AB - PURPOSE: This study investigated the association between diabetes mellitus and DXA-based sarcopenia among older Korean adults using the data from the 2024 Korea National Health and Nutrition Examination Survey (KNHANES).
METHODS: This cross-sectional study included 1,427 adults aged ≥ 65 years from KNHANES 2024. Sarcopenia was defined as the coexistence of low muscle mass and low handgrip strength according to the Asian Working Group for Sarcopenia criteria. The diabetes status was categorized into normal, prediabetes, and diabetes groups based on fasting glucose, glycated hemoglobin, medication use, insulin therapy, and physician diagnosis. Complex sample logistic regression analyses were performed using the sampling weights, strata, and clusters. Adjusted odds ratios (ORs) and 95% confidence intervals (CIs) were calculated.
RESULTS: The prevalence of sarcopenia was 11.96%, 8.11%, and 15.14% in the normal group, prediabetes group, and diabetes group, respectively. In the crude model, prediabetes was inversely associated with sarcopenia (OR = .650, 95% CI: .432–.978), whereas diabetes showed no significant association. After adjusting for sociodemographic variables, the inverse association remained significant (OR = .618, 95% CI: .405–.943). After additional adjustment for BMI and health behaviors, neither prediabetes nor diabetes was significantly associated with sarcopenia.
CONCLUSION: The diabetes status was not independently associated with DXA-based sarcopenia after adjustment for obesity and lifestyle-related factors in older Korean adults.
