        <sec>
          <title>Objectives:</title>
          <p>This study aimed to analyze research literature, created over the past 10 years, on occupational stress among domestic dental hygienists and identify the major factors affecting occupational stress.</p>
        </sec>
        <sec>
          <title>Methods:</title>
          <p>Of the 192 articles found through literature searches, 26 were selected for the analysis after reviewing titles, abstracts, and full texts. General characteristics of each study, measurement tools employed, and data related to pertinent factors were systematically analyzed.</p>
        </sec>
        <sec>
          <title>Results:</title>
          <p>Seven instruments were utilized to measure job stress. Among the variables exhibiting positive correlation with occupational stress, turnover intention was the most prevalent, while variables showing negative correlation were predominantly associated with job satisfaction. Factors influencing occupational stress included emotional labor, workload, organizational social capital, and experiencing verbal abuse.</p>
        </sec>
        <sec>
          <title>Conclusions:</title>
          <p>With regard to the most influential factors, &#x201C;emotional labor&#x201D; and &#x201C;turnover intention&#x201D; had the greatest impacts on job stress and occupational stress, respectively. Based on the findings of this study, follow-up research needs to be conducted to suggest ways to prevent, manage, and reduce occupational stress among dental hygienists</p>

      <title>서론</title>
      <p>치과 진료의 패러다임이 치료 중심에서 포괄적인 치위생 관리로 변화되고 있어 치과위생사의 업무 수행에 대한 역할의 전문성과 다양화가 더욱 요구되고 있다<xref ref-type="bibr" rid="r001">[1]</xref>. 병원조직은 전문적인 의료서비스를 환자들에게 제공하기 위하여 다양한 의료 종사자들이 공조하는 업무적 특성을 가지므로 직무 스트레스가 다른 조직보다 비교적 높다<xref ref-type="bibr" rid="r002">[2]</xref>. 또한, 직무스트레스로 인한 이직률과 퇴직률도 높은 실정이다<xref ref-type="bibr" rid="r003">[3]</xref>.</p>
      <p>직무스트레스란 직장에서 잠재적인 긴장 상태를 불러올 수 있는 스트레스의 요인이 모두 포함되는 개념으로, 직장의 업무 수행 요구가 구성원의 업무 능력을 넘어갈 때 발생한다<xref ref-type="bibr" rid="r004">[4]</xref>. 직장은 현대를 살아가는 사람들에게 경제적으로 중요한 의미일 뿐 아니라, 개인의 정체성 측면에서도 의미가 있다. 