        <sec>
          <title>Objectives:</title>
          <p>This study aimed to evaluate the oral symptoms experiences of Korean adolescents among multicultural families.</p>
        </sec>
        <sec>
          <title>Methods:</title>
          <p>Data on 985 participants from the 16th Korea Youth Risk Behavior Web-based Survey were collected and analyzed using complex sample descriptive statistics.</p>
        </sec>
        <sec>
          <title>Results:</title>
          <p>The foreign-born mother, foreign-born parents, and foreign-born father rates were 77.2%, 17.3%, and 5.4%, respectively. The toothache, gingival bleeding, tooth fracture, and tooth sensitivity experience rates were 21.3%, 18.8%, 13.3%, and 30.4%, respectively. The tooth fracture rate was highest in the foreign-born parents group (24.6%). The toothache odds ratio (OR) was 1.71 (95% CI: 1.24–2.37) times higher in girls. The foreign-born mother group was 2.47 (95% CI: 1.09-5.60) times higher than that of the foreign-born father or both parents foreign-born groups. The smoking group was 2.03 (95% CI: 1.20-3.43) times higher than that of the non-smoking group. The gingival bleeding OR was 5.11 (95% CI: 1.80-14.53) times higher in the lowest economic status group. The tooth fracture OR was 3.44 (95% CI: 1.01-11.70) times higher in the lowest economic status group. The tooth sensitivity OR was 1.53 (95% CI: 1.14-2.04) times higher in girls.</p>
        </sec>
        <sec>
          <title>Conclusions:</title>
          <p>It is necessary to establish a program to promote oral health of adolescents from multicultural families.</p>

      <title>서론</title>
      <p>우리나라는 결혼 이민자 및 외국인 근로자 등 국내 유입 증가에 따라 체류하는 다문화·외국인 등 이주 배경 인구는 증가하는 추세에 있으며, 국제결혼 본격화로 이주여성이 대거 유입되면서 다문화가정이 점점 늘어나고 있다<xref ref-type="bibr" rid="r001">[1]</xref>.</p>
      <p>다문화가정이란 국내에 거주하고 있는 외국인이 포함된 가정을 말한다<xref ref-type="bibr" rid="r002">[2]</xref>. 우리나라의 많은 다문화가정이 한국인 아버지와 외국인 어머니 가정으로 구성되어 있고<xref ref-type="bibr" rid="r003">[3]</xref>, 그 밖에도 국제결혼가정, 유학생, 화교 및 북한 이탈주민(새터민) 등을 포함한다<xref ref-type="bibr" rid="r004">[4]</xref>.</p>
