xref ref-type="bibr" rid="r060">60]</xref>에서 위험요인으로 밝혀진 바가 있다. 감정 소비가 많은 치과위생사의 직업적 특성은 소진과 스트레스를 많이 경험한 경우 이직으로 이어짐<xref ref-type="bibr" rid="r059">[59]</xref>을 알 수 있었다. 따라서 스트레스를 관리하여 대처할 수 있는 상황 훈련이 도움이 될 것<xref ref-type="bibr" rid="r055">[55]</xref>이라고 생각된다. 위험요인에서도 생태체계 중 미시체계가 가장 높은 상관성을 보였고 전체 위험요인 중에서 중간 이상 효과크기를 나타내는 요인 또한 모두 미시체계에 속해있었다. 그러므로 보호요인과 마찬가지로 미시체계의 주요 위험요인을 식별하여 관리하는 것이 중요하다고 사료되었다.</p>
      <p>관련요인 중 직무만족과 내부마케팅은 보호요인과 위험요인에 공통으로 포함되어 있었으나 유의수준은 보호요인에서만 확인되었고, 간호사의 이직의도를 메타분석한 연구에서도 직무만족과 내부마케팅은 보호요인으로 보고되고 있었다<xref ref-type="bibr" rid="r014">[14]</xref>. 따라서 이직의도를 감소시키기 위해서는 직무만족을 높여야 한다는 선행결과<xref ref-type="bibr" rid="r021">[21</xref>,<xref ref-type="bibr" rid="r051">51</xref>,<xref ref-type="bibr" rid="r054">54</xref>,<xref ref-type="bibr" rid="r061">61]</xref>를 지지할 수 있을 것이다. 내부마케팅은 조직 내의 인적자원을 대상으로 마케팅 활동을 하여 직무만족을 높이고 동기를 유발하여 궁극적으로 서비스의 품질을 높이려는 것<xref ref-type="bibr" rid="r062">[62]</xref>으로 본 연구에서는 보호요인(ES<sub>r</sub>=-0.346)에서만 유의성을 나타냈다. 따라서 내부마케팅은 이직의도를 예방할 수 있는 보호요인으로 해석하는 것이 합당하다고 볼 수 있다.</p>
      <p>연구의 포함된 개별연구의 일반적 특성 중 주요한 범주를 특정하여 이직의도의 차이를 비교하였다. 출판연도는 2016년 이후에 출판된 문헌에서 보호요인(ES<sub>r</sub>=-0.546)과 위험요인(ES<sub>r</sub>=0.421) 모두 이직의도가 높게 나타났다. 이는 시간이 경과해도 이직의도를 감소시킬 수 있는 해결방안이 마련되지 않았기 때문인 것으로 생각된다. 일반적으로 출판편향은 학술지 논문이 학위논문보다 효과크기가 높을 경우를 말한다. 그러나 본 연구에서는 보호요인에서는 학술지 논문(ES<sub>r</sub>=-0.489), 위험요인에서 학위논문(ES<sub>r</sub>=0.402)의 효과크기가 높아 출판편향에 대한 해석에 신중을 기할 필요가 있었다. 연구대상 지역은 보호요인(ES<sub>r</sub>=-0.519)과 위험요인(ES<sub>r</sub>=0.369) 모두 비수도권 지역의 효과크기가 컸으며 통계적으로 유의하였다. 이러한 결과는 치과위생사가 고임금, 좋은 근무환경 등을 기대하여 서울, 경기, 인천으로 대표되는 수도권 지역으로 집중된 결과라고 보고된다<xref ref-type="bibr" rid="r063">[63]</xref>. 보건의료인력 실태조사<xref ref-type="bibr" rid="r007">[7]</xref>에서도 치과위생사의 이직 경로가 수도권 지역으로의 이직이 이루어진 경우가 41%로 조사되어 이직문제에서 지역 간의 격차 해소에 대한 방안도 마련되어야 할 것이다. 측정도구는 보호요인에서 박<xref ref-type="bibr" rid="r044">[44]</xref>의 도구(ES<sub>r</sub>=-0.631), 위험요인에서 주<xref ref-type="bibr" rid="r047">[47]</xref>의 도구(ES<sub>r</sub>=0.452)가 가장 큰 효과크기를 나타냈다. 이는 치과위생사 이직의도의 크기가 대상자의 특성관련 요인 외에도 측정도구에 따라 차이가 있는 것이므로 치과위생사의 특성에 맞는 합리적 측정도구를 일관되게 사용하면 비교분석에 도움이 될 것으로 사료되었다.</p>