치과위생사의 직무스트레스에 관한 국내 선행연구는 일반적 특성<xref ref-type="bibr" rid="r005">[5]</xref>, 임상경력<xref ref-type="bibr" rid="r006">[6]</xref>, 우울 증상<xref ref-type="bibr" rid="r007">[7]</xref>, 근무 현황에 따른 차이<xref ref-type="bibr" rid="r008">[8]</xref>를 알아본 연구가 있었다. Han 등<xref ref-type="bibr" rid="r008">[8]</xref>의 연구에서는 임상경력이 낮은 3년 이하인 경우 직무 자율성에 대한 스트레스가 많았고, 7년 차 이상인 경우 관계갈등, 보상부적절 스트레스가 많았다. 직급에 따라 팀원인 경우 직무자율, 직장문화 스트레스가 높았고, 팀장인 경우 관계갈등 스트레스가 높았다. 따라서 직무스트레스의 하위요인은 치과위생사의 임상경력과 직급에 따라 차이가 있는 것으로 나타났다. 일반적 특성 외에 다른 변수와의 관계를 알아본 연구에서는 감정노동<xref ref-type="bibr" rid="r009">[9]</xref>, 업무량<xref ref-type="bibr" rid="r010">[10]</xref>, 언어폭력<xref ref-type="bibr" rid="r011">[11]</xref>이 영향을 미치는 것으로 나타났고, 이직의도<xref ref-type="bibr" rid="r012">[12]</xref>, 직무만족<xref ref-type="bibr" rid="r013">[13]</xref>, 직무소진<xref ref-type="bibr" rid="r006">[6]</xref>, 조직몰입<xref ref-type="bibr" rid="r007">[7]</xref>에 영향을 미치는 것으로 나타났다.</p>
      <p>간호학에서는 2014년부터 2018년까지 경력직 간호사의 직무스트레스와 중재 프로그램의 효과를 문헌 분석하여 임상에서 직무스트레스를 감소시킬 수 있는 활용 가능한 프로그램을 개발하기 위해 노력하고 있다<xref ref-type="bibr" rid="r014">[14]</xref>. 또한, 국내 작업치료사를 대상으로 한 문헌 분석은 2000년부터 2011년까지 사용된 측정 도구와 주요 요인을 분석하였으며 조직 관련 요인이 직무스트레스에 가장 큰 영향을 미치는 것으로 나타났다<xref ref-type="bibr" rid="r015">[15]</xref>. Lim과 Park<xref ref-type="bibr" rid="r016">[16]</xref>은 체계적 문헌고찰을 통해 치과위생사의 직무만족과 관련된 요인을 파악하고자 하였다. 그러나 치위생학 분야에서 직무만족과 대비되는 개념인 직무스트레스에 대해서는 체계적인 분석이 시행되지 않았다. 1998년부터 치과위생사를 대상으로 한 직무스트레스와 관련된 연구가 진행되었지만, 연구마다 측정도구, 표본 수, 측정 변수에 대한 차이가 있고 관련 변수를 포괄적으로 측정하지 않았기 때문에 결정적인 관련 변수에 대한 결론을 내리기 어렵다.</p>
      <p>따라서 본 연구 목적은 지난 10년간 국내에 출판된 치과위생사의 직무스트레스에 관한 문헌을 분석해 측정 도구 및 주요 요인을 파악하고, 치과위생사의 직무스트레스를 감소시키기 위한 방법을 모색하는 연구에 기초 자료를 제공하고자 한다.</p>
    </sec>
    <sec id="s2" sec-type="other">
      <title>본론</title>
      <sec id="s2a">
        <title>1. 연구설계</title>
        <p>본 연구는 치과위생사의 직무스트레스에 관한 문헌을 수집하여 측정도구와 영향요인을 분석하는 체계적 문헌고찰 연구이다.</p>
        <sec id="s2aa">
          <title>1) 문헌검색</title>
          <p>본 연구는 PRISMA (Preferred Reporting Items for Systematic Reviews and Meta-analysis)의 국문판 PRISMA에 따라 국내 연구를 분석하였다. 2023년 9월 17일부터 2023년 9월 23일까지 문헌 검색이 이루어졌으며 2013년부터 2023년 8월까지 국내 학술지에 게재된 연구를 대상으로 하였다. 데이터베이스는 학술정보원 (RISS), 과학기술통합서비스(NDSL), 학술논문지식베이스(DBPIA), 한국학술정보(KISS)를 이용하였고다. 검색어는 &#x2018;치과위생사&#x2019;, &#x2018;스트레스&#x2019;, &#x2019;직무스트레스&#x2019;로 제목과 전문에 검색어가 포함된 문헌을 수집하였다.</p>
        </sec>
        <sec id="s2ab">
          <title>2) 문헌 선정과 배제 기준</title>