      <p>교육부에 따르면, 2021년 기준으로 다문화가정 자녀는 160,058명이며 2012년(46,954명) 대비 240.8% 증가하여 전체 학생 중 다문화 학생이 차지하는 비율은 0.7%에서 3.0%로 높아졌고, 특히 중학생은 0.5%에서 25%로 5배, 고등학생은 0.2%에서 1.1%로 증가하였다<xref ref-type="bibr" rid="r005">[5]</xref>.</p>
      <p>매년 증가하는 다문화가정 청소년은 일반가정 청소년에 비해 언어, 문화적 갈등뿐만 아니라 편견과 차별, 사회적&#xB7;경제적 문제 등에 노출되어 있다. 사회적&#xB7;경제적 문제로 이혼하는 가정이 많아지고 있으며, 경제적 빈곤을 해결하고자 하루 종일 일하는 부모들의 자녀 교육과 성장기 때의 사랑에 대한 욕구를 충족시켜 주지 못하고, 학업의 어려움으로 학업 중단율이 지속적으로 증가하고 있다. 편견과 차별에서 오는 정신적인 문제와 청소년기 스트레스는 다양한 건강 상태와 관련이 있으며, 특히 어머니가 외국 국적인 경우 건강관리의 소홀, 불충분한 영양 섭취, 편식<xref ref-type="bibr" rid="r006">[6]</xref> 및 구강 건강에도 영향을 미쳐 음주와 흡연, 구강질환의 이환율을 높이는 중요한 요인이 되고 있다<xref ref-type="bibr" rid="r007">[7]</xref>. 청소년건강행태조사 결과에 따르면, 우리나라 청소년의 19.5%는 치은 출혈 경험이 있으며, 38.3%가 치통을 경험한 것으로 조사되었다<xref ref-type="bibr" rid="r008">[8]</xref>.</p>
      <p>선행연구에서 일반가정과 비교했을 때 다문화가정 청소년의 구강 건강행태가 더 나쁘고<xref ref-type="bibr" rid="r009">[9]</xref>, 사회경제적 수준이 낮은 가정의 자녀들에게서 구강 건강 상태가 불량하였다<xref ref-type="bibr" rid="r010">[10]</xref>. 이는 다문화가정 결혼이주여성들의 구강 관리 능력이 미흡하고<xref ref-type="bibr" rid="r003">[3]</xref>, 다문화가정 청소년들은 그렇지 않은 가정에 비해 부모의 구강 보건에 대한 관심이 부족하며<xref ref-type="bibr" rid="r011">[11]</xref>, 의사소통의 문제와 동행인의 부재로 인한 부적절한 질병 치료<xref ref-type="bibr" rid="r012">[12]</xref> 및 체계적인 구강 건강증진 프로그램 운영의 미흡<xref ref-type="bibr" rid="r013">[13]</xref> 등으로 다문화가정 청소년에게 집중적인 구강 관리 서비스 지원이 필요한 실정이다.</p>
      <p>다문화가정 청소년을 대상으로 하는 연구를 살펴보면, 다문화가족과 일반가족 청소년의 구강 건강행태<xref ref-type="bibr" rid="r014">[14]</xref>, 다문화가정 청소년의 건강 위험행동<xref ref-type="bibr" rid="r015">[15]</xref>, 결혼이주여성인 어머니를 중심으로 한 연구<xref ref-type="bibr" rid="r003">[3]</xref>가 대부분이었고, 다문화가족 청소년의 성별에 따른 주관적 구강 증상 경험의 비교 연구<xref ref-type="bibr" rid="r016">[16]</xref>, 다문화가족과 일반가족 청소년의 주관적 구강 증상<xref ref-type="bibr" rid="r017">[17]</xref> 등이 진행되었으나 우리나라 다문화가정 청소년을 대상으로 아버지와 어머니를 외국 국적에 따라 분류하고 구강 증상 경험에 영향을 미친 요인을 분석한 연구는 미미한 실정이다.</p>
      <p>따라서 본 연구에서는 질병관리청에서 실시하는 자료 중 제16차(2020년) 청소년건강행태조사자료<xref ref-type="bibr" rid="r018">[18]</xref>를 이용하여 부모의 국적에 따른 다문화가정 청소년의 구강 증상 경험을 비교 분석하고 향후 이들을 대상으로 한 구강 건강증진 프로그램 개발에 필요한 기초자료를 제공하고자 한다.</p>
