ABSTRACT

This study analysis privatization of the health service system in Sweden by perspective of historical Institutionalism. In recent, despite of privatization, Sweden continues to keep the Beveridge type of health care system which bases on tax and universalism. In other word, Swedish health care has sustainable and change at the same time. In historical Institutionalism, this is explained by both path dependency and gradual path evolution. In term of path dependency, Sweden built a monopoly integrated mode which has strong nationalization embedded deeply in ideology of social democratic welfare system, This has affected the choice of policy. Although health care reform have performed during 30 years, the strategy of privatization could not change the fundamental system. Just it is alternative of government failure characterized on inefficiency, long waiting time and hieratical administration and so on. In the result, Swedish health care reform is partial privatization not perfect. In term of gradual path evolution, the health care system have been change slowly within boundary of social democratic system. in mid 1980s~ 1990s, Sweden introduced the purchaser·provider split mode1 which aims to increase efficiency and fiscal austerity by privatization. In 2000s, the purchaser·provider split mode1 change to the free choice model which focuses on competition, patient choice and local government autonomy.
