ABSTRACT

Cross-institutional medical image exchange is fundamental to patient-centered care, but transfer histories of sensitive images must be retained in a trustworthy form. This paper presents VADREX (Verifiable Audit for Revocation-Aware DICOM Exchange), an audit protocol and implementation framework that lets patients verify the finality of an anchored consent-revocation entry within the checked log history. VADREX combines append-only logs and log-backed maps, and uses cross-referenced entries and signatures to resolve disputes when the required records are anchored. It does not authenticate the revocation-request time or prove the absence of unlogged physical transfers. In a two-institution DICOMweb evaluation, proof size less than doubled as the log grew from 10³ to 10⁶ entries, individual proof verification took a few milliseconds, and all 90 supplementary evaluation outcomes across ten repetitions matched expectations.