    </sec>
    <sec id="s2" sec-type="methods">
      <title>연구방법</title>
      <sec id="s2a">
        <title>1. 연구대상 및 방법</title>
        <p>본 연구는 우리나라 중·고등학생을 대상으로 자기 기입식 조사방법으로 질병관리청에서 실시하는 자료 중 제16차 청소년건강행태조사 자료를 이용하였다. 전체 대상자 54,948명 중 구강 증상 경험을 묻는 설문에 응답하고 양쪽 부모가 있는 다문화가정 청소년 985명을 최종분석대상으로 하였다. 본 연구에 사용된 자료는 제16차 청소년건강행태조사 원시자료로 정부승인통계 조사(승인번호 제117058호)이며, 생명윤리 및 안전에 관한 법률 시행규칙 제2조 2항 1호에 의해 기관생명윤리위원회 심의 없이 수행되었다.</p>
      </sec>
      <sec id="s2b">
        <title>2. 연구도구</title>
        <p>다문화가정은 양쪽 부모가 모두 있는 다문화가정의 청소년을 대상으로 아버지가 외국 국적인 그룹, 어머니가 외국 국적인 그룹, 양쪽 부모 모두 외국 국적인 그룹으로 분류하였다. 구강 증상 경험에 관한 변수는 설문조사를 통해 최근 12개월 동안 경험한 구강질환 증상을 묻는 설문을 분류하여 &#x2018;치아가 쑤시고 욱신거리고 아픔&#x2019; 증상은 &#x2018;치아 통증&#x2019;, &#x2018;잇몸이 아프거나 피가 남&#x2019; 증상은 &#x2018;치은 출혈&#x2019;, &#x2018;치아가 깨지거나 부러짐&#x2019;은 &#x2018;치아 파절&#x2019;, &#x2018;차갑거나 뜨거운 음료 혹은 음식을 마시거나 먹을 때 치아가 아픔&#x2019; 증상은 &#x2018;치아 시림&#x2019;으로 분류하였다.</p>
      </sec>
      <sec id="s2c">
        <title>3. 자료분석</title>
        <p>본 연구의 자료는 집락은 군집변수, 가중치는 단일가중치, 통합후층은 계층변수로 지정하여 복합표본 설계분석을 시행하였고, 유한모집단수정계수(FPC: Finite Population Correction)를 사용하여 계층별 크기를 지정하고 계획 파일을 생성하였다.</p>
        <p>부모의 국적에 따른 일반적 특성, 구강 증상 경험별 일반적 특성 및 부모의 국적과 구강 증상 경험의 관련성은 복합표본 교차분석을 시행하였고, 구강 증상 경험의 오즈비는 복합표본 로지스틱회귀분석을 시행하였다. 통계분석은 IBM SPSS program (ver 21.0; IBM Corp., Armonk, NY, USA)을 이용하였고 통계적 유의성 검정은 0.05로 설정하였다.</p>
      </sec>
    </sec>
    <sec id="s3" sec-type="results">
      <title>연구결과</title>
      <sec id="s3a">
        <title>1. 부모의 국적에 따른 일반적 특성</title>
        <p>부모의 국적에 따른 전체 대상자의 일반적 특성의 분포를 분석하였다&#x3C;<xref ref-type="table" rid="t001">Table 1</xref>&#x3E;. 전체 대상자 985명 중 어머니의 국적이 외국인 경우는 77.2%(784명)로 가장 높고, 양쪽 부모 모두 외국 국적인 경우는 17.3%(149명), 아버지의         <p>아버지가 외국 국적인 그룹과 양쪽 부모 모두 외국 국적인 그룹은 대도시 거주자가 가장 많았고(55.1%, 52.9%), 어머니가 외국 국적인 그룹은 중&#xB7;소도시 거주자가 46.8%로 가장 많았다(<italic>p</italic>&#x3C;0.001). 학교 성적은 아버지가 외국 국적인 그룹과 양쪽 부모 모두 외국 국적인 그룹은 중위권 비율이 각각 29.3%, 42.0%로 가장 높았고, 어머니가 외국 국적인 그룹은 중-하위권 비율이 30.1%로 가장 높았으며(<italic>p</italic>=0.050), 아버지의 학력은 세 그룹 모두 고등학교 졸업이 가장 높았다(<italic>p</italic>&#x3C;0.001). 어머니 학력은 아버지와 어머니가 각각 외국 국적인 그룹에서는 대학 졸업 이상이 46.8%, 27.0%로 가장 높고, 양쪽 부모 모두 외국 국적인 그룹에서는 고등학교 졸업이 30.2%로 가장 높았다(<italic>p</italic>=0.044). 흡연 경험 여부는 세 그룹 모두 흡연 경험이 없는 비율이 유의하게 높았다(<italic>p</italic>=0.027).</p>