          <p>NDSL에서 69편, DBPIA에서 76편, RISS에서 9편, KISS에서 38편의 문헌이 검색되었다. 연구자 3인이 검색된 자료를 검토하여 학술지 중복으로 113편을 1차 제외하였다. 선별 후 회의를 통하여 문헌을 선정하였다. 제목과 초록 내용을 검토하여 연구 대상자가 치과위생사가 아닌 경우와 본 연구의 주제와 벗어난 문헌 35편, 학위논문 16편을 2차로 제외하였다. 원문을 볼 수 없는 문헌 2편을 3차로 제외하였다. 최종 26편의 문헌이 선정되었다&#x3C;<xref ref-type="fig" rid="f001">Fig. 1</xref>&#x3E;.</p>

          <title>3) 자료분석</title>
          <p>최종 선정된 문헌을 분석하기 위하여 구조화된 양식을 통해 분석을 시행하였다. 해당 양식은 저자, 제목, 출판 연도, 표본 수, G*power 사용, 연구 방법, 연구 목적에 대한 일반적 특성과 직무스트레스 측정도구 출처, 문항 수, 척도, 하위요인, 관련 요인 자료를 추출할 수 있도록 구성하였다. 각 문헌을 연구자들이 분석한 후 토론을 거쳐 내용을 최종 결정하였다.</p>
        </sec>
      </sec>
      <sec id="s2b">
        <title>2. 문헌의 일반적 특성</title>

        <title>3. 측정도구 분석</title>
        <p>본 연구의 분석 대상 문헌 26편에서 사용된 도구는 &#x3C;<xref ref-type="table" rid="t002">Table 2</xref>&#x3E;와 같다. 원 도구는 총 7개로 나타났다. Chang 등<xref ref-type="bibr" rid="r017">[17]</xref>의 도구는 산업안전보건연구원의 한국형 직무 스트레스 요인의 내용을 기반으로 개발되었으며, 원 측정도구는 43문항이고, 단축형은 24문항으로 구성되었다<xref ref-type="bibr" rid="r017">[17]</xref>.         <p>Ivancevich와 Matterson<xref ref-type="bibr" rid="r028">[28]</xref>의 도구는 2편의 문헌에서 사용되었다. 이 도구는 기업에 근무하는 직장인의 스트레스를 측정하기 위하여 개발된 도구로 21문항으로 구성되어 있으며, 어떤 저자가 번역했는지 확인할 수 없었지만 Cho와 Moon<xref ref-type="bibr" rid="r029">[29]</xref>, Kwon 등<xref ref-type="bibr" rid="r030">[30]</xref>이 수정하여 사용하였다. Kim과 Gu<xref ref-type="bibr" rid="r031">[31]</xref>의 도구는 근무 중에 간호사가 경험하는 스트레스를 측정하기 위하여 개발되었으며, 3편의 문헌에서 사용되었다. Bai<xref ref-type="bibr" rid="r032">[32]</xref>의 도구는 정신과 간호사의 업무 스트레스 측정하기 위하여 개발되었으며, 이 도구를 수정하여 사용한 문헌은 2편으로 나타났다. Lee와 Park<xref ref-type="bibr" rid="r033">[33]</xref>의 문헌은 직무 스트레스 척도 제작에 관한 연구로 남성 사무직 근로자만을 대상으로 하였으며, 이 문헌의 도구를 수정하여 사용한 문헌은 3편이었다. Carlson 등<xref ref-type="bibr" rid="r034">[34]</xref>, Lee 등<xref ref-type="bibr" rid="r035">[35]</xref>의 측정도구를 각각 수정하여 사용한 문헌이 있었다. Carlson 등<xref ref-type="bibr" rid="r034">[34]</xref>의 도구는 일-가정 갈등을 측정하기 위해 개발된 도구로 Lu 등<xref ref-type="bibr" rid="r036">[36]</xref>의 문헌에서 의사의 직무스트레스를 측정하기 위해 수정한 것을 Han과 Cho<xref ref-type="bibr" rid="r037">[37]</xref>가 치과위생사의 직무스트레스를 측정하기 위해 수정하여 사용하였다. Lee 등<xref ref-type="bibr" rid="r035">[35]</xref>의 도구는 남녀 7개 직종 군의 직업으로 직무 스트레스 평가를 위하여 개발된 도구로 Lee등<xref ref-type="bibr" rid="r011">[11]</xref>이 수정하여 사용하였다.</p>
      </sec>
      <sec id="s2d">
        <title>4. 직무스트레스 관련 변수</title>

