ABSTRACT

Objectives This study aimed to develop a resistance exercise program for cancer survivors based on Sasang constitutional medicine. The program was designed to be tailored to the physiological and psychological characteristics of each Sasang constitution, supporting muscle strength recovery, fatigue reduction, and quality of life improvement after cancer treatment. Methods A literature review was conducted to identify exercise components and physiological mechanisms relevant to resistance training for cancer survivors. Evidence from studies on cancer rehabilitation and Sasang constitutional characteristics was integrated to construct a draft program. The preliminary structure included common resistance movements and constitution-specific modifications. A two-round expert review was performed with five specialists in Sasang medicine, rehabilitation medicine, and exercise physiology to evaluate the program’s validity and feasibility. Consensus analysis was conducted using central tendency, convergence, and concordance indicators. Results Expert evaluation demonstrated a high level of agreement (median scores ≥ 7.0) and acceptable consistency across all domains (concordance ≥ 0.8; convergence ≤ 1.0). Based on expert feedback, the exercise sequence, load intensity, and constitution-specific adaptations were refined. The finalized program consists of resistance exercise protocols considering the musculoskeletal balance, fatigue tolerance, and metabolic patterns unique to each Sasang constitution. Conclusions The Sasang Constitution-based resistance exercise program developed in this study provides a feasible and clinically applicable guideline for cancer survivors. By integrating constitutional theory with resistance exercise principles, this study presents a new model for personalized rehabilitation within Korean medicine.