        <title>2. 일반적 특성에 따른 구강 증상 경험</title>
        <p>일반적 특성에 따른 구강 증상 경험의 비율은 &#x3C;<xref ref-type="table" rid="t002">Table 2</xref>&#x3E;와 같다. 전체 대상자 중 구강 증상을 경험한 비율은 치아 통증이 21.3%, 치은 출혈 18.8%, 치아 파절 13.3%, 치아 시림 증상은 30.4%였다.</p><table-wrap         <p>치아 통증 경험의 비율은 여학생(24.5%)이 남학생(17.7%)보다 높았고(<italic>p</italic>=0.014), 학년별로는 고등학교 2학년(36.5%)이 가장 높고 중학교 1학년(12.5%)은 가장 낮았다(<italic>p</italic>&#x3C;0.001). 학교 성적이 중상위인 그룹이 26.7%로 가장 높고, 중위 그룹은 14.3%로 가장 낮았다(<italic>p</italic>=0.015). 가계경제 상태가 낮을수록 치아 통증 경험의 비율이 높아지는 경향을 보였고(<italic>p</italic>=0.012), 음주와 흡연 경험이 있는 그룹(<italic>p</italic>=0.020, <italic>p</italic>=0.001), 치면열구전색 경험이 있는 그룹은 없는 그룹보다 치아 통증 경험의 비율이 유의하게 높았다(<italic>p</italic>&#x3C;0.001). 치은 출혈 경험은 학업성적이 상위권인 그룹이 27.5%로 가장 높고, 중위권 그룹은 14.4%로 가장 낮았다(<italic>p</italic>=0.034). 가계경제 상태가 낮을수록 치은 출혈 경험의 비율이 높았고(<italic>p</italic>=0.003), 흡연 경험이 있는 그룹(30.7%)은 없는 그룹(17.6%)에 비해 치은 출혈 경험의 비율이 높았다(<italic>p</italic>=0.008). 치아 파절 경험의 비율은 중소도시 거주자가 16.3%로 가장 높았고(<italic>p</italic>=0.018), 아버지 학력이 중학교 이하인 그룹(22.6%)에서 가장 높았다. 음주와 흡연 경험이 있는 그룹, 치면열구전색 경험이 있는 그룹에서 치아 파절 경험의 비율이 유의하게 높았다. 치아 시림 증상은 성별로는 여학생(34.2%)이 남학생(26.0%)보다 높고(<italic>p</italic>=0.008), 가계경제 상태가 낮을수록 유의하게 높았으며(<italic>p</italic>=0.041), 치면열구전색 경험이 있는 그룹에서 유의하게 높았다(<italic>p</italic>=0.001).</p>

        <title>4. 구강 증상 경험의 오즈비</title>
        <p>일반적 특성에 따른 구강 증상 경험의 오즈비는 다음과 같다&#x3C;<xref ref-type="table" rid="t004">Table 4</xref>&#x3E;. 치아 통증 경험의 오즈비는 여학생이 1.71(95% CI: 1.24-2.37)배 높고, 학년별로는 고등학교 2학년이 3.41(95% CI: 1.95-5.97) 배로 가장 높았다. 부모의 국적별로는 아버지의 국적이 외국인 그룹을 기준으로 했을 때 어머니가 외국인인 그룹에서 치아 통증 경험의 오즈비가 2.47(95% CI: 1.09-5.60)배 높았으며, 흡연 경험이 있는 그룹이 그렇지 않은 그룹에 비해 2.03(95% CI: 1.20-3.43)배 높았다. 치은 출혈 경험은 가계경제 상태가 가장 낮은 그룹이 5.11(95% CI: 1.80-14.53)배 높았고, 흡연 경험이 있는 그룹이 2.17(95% CI: 1.20-3.90) 배 유의하게 높았으며 치아 파절 경험은 가계경제 상태가 가장 낮은 그룹에서 3.44(95% CI: 1.01-11.70)배 높았다. 치아 시림 증상 경험의 오즈비는 남학생에 비해 여학생이 1.53(95% CI: 1.14-2.04)배 높았고, 아버지       <title>총괄 및 고안</title>