        <p>직무와 관련된 변수는 이직의도, 직무소진에서 정적(+)상관관계가 나타났으며<xref ref-type="bibr" rid="r006">[6</xref>,<xref ref-type="bibr" rid="r018">18</xref>,<xref ref-type="bibr" rid="r038">38</xref>-<xref ref-type="bibr" rid="r040">40]</xref>, 직무 만족, 조직 몰입, 조직 커뮤니케이션, 일터 영성, 일-가정 균형에서 부적(-)상관관계가 나타났다<xref ref-type="bibr" rid="r006">[6</xref>,<xref ref-type="bibr" rid="r018">18</xref>,<xref ref-type="bibr" rid="r025">25</xref>,<xref ref-type="bibr" rid="r027">27</xref>,<xref ref-type="bibr" rid="r029">29</xref>,<xref ref-type="bibr" rid="r038">38</xref>-<xref ref-type="bibr" rid="r040">40]</xref>.</p>
        <p>신체적 건강과 관련된 변수는 피로 증상, 수면, 상지 기능에서 정적(+)상관관계를 나타냈으며<xref ref-type="bibr" rid="r018">[18</xref>,<xref ref-type="bibr" rid="r020">20</xref>,<xref ref-type="bibr" rid="r030">30]</xref>. 건강상태에서 부적(-)상관관계가 나타났다<xref ref-type="bibr" rid="r023">[23]</xref>.</p>
        <p>심리 및 감정과 관련된 변수는 감정노동, 언어폭력에서 정적(+)상관관계를 나타냈으며<xref ref-type="bibr" rid="r011">[11</xref>,<xref ref-type="bibr" rid="r019">19</xref>,<xref ref-type="bibr" rid="r020">20</xref>,<xref ref-type="bibr" rid="r023">23]</xref>, 낙관성, 자기효능감, 자아존중감에서 부적(-)상관관계가 나타났다<xref ref-type="bibr" rid="r019">[19</xref>,<xref ref-type="bibr" rid="r029">29]</xref>.</p>
      </sec>
      <sec id="s2e">
        <title>5. 직무스트레스가 영향을 미치는 변수</title>
        <p>각 문헌에서 확인된 직무스트레스가 영향을 미치는 관련 변수는 &#x3C;<xref ref-type="table" rid="t004">Table 4</xref>&#x3E;와 같다. 직무스트레스가 이직의도, 직무 만족, 직무 소진, 조직 몰입, 일-가정 균형, 삶의 질, 신체적, 심리 사회적, 건강 상태 요인에 유의한 영향을 미쳤다<xref ref-type="bibr" rid="r006">[6</xref>,<xref ref-type="bibr" rid="r007">7</xref>,<xref ref-type="bibr" rid="r009">9</xref>,<xref ref-type="bibr" rid="r010">10</xref>,<xref ref-type="bibr" rid="r012">12</xref>,<xref ref-type="bibr" rid="r013">13</xref>,<xref ref-type="bibr" rid="r018">18</xref>,<xref ref-type="bibr" rid="r022">22</xref>,<xref ref-type="bibr" rid="r027">27</xref>,<xref ref-type="bibr" rid="r030">30</xref>,<xref ref-type="bibr" rid="r037">37</xref>-<xref ref-type="bibr" rid="r040">40]</xref>. 그중 직무 스트레스가 영향을 미친 요인으로 이직의도가 9편으로 가장 많았다. 그리고 직무스트레스가 높을수록 직무만족, 일-가정 균형 요인은 낮은 것으로 나타났다<xref ref-type="bibr" rid="r027">[27</xref>,<xref ref-type="bibr" rid="r037">37]</xref>. 