Analysis of Stage-specific Dental Contact Ratios during Endotracheal Intubation and Perceived Usefulness of Visual Feedback-based Training

ABSTRACT

Dental contact during endotracheal intubation is an important procedural event that may lead to dental trauma, including tooth fracture, luxation, and periodontal injury. Although dental trauma is one of the common complications associated with laryngoscopy and tracheal intubation, operators may fail to recognize actual dental contact during airway management training. This study aimed to analyze stage-specific dental contact ratios during endotracheal intubation and to evaluate participants' perceived usefulness of visual feedback-based training using a dental contact sensor. A total of 46 participants were included in the final analysis. Dental contact ratios were calculated by dividing the dental contact time at each intubation stage by the duration of the corresponding stage. Data were analyzed using descriptive statistics, chi-square tests, binary logistic regression analysis, Wilcoxon signed-rank tests, Cochran's Q tests, and Friedman tests using SPSS Statistics version 27.0. The Wilcoxon signed-rank test showed a significant difference between perceived contact time and actual total dental contact time (Z=-3.604, p<.001). Cochran's Q test showed significant differences in dental contact occurrence across intubation stages (Q=57.384, p<.001). The Friedman test also showed significant differences in stage-specific dental contact ratios (χ²=78.045, p<.001). The highest mean contact ratio was observed during the glottic exposure stage (57.2%), followed by tube insertion (53.6%) and 45-degree angle adjustment (41.8%). These findings suggest that dental contact occurs predominantly during specific procedural stages and that visual feedback-based training may be useful for improving dental contact awareness and preventing dental injury during airway management education.
