Objective: This study aims to understand the effect of client-centric occupational therapy on the activity participation in subjects with a special dementia ratings, and to provide a model of an activity-centric program for such subjects. Methods: Three subjects were classified as having a special dementia ratings. Their ADL and levels of activities were investigated using a Modified Barthel Index, the Korean Instrumental Activities of Daily Living, and the Korean Activity Card Sort. The goals of their interventions were established using the Canadian Occupational Performance Measure. The activities selected were classified into 20 steps. The changes in activity implementation (correct response to the steps) during the intervention period were presented visually through a graph. Results: Their activity level scores were increased, and the task implementation rate, implementation level, and satisfaction level of the intervention activity were improved. Conclusion: The activity centric program used to apply client-centric occupational therapy suggested in this study can provide a practical model of occupational therapy for subjects with a special grade of dementia, and can contribute to securing the professionalism of occupational therapy in the field of dementia services.