직무스트레스의 하위요인으로는 직무 요구, 직무 자율성 결여, 관계 갈등, 조직체계, 보상 부적절, 직무 불안정, 직장문화, 직업의 장래성, 직장 내 대인관계, 감정 상태,         <p>직무스트레스의 하위요인인 직무 요구, 직무 자율성 결여, 관계갈등, 조직체계, 보상 부적절, 직장문화, 직업의 장래성, 직장 내 대인관계, 감정 상태, 역할 모호성, 역할갈등 11가지 요인이 모두 높았던 경우에 관련 변수 중 직무만족 요인이 낮은 것으로 나타났다<xref ref-type="bibr" rid="r005">[5</xref>,<xref ref-type="bibr" rid="r006">6</xref>,<xref ref-type="bibr" rid="r012">12</xref>,<xref ref-type="bibr" rid="r038">38</xref>,<xref ref-type="bibr" rid="r041">41]</xref>. 하위요인관계 갈등, 조직체계, 직장문화, 조직 불안정이 높은 경우 변수 사회적 건강 상태가 낮은 것으로, 직무 자율성 결여, 직장문화, 조직 불안정이 높을 경우 정신적 건강 상태가 낮은 것으로, 직장문화가 높은 경우 신체적 건강 상태가 낮은 것으로 나타났다<xref ref-type="bibr" rid="r023">[23]</xref>. 자기 계발 스트레스, 직무만족도 스트레스, 업무상 스트레스가 높을 경우 삶의 질이 낮은 것으로 나타났고<xref ref-type="bibr" rid="r026">[26]</xref>, 직장문화가 높을 경우 심리 사회적 스트레스도 높은 것으로 나타났다<xref ref-type="bibr" rid="r021">[21]</xref>. 역할 모호성과 역할갈등이 높을 경우 이직 의도도 높은 것으로 나타났고, 역할 모호성이 높을 경우 조직몰입이 낮은 것으로 나타났다<xref ref-type="bibr" rid="r006">[6]</xref>.</p>

        <title>6. 직무스트레스에 영향을 미치는 변수</title>

        <title>7. 문헌의 질 평가</title>
        <p>문헌의 질 평가를 실시하기 위해 Cummings와 Estabrooks<xref ref-type="bibr" rid="r042">[42]</xref>의 연구에서 사용된 &#x2018;Quality assessment and validity tool for correlational studies&#x2019;를 사용하였다. 질 평가 도구는 총 13개 문항으로 연구 설계 문항 1개, 표본과 관련된 문항 5개, 측정과 관련된 문항 5개, 통계분석과 관련된 문항 2개로 구성되었다. 각 문항에 관한 질문에 &#x2018;예&#x2019;라고 응답할 경우 1점, &#x2018;아니오&#x2019;인 경우 0점으로 처리하여 총 0-13점까지의 점수 범위를 가진다. 총점에 대한 범주는 3개로 구분하였고 10-13점은 문헌의 질이 &#x2018;높음(High)&#x2019;, 5-9점은 &#x2018;중간(Medium)&#x2019;, 0-4점은 &#x2018;낮음(Low)&#x2019;으로 평가하였다.</p>

      <title>결론</title>
      <p>본 연구는 치과위생사의 직무스트레스에 대한 2013년부터 2023년까지의 선행연구를 분석하여 직무스트레스의 연구에 기초자료를 제공하고 직무스트레스를 감소시킬 수 있는 방안을 마련하는 데 도움이 되고자 시행되었다.</p>
      <p>분석에 포함된 문헌 26편으로 모두 설문조사를 통한 양적연구였다. 양적연구는 통계분석을 통해 일반화할 수 있는 결과를 산출할 수 있지만 설문조사로 측정 가능한 변수만 파악할 수 있기 때문에 직무스트레스와 관련된 변수를 파악하는 데 제한이 있다고 생각된다.</p>
      <p>설문조사연구에서 연구 결과의 검정력과 신뢰도를 높이기 위해서는 통계분석방법에 따라 적절한 표본 수를 결정하는 것이 중요하다<xref ref-type="bibr" rid="r043">[43]</xref>. G*power는 통계분석방법에 따라 표본 수를 산출하고 검정력을 계산해주기 때문에 근거 있는 표본 수 산출이 가능하다<xref ref-type="bibr" rid="r044">[44]</xref>. 그러나 8편의 문헌에서만 G*power를 사용하였고, 나머지 문헌에서는 표본 수 산출에 대한 적절한 근거가 제시되지 않았다. 따라서 향후 직무스트레스와 관련된 연구에서는 표본 수 산출에 대한 적절한 근거 제시가 필요하다.</p>