      <p>다문화가정은 국제결혼, 이주노동자 등의 급증으로 다문화가정 청소년의 비율이 매년 증가 추세이며 지속적인 증가가 예상되므로 이들에 대한 관심이 필요하다<xref ref-type="bibr" rid="r019">[19]</xref>. 다문화가정 청소년들은 부모의 사회적&#xB7;문화적 차이 및 경제적인 면에서 취약하며<xref ref-type="bibr" rid="r013">[13</xref>], 정서적&#xB7;신체적으로 성장 발달을 지연시키는 건강 문제를 갖고 있다고 하였다.</p>
      <p>이에 본 연구는 우리나라 다문화가정 청소년의 구강질환 증상에 미치는 요인을 알아보기 위해 제16차 청소년건강행태조사<xref ref-type="bibr" rid="r018">[18]</xref> 자료를 이용하였다.</p>
      <p>구강 증상 경험 중 치아 파절 증상은 어머니 또는 아버지가 외국인인 그룹보다 양쪽 부모가 모두 외국인인 그룹에서 24.6%로 가장 높게 나타났다. 이는 다문화 청소년의 가정에서 구강 관리가 제대로 이루어지지 않고 있다고 판단할 수 있다. 또한, 가계경제 상태가 낮을수록 치아 통증과 치은 출혈 경험의 비율이 높아지는 경향을 보임으로써 경제 상태가 낮은 수준일 때 청소년의 구강 증상 경험이 높다는 Chun과 Lee<xref ref-type="bibr" rid="r020">[20]</xref>, Park과 Jung<xref ref-type="bibr" rid="r014">[14]</xref>의 연구결과와 일치하였다. 다문화가정의 낮은 경제 수준은 청소년의 구강 상태에 좋지 않은 영향을 끼칠 뿐만 아니라 지속적인 구강질환 증상의 경험에도 불구하고 방치되고 있는 것으로 판단된다. 따라서, 다문화가정 청소년에게 적합한 구강 보건교육과 맞춤형 구강 건강서비스를 제공하여 구강 건강을 향상 시킬 수 있어야 할 것이다<xref ref-type="bibr" rid="r021">[21]</xref>.</p>
      <p>Bae<xref ref-type="bibr" rid="r009">[9]</xref>와 Han 등<xref ref-type="bibr" rid="r022">[22]</xref>의 결과처럼 치면열구전색을 경험하지 않은 청소년에 비해 경험한 청소년에게서 치아통증이 높게 나타났고, 치면열구전색을 경험하지 않은 그룹은 경험한 그룹에 비해 치아 통증, 치아 파절, 치아 시림 증상이 감소하는 것으로 분석되었다. 이것은 다문화가정 청소년 대상의 구강 보건사업 수혜가 증가하여 예방 처치 경험이 많은 것에 비해 구강 건강관리 및 교육 부족으로 인해 인과 결과가 반대로 나타난 현상으로 해석된다.</p>
      <p>흡연 경험이 있는 청소년은 없는 그룹에 비해 치은 출혈 경험의 비율이 유의하게 높았고, 오즈비 또한 2.17배 유의하게 높게 나타났다. Park<xref ref-type="bibr" rid="r017">[17]</xref>의 연구에서 흡연과 음주 경험이 있는 청소년은 없는 청소년에 비해 구강 증상 경험 위험이 각각 1.17배와 1.31배 높았고, 흡연 경험이 있는 학생에서 잇몸 출혈률이 높게 나타났다는, Jun과 Kim<xref ref-type="bibr" rid="r023">[23]</xref>의 연구와 일치하는 결과를 보인다. 흡연과 음주의 경험이 있는 청소년에게서 치아 통증이 유의하게 높게 나타난 것은, Lee 등<xref ref-type="bibr" rid="r024">[24]</xref>, Kim과 Han<xref ref-type="bibr" rid="r025">[25]</xref> 및 Kim과 Han<xref ref-type="bibr" rid="r026">[26]</xref>의 결과와 마찬가지로 흡연 및 음주가 청소년의 신체 건강뿐만 아니라 구강질환 발생 및 구강 건강의 위험요인이 되므로 주의 깊은 관리가 필요하다.</p>