      <p>이직의도 전체 관련요인 중 유의한 값을 나타낸 요인은 24개였으며, 중간효과 크기 이상을 나타낸 15개요인 중 10개 요인이 보호요인이었다. 또한 우선순위 탐색결과 조직몰입이 가장 높은 상관성을 나타냈고, 조직후원인식과 직무만족이 뒤를 이어 상위 3위를 보호요인이 차지하고 있었다. 이는 보호요인이 위험요인에 비해 이직에 대한 영향력이 높음을 설명하는 것으로 위험요인을 제거하는 것보다 보호요인을 촉진하는 전략이 이직률 감소에 효과적임을 알 수 있다.</p>
      <p>본 연구는 국내 임상치과위생사의 이직의도와 관련된 개별연구들을 바탕으로 보호요인과 위험요인의 관점에서 효과크기를 분석하였다. 하지만 관련요인의 효과크기를 분석하기 위해 상관분석을 시행한 문헌만을 선정하였고, 사례 수가 적은 요인들도 분석에 활용되었으며, 선행된 메타분석이 거의 없어 비교가 불가능하였다는 제한점이 있다. 그러나 치과위생사 이직의도의 관련요인을 체계적으로 고찰하고 분석하였으며, 보호와 위험요인으로 구분하여 상관의 방향성을 고려하여 중점적으로 다루어야 할 요인의 효과크기를 제시하였다는 것이며, 무엇보다 다양한 매개요인들을 고려하여 근거기반 실무 및 연구를 수행하는데 연구의 의의가 있다.</p>
    </sec>
    <sec id="s5" sec-type="conclusions">
      <title>결론</title>
      <p>본 연구는 국내 임상치과위생사들의 이직의도 관련요인을 생태체계이론에 적용하여 유목화하고 상관의 방향에 따라 보호요인과 위험요인으로 구분하여 효과크기를 종합적으로 메타분석하였다. 2000년 1월 1일부터 2022년 1월 11일까지 등록된 문헌을 검색하여 PICO 기준에 맞는 학술지 논문 40편, 학위논문 10편 총 54편을 선별하여 관련요인을 추출하였다.</p>
      <p>1. 문헌을 통해 밝혀진 치과위생사의 이직의도 관련요인은 전체 34개였으며, 이중 17개의 보호요인과 10개의 위험요인이 메타분석에 포함되었다.</p>
      <p>2. 전체 효과크기는 보호요인(ES<sub>r</sub>=-0.458)이 위험요인(ES<sub>r</sub>=0.352)보다 높았으며, 두 요인 모두 중간정도의 수준이었다.</p>
      <p>3. 생태체계별 이직의도와 가장 관련성이 높은 요인은 보호요인으로 미시체계에 속한 조직몰입(ES<sub>r</sub>=-0.595)과 조직후원인식(ES<sub>r</sub>=-0.566)으로 큰 효과크기를 나타냈다. 이외에도 보호요인 중 미시체계의 직무만족(ES<sub>r</sub>=-0.497)과 개인의 자기성장성(ES<sub>r</sub>=-0.423), 중간체계의 보상(ES<sub>r</sub>=-0.417)과 외체계의 내부마케팅(ES<sub>r</sub>=-0.346)이 중간정도 크기였다. 위험요인에서도 미시체계의 소진(ES<sub>r</sub>=0.464)과 직무스트레스(ES<sub>r</sub>=0.427), 직장 내 괴롭힘(ES<sub>r</sub>=0.335), 역할갈등(ES<sub>r</sub>=0.322), 감정노동(ES<sub>r</sub>=0.307)이 중간정도 크기를 나타냈다.</p>
      <p>위와 같이 치과위생사의 이직의도는 보호 및 위험요인 모두에서 미시체계에 속한 요인이 큰 효과크기를 나타냈고, 가장 큰 효과크기를 나타낸 요인도 미시체계의 보호요인이었다. 따라서 치과위생사의 이직의도를 효과적으로 억제하고 관리하기 위해서는 보호요인에 중점을 두고 위험요인에 대응하는 적절한 전략과 중재방안을 마련하는 것이 필요할 것으로 사료되었다.</p>