      <p>26편의 문헌 분석 결과 직무스트레스를 측정하기 위해 7개의 측정도구가 사용되었는데, 7개의 설문 도구 Likert 척도는 4점 척도 1개, 5점 척도 4개, 6점 척도 2개로 26편의 문헌에서는 4점 척도의 설문 도구를 사용한 문헌이 가장 많았다. 4점 척도는 중립적인 문항이 없기 때문에 직무스트레스 정도가 높고, 낮음을 확실하게 측정되었다고 생각된다. 원 문헌들을 분석한 결과 Chang 등<xref ref-type="bibr" rid="r017">[17]</xref>이 개발한 측정도구를 주로 사용하였다. 그러나 그중 몇 개의 문헌은 Chang 등<xref ref-type="bibr" rid="r017">[17]</xref>의 도구를 그대로 사용했는지, 수정의 과정과 근거의 여부를 구체적으로 알 수 없었다. 타당성의 유무 또한 확인할 수가 없었기 때문에 측정도구를 인용하는 과정에서 실수가 발생하거나 원 도구의 내용이 훼손된 것으로 사료되어 문헌을 수집하는 과정에서 어려움이 있었다. 따라서 연구자들은 도구를 인용할 때 출처를 확인하고, 연구 주제와 적합한 도구가 맞는지 확인하는 과정이 필요하다고 생각된다. 그리고 모든 연구의 도구에서 치과위생사가 아닌 직군만을 대상으로 하여 적용 범위가 제한적이라고 볼 수 있다. 따라서 치과위생사의 현재 근무환경과 적합한 직무스트레스 측정도구의 개발이 필요하다.</p>
      <p>직무스트레스와 상관관계가 있는 변수는 이직의도와 직무만족이 가장 많았는데, 이직의도와 정적(+)인 상관관계가 나타났고, 직무만족과 부적(-)인 상관관계가 나타났다. 직무스트레스가 영향을 미친 요인으로는 이직의도가 가장 많았다. 직무스트레스가 높은 경우 신체적 건강<xref ref-type="bibr" rid="r013">[13</xref>,<xref ref-type="bibr" rid="r014">14</xref>,<xref ref-type="bibr" rid="r018">18</xref>,<xref ref-type="bibr" rid="r030">30]</xref>이나 이직의도에 부적인 영향을 미쳤고, 직무스트레스를 관리할수록 삶의 질이 높게 나타났다<xref ref-type="bibr" rid="r026">[26]</xref>. 간호사를 대상으로 한 연구 중 Kwak과 Hyun<xref ref-type="bibr" rid="r015">[15]</xref>의 연구에서도 직무스트레스가 낮을수록 직무만족이 높아지고, 직무스트레스가 높을수록 이직의도가 높은 것으로 나타났다. Lee와 Lee<xref ref-type="bibr" rid="r045">[45]</xref>의 연구에서도 간호사의 직무스트레스와 직무만족이 부적(-) 상관관계로 나타났다. 따라서 치과위생사의 이직률이 높은 것은 다른 직업군보다 직무스트레스가 높다는 것으로 생각되며, 직무스트레스를 감소시키기 위한 노력은 궁극적으로 직무만족을 높여주고 이직의도를 감소시키는 데 기여할 수 있다고 생각된다.</p>
      <p>직무스트레스에 영향을 미친 요인으로는 감정노동, 업무량, 언어폭력 경험 등으로 나타났으며, 이중 감정노동이 가장 많았다. 감정노동이 많을수록 직무스트레스는 증가하였는데<xref ref-type="bibr" rid="r010">[10</xref>,<xref ref-type="bibr" rid="r011">11</xref>,<xref ref-type="bibr" rid="r019">19]</xref>, 감정노동의 하위요인까지 살펴본 논문에서는 체험정서가 높을수록 직무스트레스는 감소하였으며, 표현정서가 높을수록 직무스트레스는 증가하였다<xref ref-type="bibr" rid="r020">[20]</xref>. Kim과 Kang<xref ref-type="bibr" rid="r046">[46]</xref>의 연구에서는 감정노동이 직무스트레스에 영향을 미치는 것으로 나타났으며, 간호사를 대상으로 한 연구 중 Joung과 Park<xref ref-type="bibr" rid="r047">[47]</xref>의 연구에서도 언어폭력 경험이 직무스트레스를 높이는 것으로 나타났다. 치과위생사는 임상에서 다양한 의료 종사자와 협력하며 환자에게 전문적인 의료서비스를 제공해야 하는 직업적 특성으로 인해 감정노동이 많고 이에 따른 직무 스트레스가 높은 것으로 생각된다.</p>
      <p>본 연구는 치과위생사를 대상으로 직무스트레스에 관한 연구 중 지난 10년간 연구만 문헌 분석을 했으며, 2013년 전에 연구된 문헌에 대해서는 직무스트레스와 관련된 요인을 모두 고찰하지 못한 한계점이 있다. 또한 국내 논문을 대상으로 문헌고찰을 했기 때문에 국외에서 이루어진 연구는 분석에 포함되어 있지 않다. 따라서 본 연구를 기초로 치과위생사의 직무스트레스에 대한 분석이 보다 포괄적으로 진행된 연구가 필요할 것으로 생각된다. 이러한 한계점에도 불구하고 국내 치과위생사의 직무스트레스와 관련된 요인을 파악하고 관련 문헌을 체계적으로 분석했다는 것에 의의가 있다.</p>