      <p>치아 통증 경험의 관련 요인 중 성별에서 여학생의 오즈비가 남학생에 비해 1.71배 높게 나타나 여학생이 남학생에 비해 1.52배 높게 나타났다는 Han 등<xref ref-type="bibr" rid="r022">[22]</xref>의 연구 결과와 유사하였다. 이는 호르몬 및 유전 문화적 영향의 결과로 여성이 남성에 비해 치아우식 발생률이 높고 시기적으로 조기에 맹출하는 제1대구치가 우식의 환경에 빨리 접하게 되어 구강질환 이환율이 증가한다는 연구에 뒷받침된다<xref ref-type="bibr" rid="r027">[27]</xref>. 또한, 아버지가 외국인인 그룹에 비해 어머니가 외국인인 그룹에서 치아 통증 경험의 오즈비가 2.47배 높았다. Lee 등<xref ref-type="bibr" rid="r024">[24]</xref>은 다문화 이주여성의 우식경험 영구치 지수는 우리나라 성인보다 높고, 구강 건강 상태가 열악하다고 하였다. 대부분의 이주여성은 필리핀, 베트남 등 주로 개발도상 국가에서 이주해오는 경우가 많아 구강 건강의 중요성에 대한 인식이 낮고, 주 양육자가 여성인 우리나라와 아시아권의 문화적 특성으로 봤을 때 자녀의 구강 건강에 관한 관심은 아버지보다는 어머니의 태도와 인식이 많이 개입되었을 가능성이 있다. 아버지의 학력이 중학교 이하로 가장 낮은 그룹에서 치아 시림 증상의 오즈비가 유의하게 높았다. Kim 등<xref ref-type="bibr" rid="r028">[28]</xref>은 아버지, 어머니의 교육 수준이 낮을수록 우식경험영구치수가 많고 본인의 구강 건강 상태가 매우 나쁘다고 인지한다고 보고하여 본 연구와 유사한 결과를 보였다. 흡연 경험이 있는 그룹이 그렇지 않은 그룹에 비해 치아 통증 경험 및 치은 출혈 경험의 오즈비 모두 유의하게 높고, 가계경제 상태가 가장 낮은 그룹에서 치은 출혈 경험의 오즈비가 유의하게 높았다. 이러한 결과는 흡연 경험이 있는 청소년이 없는 청소년에 비해 주관적 구강 증상 경험이 1.17배 유의하게 높고, 주관적 경제 상태가 하인 그룹에서 1.71배 높다고 보고한 Park <xref ref-type="bibr" rid="r017">[17]</xref>의 연구와 유사하다.</p>
      <p>본 연구에 활용된 자료로는 시간적 선후 관계를 확인할 수 없고 명확한 진단 결과로 받아들이기에는 다소 제한점을 가진다. 따라서, 추후 연구에서는 직접 진단검사 결과를 이용한 임상적 평가가 뒷받침되는 후속 연구가 지속되어 다문화가정 청소년의 구강 건강 프로그램 개발 및 복지정책사업의 기초자료로 활용되기를 바란다.</p>
    </sec>
    <sec id="s5" sec-type="conclusions">
      <title>결론</title>
      <p>본 연구는 제16차 청소년건강행태조사 원시자료를 이용하여 다문화가정 청소년의 구강 증상 경험에 영향을 미치는 요인을 분석하였으며 결론은 다음과 같다.</p>
      <p>1. 구강 증상 경험 중 치아 파절 증상은 양쪽 부모가 모두 외국인인 그룹에서 가장 높고, 어머니가 외국인인 그룹에서는 가장 낮았다.</p>
      <p>2. 치아 통증 경험의 오즈비는 여학생이 1.71배 높고, 고등학교 2학년이 3.41배로 가장 높았다. 부모의 국적별로는 어머니가 외국인인 그룹에서 치아 통증 경험의 오즈비가 2.47배 높았으며, 흡연 경험이 있는 그룹은 없는 그룹에 비해 2.03배 높았다.</p>
      <p>3. 치은 출혈 경험의 오즈비는 가계경제 상태가 가장 낮은 그룹이 5.11배, 흡연 경험이 있는 그룹이 2.17배 높았다. 치아 파절 경험은 가계경제 상태가 가장 낮은 그룹에서 3.44배 높았고, 치아 시림 증상 경험의 오즈비는 여학생이 1.53배, 아버지 학력이 중학교 이하인 그룹에서 1.79배 유의하게 높았다.</p>
      <p>이상의 결과로, 다문화가정 청소년은 사회적·경제적 문제로 취약한 구강 증상을 경험하고 있으므로 청소년의 구강 건강증진을 위해 맞춤형 구강 보건교육 프로그램을 구축하고 적절한 지원이 이루어져야 할 것이다.</p>
