ABSTRACT

PURPOSE: This study compared the effects of robotic spinal decompression therapy and lumbar stabilization exercise on pain, functional disability, lumbar range of motion, and structural changes in patients with lumbar disc herniation. METHODS: Thirty-four patients diagnosed with lumbar disc herniation were assigned randomly to a robotic decompression group (n = 17) or a lumbar stabilization exercise group (n = 17). Both groups received 30-minute interventions three times per week for eight weeks. The outcome measures included the pain intensity using the visual analog scale (VAS), disability using the Korean version of the Oswestry Disability Index (KODI), lumbar range of motion, and disc protrusion measured on sagittal magnetic resonance images as the distance between the posterior vertebral body line and the tip of the herniated disc. The MRI measurements were performed twice by a blinded orthopedic specialist, and the mean value was used for analysis. RESULTS: Both groups showed significant improvements in pain, functional disability, and lumbar range of motion (p < .05). Lumbar extension range of motion showed significantly greater improvement in the stabilization exercise group than in the robotic decompression group (p < .05). On the other hand, no significant between-group differences were observed in most outcome variables (p > .05). The robotic decompression group showed a significantly greater decrease in disc protrusion than the stabilization exercise group (p < .05). CONCLUSION: Robotic spinal decompression therapy and lumbar stabilization exercise are effective non-surgical interventions for improving pain and functional recovery in patients with lumbar disc herniation.
